Abstract
This article provides a description of the microscale models and mechanisms for deformation and fracture. Macroscale and microscale appearances of ductile and brittle fracture are discussed for various specimen geometries and loading conditions. The article reviews the general geometric factors and materials aspects that influence the stress-strain behavior and fracture of ductile metals. It highlights fractures arising from manufacturing imperfections and stress raisers. The article presents a root cause failure analysis case history to illustrate some of the fractography concepts.

Abstract
A bomb retaining ring fabricated from type 302 stainless steel unwrapped during a practice flight, causing the bomb fins to deploy. The retaining ring was able to unwrap itself because it was thinner and softer than required. Hardness testing, metallography, and tensile testing confirmed that the component was in the annealed condition and not in the required work-hardened 1/4-hard condition.

Abstract
Welded low-carbon steel bomb fins were rejected because of poor weld practice. Visual and metallographic examination revealed that the resistance plug welds that attach the outer skin to the inner spar displayed inadequate weld penetration. Recommended changes to the resistance welding parameters resulted in acceptable welds.

Abstract
Cluster bomb tailcone assemblies each containing two aluminum die-cast components were rejected because of the poor surface condition of the die castings. Numerous heat checks were found on the surfaces of the tailcones and radiographic inspection revealed inclusions, gas holes, and shrinkage defects in the castings. Most of the components failed to meet required mechanical properties because of these casting defects.

Abstract
Suspension lugs fabricated from AISI 4340 steel used to facilitate loading of bombs onto the underside of military aircraft could not sustain required loads during routine proof load testing. Three failed lugs underwent visual examination, chemical analysis, metallography, hardness testing, scanning electron microscopy, and energy-dispersive x-ray spectroscopy. It was determined that the failures were due to forging defects. Both forging laps and seams acted as stress concentrators when the lugs were loaded during proof testing.

Abstract
Aircraft missile launcher attachment bolts fabricated from cadmium-coated Hy-tuf steel were found broken. Subsequent analysis of the broken bolts indicated three causes of failure. First, the bolts had been carburized, which was not in conformance with the heat treating requirements. Second, macroetching showed that the bolts has been machined from stock rather than forged, and the threads cut rather than rolled. It was also determined that hydrogen-assisted stress-corrosion cracking also played a part in the failure of the high-strength bolts.

Abstract
Breech bolt assemblies from the Gatling guns used on fighter aircraft failed during firing tests. Metallography of the failed components revealed considerable decarburization which resulted in a loss of surface hardness. It was also determined that the maraging steel components were not in the nitrided condition as was required. This resulted in lower wear and fatigue resistance. These components also had a silicon content nearly double of that specified. The high silicon content lowered the notch tensile strength and toughness of the components.

Abstract
Rotor blades in the compressor section of a J79 engine had failed. Optical, stereoscopic, microhardness testing, and SEM examinations were conducted to determine the cause. The blades were made of STS403 and were used uncoated. They were damaged over an extensive area, from the 15th through the 17th compressor stages, as were stator vanes and casing sections. The fractured surface of the 17th blade showed multiple origins along with secondary cracking and extensive propagation that preceded separation. The metallographic analysis of the microstructure suggested work hardening. Based on the results, the cause of the fractured blade was high-amplitude fatigue due to severe stall. After normal engine usage of five months, the blade fractured sending fragments throughout the combustion and turbine sections.

This article describes some of the factors and conditions that might be considered when approaching a failure analysis problem. It focuses on the key principles, objectives, practices, and procedures of failure analysis. The article provides guidelines on the preparation of a protocol for a failure analysis. It also demonstrates the proper approaches to failure analysis.

Abstract
A fire in a storage yard engulfed several propane delivery trucks, causing one of them to explode. A series of elevated-temperature stress-rupture tears developed along the top of the truck-mounted tank as it was heated by the fire. Unstable fracture then occurred suddenly along the length of the tank and around both end caps, following the girth welds that connect them to the center portion of the tank. The remaining contents of the tank were suddenly released, aerosolized, and combusted, creating a powerful boiling liquid expanding vapor explosion (BLEVE). Based on the metallography of the tank pieces, the approximate tank temperature at the onset of explosion was determined. Metallurgical analysis provided additional insights as well as a framework for making tanks less susceptible to this destructive failure mechanism.

