Abstract
A 22 m (72 ft) diameter filled grain storage bin made from a 0.2% carbon steel collapsed at a temperature of −1 to 4 deg C (30 to 40 deg F). Failure analysis indicated that fracture occurred in a two-step process: first downward, by ductile failure of small ligament from a bolt hole near the bottom of the tank to create a crack 25 mm (1 in.) long, and then upward, by brittle fracture through successive 1.2 m (4ft) wide sheets of ASTM A446 material. Site investigation showed that the concrete base pad was not level. Chemical analysis indicated that the material had a high nitrogen content (0.020%). The allowable stress based on yield was estimated using four different design criteria. Correlation among those results was poor. The different criteria indicated that the material was loaded from the maximum allowable to approximately 30% less than allowable. Nevertheless, at this stress level, fracture mechanics indicated that the 25 mm (1 in.) starter crack exceeded or was very near the critical crack length for the material. Additional factors not taken into account in the design equations included cold work from a hole punching operation, thread imprinting in bolt holes, and an additional hoop stress created by forcing an incorrectly formed panel to fit the pad base radius. These factors increased the nominal design stress to a sufficiently large value to cause the critical crack length to be exceeded.

Abstract
Plastic product failures are directly attributed to one of the following four reasons: omission of a critical performance requirement, improper materials specification, design error, and processing/manufacturing error. Therefore, product failures can be minimized or eliminated if all of these parameters are comprehensively examined during the design process. This article focuses on all of these factors, except processing-related failures, which are outside the design and engineering domain. It is dedicated to the identification and avoidance of common problems associated with the selection and designing of plastic parts. The article provides information on the material-related design criteria that depend on the applications, environmental conditions of use, and performance requirements. It discusses physical properties of plastics based on their relevance to real-world environmental conditions. The most-common design problems related to design considerations are also covered.

Abstract
A detailed investigative failure analysis was conducted on an autoclave which blew apart in a furnace for no apparent reason. Bolt failure resulted in separation of the autoclave lid and subsequent destruction of the furnace. Analysis using metallography, fractography, mechanical testing and exemplar tests were performed on the bolt material. Mechanical engineering analysis and leak-before-break criteria were extensively analyzed. Results led to only one possible conclusion: that an explosion occurred within the autoclave. Suggestions for autoclave design are presented as a result of the analysis.

Abstract
When a large LPG low-carbon steel storage tank was put into service for the first time and filled beyond the proof testing level, a brittle fracture crack initiated at a fillet weld between a stiffener ring and the wall. The crack propagated to a length of 5.5 m and arrested. Analysis showed that the plates satisfied the criteria of BS 4741. It was concluded that the cause of crack initiation was the lack of a mouse hole at the junction between the stiffening ring and the wall of the tank. The tank was repaired and put back in service. When it was filled beyond the proof test level, again a brittle crack was initiated at a horizontal weld defect and propagated vertically, destroying the tank and the liquefaction plant. The initiation site was a thumbnail elliptical crack in a horizontal weld, having a depth of 1.5 mm, and a length of 4.5 mm. This showed that as late the mid-1970s, misunderstanding of brittle fracture led to the wrong design and construction of an LPG storage tank. The best design specification is to use a correlation between LAST, the Lowest Anticipated Service Temperature, and the DBTT measured by either Charpy tests or DTT.

Abstract
This paper describes the remote ultrasonic (UT) examinations of a high-level radioactive waste storage tank at the Savannah River Site in South Carolina. The inspections, carried out by E.R. Holland, R.W. Vande Kamp, and J.B. Elder, were performed from the contaminated, annular space of the 46 year old, inactive, 1.03 million gallon waste storage tank. A steerable, magnetic wheel wall crawler was inserted into the annular space through small (6 in., or 150 mm, diam) holes/risers in the tank top. The crawler carried the equipment used to simultaneously collect data with up to four UT transducers and two cameras. The purpose of this inspection was to verify corrosion models and to investigate the possibility of previously unidentified corrosion sites or mechanisms. The inspections included evaluation of previously identified leak sites, thickness mapping, and crack detection scans on specified areas of the tank. No indications of reportable wall loss or pitting were detected. All thickness readings were above minimum design tank-wall thickness, although several small indications of thinning were noted. The crack detection and sizing examinations revealed five previously undetected indications, four of which were only partially through-wall. The cracks that were examined were found to be slightly longer than expected but still well within the flaw size criteria used to evaluate tank structural integrity.

Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. The article also describes the role of the materials engineer in the design and materials selection process. It provides information on the significance of materials selection in both the prevention and analysis of failures.

Abstract
Reliability-centered maintenance (RCM) is a systematic methodology for preventing failures. This article begins by discussing the history of RCM and uses Society of Automotive Engineers (SAE) all-industry standard JA1011 as its model to describe the key characteristics of an RCM process. It then expands on questions involved in RCM process, offering definitions when necessary. Next, the article describes the approach of RCM to failure modes and effects analysis (FMEA), the failure management policies available under RCM, and the criteria of RCM for deciding when a specific failure management policy is technically feasible. Then, after discussing the ways that RCM classifies failure effects in terms of consequences, it describes how RCM uses failure consequences to identify the best failure management policy for each failure mode. Next, the building blocks of RCM are put together to create a failure management program. The article ends with a discussion on some practical issues pertaining to RCM that lie outside the scope of SAE JA1011.

