Abstract
A new supplier for aluminum die castings was being evaluated, and the castings failed to meet the durability test requirements. Specifically, the fatigue life of the castings was low. Initial inspection of the fatigue fracture surfaces revealed large-scale porosity visible to the naked eye. New castings with reduced porosity also failed the durability tests. The fatigue fracture surfaces of additional casting fragments were very rough and contained multiple ratchet marks along the inner fillet. These observations indicated the fatigue process was heavily influenced by the presence of surface imperfections. Improving the surface finish or choosing a stronger alloy, were more likely to improve part durability than reducing the porosity.

Abstract
Gears can fail in many different ways, and except for an increase in noise level and vibration, there is often no indication of difficulty until total failure occurs. This article reviews the major types of gears and the basic principles of gear-tooth contact. It discusses the loading conditions and stresses that effect gear strength and durability. The article provides information on different gear materials, the common types and causes of gear failures, and the procedures employed to analyze them. Finally, it presents a chosen few examples to illustrate a systematic approach to the failure examination.

Abstract
This paper presents a failure analysis of a reverse shaft in the transmission system of an all-terrain vehicle (ATV). The reverse shaft with splines fractured into two pieces during operation. Visual examination of the fractured surface clearly showed cracks initiated from the roots of spline teeth. To find out the cause of fracture of the shaft, a finite element analysis was carried out to predict the stress state of the shaft under steady loading and shock loading, respectively. The steady loading was produced under normal operation, while the shock loading could be generated by an abrupt change of operation such as start-up or sudden braking during working. Results of stress analysis reveal that the highest stressed area coincided with the fractured regions of the failed shaft. The maximum stress predicted under shock loading exceeded the yield strength and was believed to be the stimulant for crack initiation and propagation at this weak region. The failure analysis thus showed that the premature fatigue fracture of the shaft was caused by abnormal operation. Finally, some suggestions to enhance service durability of the transmission system of ATV are discussed.

Critical heat exchanger components are usually manufactured from durable steels, such as stainless steel, which exhibit good strength and corrosion resistance. Failure of a heat exchanger occurred due to specification of a plain carbon steel that did not survive service in the SO2 vapor environment. However, failure analysis showed that cavitation erosion was the responsible failure mechanism, not corrosion as might be expected.

Abstract
Aircraft missile launcher attachment bolts fabricated from cadmium-coated Hy-tuf steel were found broken. Subsequent analysis of the broken bolts indicated three causes of failure. First, the bolts had been carburized, which was not in conformance with the heat treating requirements. Second, macroetching showed that the bolts has been machined from stock rather than forged, and the threads cut rather than rolled. It was also determined that hydrogen-assisted stress-corrosion cracking also played a part in the failure of the high-strength bolts.

Abstract
This paper deals with disk drive failures that occur in the interface area between the head and disk. The failures often lead to the loss of stored data and are characterized by circumferential microscratches that are usually visible to the unaided eye. The recording media in disk drives consists of a metal, glass, ceramic, or plastic substrate coated with a magnetic material. Data errors are classified as ‘soft’ or ‘hard’ depending on their correctability. Examination has shown that hard errors are the result of an abrasive wear process that begins with contact between head and disk asperities. The contact generates debris that, as it accumulates, increases contact pressure between the read-write head and the surface of the disk. Under sufficient pressure, the magnetic coating material begins wearing away, resulting in data loss.

Abstract
The heads of two AISI 8740 steel bolts severed while being installed into an Army tank recoil mechanism. Both broke into two pieces at the head-to-shank radius and the required torque value had not been attained nor exceeded prior to the failure. A total of 69 bolts from inventory and the field were tested by magnetic particle inspection. One inventory bolt failed because of a transverse crack near the head-to-shank radius. It was deduced that either a 100% magnetic particle inspection had not been conducted during bolt manufacturing, or the crack went undetected during the original inspection. Optical and electron microscopy of the broken bolts revealed topographies and the presence of black oxide consistent with quench cracking. The two bolts failed during installation due to the presence of pre-existing quench cracks. Recommendations to prevent future failures include: ensuring that 100% magnetic particle inspections are conducted after bolts are tempered; using dull cadmium plate or an alternative to the electrode position process, such as vacuum cadmium plate or ion-plate or ion-plated aluminum, to mitigate the potential for delayed failures due to hydrogen embrittlement or stress-corrosion cracking; ensuring that the radius at the shoulder/shank interface conforms to specifications; and replacing all existing bolts with new or reinspected inventory bolts.