Abstract
Identification of alloys using quantitative chemical analysis is an essential step during a metallurgical failure analysis process. There are several methods available for quantitative analysis of metal alloys, and the analyst should carefully approach selection of the method used. The choice of appropriate analytical techniques is determined by the specific chemical information required, the condition of the sample, and any limitations imposed by interested parties. This article discusses some of the commonly used quantitative chemical analysis techniques for metals. The discussion covers the operating principles, applications, advantages, and disadvantages of optical emission spectroscopy (OES), inductively coupled plasma optical emission spectroscopy (ICP-OES), X-ray spectroscopy, and ion chromatography (IC). In addition, information on combustion analysis and inert gas fusion analysis is provided.

Abstract
This article provides an overview of metal additive manufacturing (AM) processes and describes sources of failures in metal AM parts. It focuses on metal AM product failures and potential solutions related to design considerations, metallurgical characteristics, production considerations, and quality assurance. The emphasis is on the design and metallurgical aspects for the two main types of metal AM processes: powder-bed fusion (PBF) and directed-energy deposition (DED). The article also describes the processes involved in binder jet sintering, provides information on the design and fabrication sources of failure, addresses the key factors in production and quality control, and explains failure analysis of AM parts.

Abstract
This article discusses the organization required at the outset of a failure investigation and provides a methodology with some organizational tools. It focuses on the use of problem-solving tools such as a fault tree analysis combined with critical thinking. The discussion covers nine steps to organize a good failure investigation. They are as follows: understand and negotiate goals of the investigation, obtain a clear understanding of the failure, identify all possible root causes, objectively evaluate the likelihood of each root cause, converge on the most likely root cause(s), objectively and clearly identify all possible corrective actions, objectively evaluate each corrective action, select optimal corrective action(s), and evaluate effectiveness of selected corrective action(s). Common problems detrimental to a failure investigation are also covered.

Abstract
Mechanical testing is an evaluative tool used by the failure analyst to collect data regarding the macro- and micromechanical properties of the materials being examined. This article provides information on a few important considerations regarding mechanical testing that the failure analyst must keep in mind. These considerations include the test location and orientation, the use of raw material certifications, the certifications potentially not representing the hardware, and the determination of valid test results. The article introduces the concepts of various mechanical testing techniques and discusses the advantages and limitations of each technique when used in failure analysis. The focus is on various types of static load testing, hardness testing, and impact testing. The testing types covered include uniaxial tension testing, uniaxial compression testing, bend testing, hardness testing, macroindentation hardness, microindentation hardness, and the impact toughness test.

Abstract
The various methods of furnace, torch, induction, resistance, dip, and laser brazing are used to produce a wide range of highly reliable brazed assemblies. However, imperfections that can lead to braze failure may result if proper attention is not paid to the physical properties of the material, joint design, prebraze cleaning, brazing procedures, postbraze cleaning, and quality control. Factors that must be considered include brazeability of the base metals; joint design and fit-up; filler-metal selection; prebraze cleaning; brazing temperature, time, atmosphere, or flux; conditions of the faying surfaces; postbraze cleaning; and service conditions. This article focuses on the advantages, limitations, sources of failure, and anomalies resulting from the brazing process. It discusses the processes involved in the testing and inspection required of the braze joint or assembly.

Abstract
The goal of using nondestructive evaluation (NDE) in conjunction with failure analysis is to obtain the most comprehensive set of data in order to characterize the details of the damage and determine the factors that allowed the damage to occur. The NDE results can be used to determine optimal areas upon which to focus for sectioning and metallography in order to further investigate the condition of the component. This article provides information on the inspection method available for failure analysis, including standard methods such as visual testing, penetrant testing, and magnetic particle testing. It covers the effects of various factors on the properties of the part that may impact failure analysis, describes the characterization of damage modes and crack sizes, and finally discusses the processes involved in application of NDE results to failure analysis.