Abstract
An evaluation of indications in the main turbine building column horizontal plate welds was conducted by the joint efforts of field metallography and nondestructive examinations. The turbine building main column horizontal plate welds were selected at random and were inspected to find discontinuities, metallurgical evaluation of the discontinuities, analysis of any failure modes, and determination of the best repair techniques. The welds were made with prequalified joints in accordance with AWS D1.1-77 and required only visual inspection. More sensitive inspection methods were applied to the welds in order to better define the indications found with the visual inspections. Cracks were found in 17 field welds and in two test plate welds. The causes of the cracking are related to the weld design and installation procedure. Three field welds were rejected because of the depth of the cracks. The NDT inspections, evaluations, method of field metallography, analysis and conclusions are discussed with recommendations for corrective actions in the following report.

Abstract
This article illustrates the use of the American Petroleum Institute (API) 579-1/ASME FFS-1 fitness-for-service (FFS) code (2020) to assess the serviceability and remaining life of a corroded flare knockout drum from an oil refinery, two fractionator columns affected by corrosion under insulation in an organic sulfur environment, and an equalization tank with localized corrosion in the shell courses in a chemicals facility. In the first two cases, remaining life is assessed by determining the minimum thickness required to operate the corroded equipment. The first is based on a Level 2 FFS assessment, while the second involves a Level 3 assessment. The last case involves several FFS assessments to evaluate localized corrosion in which remaining life was assessed by determining the minimum required thickness using the concept of remaining strength factor for groove-like damage and evaluating crack-like flaws using the failure assessment diagram. Need for caution in predicting remaining life due to corrosion is also covered.

Abstract
During testing of compressors under start/stop conditions, several helical suspension springs failed. The ensuing failure investigation showed that the springs failed due to fatigue. The analysis showed that during start/stop testing the springs would undergo both a lateral and axial deflection, greatly increasing the torsional stresses on the spring. To understand the fatigue limits under these test conditions, a bench test was used to establish the fatigue strength of the springs. The bench tests showed that the failed springs had an unacceptable surface texture that reduced the fatigue life. Based on an understanding of the compressor motion, a Monte Carlo model was developed based on a linear damage theory to predict the fatigue life of the springs during start/stop conditions. The results of this model were compared to actual test data. The model showed that the design was marginal even for springs with acceptable surface texture. The model was then used to predict the fatigue life requirements on the bench test such that the reliability goals for the start/stop testing would be met, thus reducing the risk in qualifying the compressor.

Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

One inch diam Type 304 stainless steel piping was designed to carry containment atmosphere samples to an analyzer to monitor hydrogen and oxygen levels during operational and the design basis accident conditions that are postulated to occur in a boiling water reactor. Only one of six lines in the system had thru-wall cracks. Shallow incipient cracks were detected at the lowest elevations of one other line. The balance of the system had no signs of SCC attack. Chlorides and corrosion deposits in varying amounts were found throughout the system. The failure mechanism was transgranular, chloride, stress-corrosion cracking. Replacement decisions were based on the presence of SCC attack or heavy corrosion deposits indicative of extended exposure time to chloride-contaminated water. The existing uncracked pipe, about 75 percent of the piping in the system, was retained despite the presence of low level surface chlorides. Controls were implemented to insure that temperatures are kept below 150 deg F, or, walls of the pipe are moisture-free or the cumulative wetted period will never exceed 30 h.

This article reviews the most common reasons for failures and the purpose of a failure investigation. It discusses the nine steps for the organization of a good failure investigation. The three basic tools that are helpful in any failure investigation, namely, a fault tree, a failure mode assessment chart, and a technical plan for resolution chart, are reviewed. The article briefly describes failure investigation pitfalls and concludes with information on the other common tools used for failure investigation and root cause determination.

When complex designs, transient loadings, and nonlinear material behavior must be evaluated, computer-based techniques are used. This is where the finite-element analysis (FEA) is most applicable and provides considerable assistance in design analysis as well as failure analysis. This article provides a general view on the applicability of finite-element modeling in conducting analyses of failed components. It highlights the uses of finite-element modeling in the area of failure analysis and design, with emphasis on structural analysis. The discussion covers the general development and both general- and special-purpose applications of FEA. The special-purpose applications of FEA covered are piping and pressure vessel analysis, impact analysis, and microelectronic and microelectromechanical systems analysis. Abstract
Accelerated life testing and aging methodologies are increasingly being used to generate engineering data for determining material property degradation and service life (or fitness for purpose) of plastic materials for hostile service conditions. This article presents an overview of accelerated life testing and aging of unreinforced and fiber-reinforced plastic materials for assessing long-term material properties and life expectancy in hostile service environments. It considers various environmental factors, such as temperature, humidity, pressure, weathering, liquid chemicals (i.e., alkalis and acids), ionizing radiation, and biological degradation, along with the combined effects of mechanical stress, temperature, and moisture (including environmental stress corrosion). The article also includes information on the use of accelerated testing for predicting material property degradation and long-term performance.