Abstract
A 13/16-in. hex socket failed while in use. Analysis (hardness testing, optical and scanning electron microscopy, and EDS) revealed that the socket was made of low carbon steel formed in a powder metallurgy process. A number of flaws were found including nonuniform wall thickness, poor geometric design with sharp corners as stress raisers, and incomplete sintering evidenced by unsintered particles. These were determined to be the primary cause of failure, although inclusions on the fracture surface containing S and Al may have played a role as well.

Abstract
A cooler of an ammonia synthesis plant was destroyed after three years of service due to the rupture of a distribution manifold. Synthesis gas under high pressure and at about 300 deg C, consisting of 10% NH3 and unconverted gas of 25% N2 and 75% H2 content, was water-cooled externally to room temperature in this unit. The fracture had the typical flat-gray fibrous structure of a material destroyed by hydrogen. Specimens for the metallographic investigation showed that the structure appeared to have been loosened by intergranular separations. DVM notched impact specimens from the affected area yielded low specific impact energy values. These are the significant characteristics of hydrogen attack. The attack penetrated to a depth of 13 to 16 mm. It was recommended that the manifolds be made of hydrogen-resistant steel instead of the unalloyed steel used.

Abstract
A design may be improvable without presenting an unacceptable risk related to safety or performance. However, design-related failures can result from an oversight in performing one of the major design activities or from a failure to balance the competing demands inherent to part design. This article focuses on design-related failures in products utilizing polymeric materials, and reviews important considerations of the design envelope of plastic parts. The article provides a non-exhaustive list and descriptions of design tools that can support the design process and the prevention of design-related failures. It also discusses the most common causes of design-related failures of plastic parts. The article can assist in both failure analysis and in the prevention of failures in which design may be a contributing factor or a root cause.

Abstract
The first-stage nozzles of a high-pressure turbine section of an industrial gas turbine exhibited leading and trailing-edge deterioration. The nozzles were made of X-40, a cobalt-base alloy, and were aluminide coated. Failure analysis determined that the deterioration was the result of hot corrosion caused by a combination of contaminants, cooling-hole blockage, and coating loss.

Abstract
Welded steel storage vessels used to hold mildly alkaline solution were produced in exactly the same manner from deep-drawn aluminum-killed SAE 1006 low-carbon steel sheet. After the cylindrical shell was drawn, a top low-carbon steel closure was welded to the inside diameter. The containers were then filled with the slightly alkaline solution, pressurized, and allowed to stand under ambient conditions. A small number, less than 1%, were returned because they began to leak in service. Inspection revealed general corrosion and pitting on the inner surfaces. However, other tanks that experienced the same service conditions developed no corrosion. Corrosion was linked to forming defects that provided sites for localized corrosion, and to lack of steam drying after cleaning, which increased susceptibility to general corrosion.

Microbial degradation in the environment is initiated by abiotic (nonliving physical or chemical) processes. Mechanical weathering and other mechanical processes are the main drivers of the initial degradation. This article presents an overview of weathering and biodegradation. Accelerated life testing and aging methodologies are increasingly being used to generate engineering data for determining material property degradation and service life (or fitness for purpose) of plastic materials for hostile service conditions. This article presents an overview of accelerated life testing and aging of unreinforced and fiber-reinforced plastic materials for assessing long-term material properties and life expectancy in hostile service environments. It considers various environmental factors, such as temperature, humidity, pressure, weathering, liquid chemicals (i.e., alkalis and acids), ionizing radiation, and biological degradation, along with the combined effects of mechanical stress, temperature, and moisture (including environmental stress corrosion). The article also includes information on the use of accelerated testing for predicting material property degradation and long-term performance.

Abstract
A bearing cup in a drive shaft assembly on an automobile was found to have failed. A detailed analysis was conducted using the QC story approach, which begins by proposing several possible failure scenarios then following them to determine the main root cause. A number of alternative solutions were identified and then validated based on chemical analysis, endurance and hardness tests, and microstructural examination. The investigation revealed that carbonitriding can effectively eliminate the type of failure encountered because it prevents through hardening of the bearing cup assembly.
