Abstract
This article provides an overview of the electrochemical nature of corrosion and analyzes corrosion-related failures. It describes corrosion failure analysis and discusses corrective and preventive approaches to mitigate corrosion-related failures of metals. These include: change in the environment; change in the alloy or heat treatment; change in design; use of galvanic protection; use of inhibitors; use of nonmetallic coatings and liners; application of metallic coatings; use of surface treatments, thermal spray, or other surface modifications; corrosion monitoring; and preventive maintenance.

Abstract
Flow forming technology has emerged as a promising, economical metal forming technology due to its ability to provide high strength, high precision, thin walled tubes with excellent surface finish. This paper presents experimental observations of defects developed during flow forming of high strength SAE 4130 steel tubes. The major defects observed are fish scaling, premature burst, diametral growth, microcracks, and macrocracks. This paper analyzes the defects and arrives at the causative factors contributing to the various failure modes.

Abstract
A carbon steel piping cross-tee assembly which conveyed hydrogen sulfide (H7S) process gas at 150 to 275 deg C (300 to 585 deg F) with a maximum allowable operating pressure of 3 MPa (450 psig) ruptured at the toe of one of the welds at the cross after several years of service. The failure was initially thought to be the result of thermal fatigue, and the internal surfaces exhibited the “elephant hide” pattern characteristic of thermal fatigue. However metallographic failure analysis found that this pattern was the result of corrosion rather than thermal fatigue. Corrosion caused failure at this location because the weld was abnormally thin as fabricated. Thus, failure resulted from inadequate deposition of weld metal and subsequent wall thinning from internal corrosion. It was recommended that the cross-tee be replaced with a like component, with more careful attention to weld quality.

Abstract
This article provides some new developments in elevated-temperature and life assessments. It is aimed at providing an overview of the damage mechanisms of concern, with a focus on creep, and the methodologies for design and in-service assessment of components operating at elevated temperatures. The article describes the stages of the creep curve, discusses processes involved in the extrapolation of creep data, and summarizes notable creep constitutive models and continuum damage mechanics models. It demonstrates the effects of stress relaxation and redistribution on the remaining life and discusses the Monkman-Grant relationship and multiaxiality. The article further provides information on high-temperature metallurgical changes and high-temperature hydrogen attack and the steps involved in the remaining-life prediction of high-temperature components. It presents case studies on heater tube creep testing and remaining-life assessment, and pressure vessel time-dependent stress analysis showing the effect of stress relaxation at hot spots.

Abstract
This article focuses on the life assessment methods for elevated-temperature failure mechanisms and metallurgical instabilities that reduce life or cause loss of function or operating time of high-temperature components, namely, gas turbine blade, and power plant piping and tubing. The article discusses metallurgical instabilities of steel-based alloys and nickel-base superalloys. It provides information on several life assessment methods, namely, the life fraction rule, parameter-based assessments, the thermal-mechanical fatigue, coating evaluations, hardness testing, microstructural evaluations, the creep cavitation damage assessment, the oxide-scale-based life prediction, and high-temperature crack growth methods.

Abstract
This article reviews the applied aspects of creep and stress-rupture failures. It discusses the microstructural changes and bulk mechanical behavior of classical and nonclassical creep behavior. The article provides a description of microstructural changes and damage from creep deformation, including stress-rupture fractures. It also describes metallurgical instabilities, such as aging and carbide reactions, and evaluates the complex effects of creep-fatigue interaction. The article concludes with a discussion on thermal fatigue and creep fatigue failures.

Abstract
Material samples collected from failed booster pumps were analyzed to determine the cause of failure and assess the adequacy of the materials used in the design. The pumps had been in service at a power plant, transporting feedwater from a deaerator to a main turbine boiler. Samples from critical areas of the pump were examined using optical and scanning electron microscopy, electrochemical analysis, and tensile testing. Based on microstructure and morphology, estimated corrosion rates, and particle concentrations in the feedwater, it was concluded that cavitation and erosion were the dominant failure mechanisms and that the materials and processes used to make the pumps were largely unsuited for the application.

Abstract
Military aircraft use a cartridge ignition system for emergency engine starts. Analysis of premature failures of steel (AISI 4340) breech chambers in which the solid propellant cartridges were burned identified corrosion as one problem with an indication that stress-corrosion cracking may have occurred. A study was made for stress-corrosion cracking susceptibility of 4340 steel in a paste made of the residues collected from used breech chambers. The constant extension rate test (CERT) technique was employed and SCC susceptibility was demonstrated. The residues, which contained both combustion products from the cartridges and corrosion products from the chamber, were analyzed using elemental analysis and x-ray diffraction techniques. Electrochemical polarization techniques were also utilized to estimate corrosion rates.

Abstract
Corrosion is the deterioration of a material by a reaction of that material with its environment. The realization that corrosion control can be profitable has been acknowledged repeatedly by industry, typically following costly business interruptions. This article describes the electrochemical nature of corrosion and provides the typical analysis of environmental- and corrosion-related failures. It presents common methods of testing of laboratory corrosion and discusses the processes involved in the prevention of environmental- and corrosion-related failures of metals and nonmetals.

Abstract
A number of plastic sleeves used in an automotive application cracked after assembly but prior to installation into the mating components. The sleeves were specified to be injection molded from a 20% glass-fiber-reinforced polybutylene terephthalate (PBT) resin. After molding, electronic components are inserted into the sleeves, and the assembly is filled with a potting compound. Investigation of the cracked parts and some reference parts available for testing included visual inspection, micro-FTIR in the ATR mode, and analysis using DSC. Subtle spectrum differences suggested degradation of the failed part material, and the thermograms supported this. The conclusion was that the failed sleeves had cracked due to embrittlement associated with severe degradation and the corresponding molecular weight reduction. The reduction in molecular weight significantly reduced the mechanical properties of the sleeves. The cause of the degradation was not evident, but the likely source appears to be the molding operation and exposure to elevated temperature for an extended period of time.

Abstract
This article reviews the analytical techniques most commonly used in plastic component failure analysis. These include the Fourier transform infrared spectroscopy, differential scanning calorimetry, thermogravimetric analysis, thermomechanical analysis, and dynamic mechanical analysis. The descriptions of the analytical techniques are supplemented by a series of case studies that include pertinent visual examination results and the corresponding images that aid in the characterization of the failures. The article describes the methods used for determining the molecular weight of a plastic resin. It explains the use of mechanical testing in failure analysis and also describes the considerations in the selection and use of test methods.

Abstract
The principal types of elevated-temperature mechanical failure are creep and stress rupture, stress relaxation, low- and high-cycle fatigue, thermal fatigue, tension overload, and combinations of these, as modified by environment. This article briefly reviews the applied aspects of creep-related failures, where the mechanical strength of a material becomes limited by creep rather than by its elastic limit. The majority of information provided is applicable to metallic materials, and only general information regarding creep-related failures of polymeric materials is given. The article also reviews various factors related to creep behavior and associated failures of materials used in high-temperature applications. The complex effects of creep-fatigue interaction, microstructural changes during classical creep, and nondestructive creep damage assessment of metallic materials are also discussed. The article describes the fracture characteristics of stress rupture. Information on various metallurgical instabilities is also provided. The article presents a description of thermal-fatigue cracks, as distinguished from creep-rupture cracks.

Abstract
This study examines the role of calcium-precipitating bacteria (CPB) in heat exchanger tube failures. Several types of bacteria, including Serratia sp. (FJ973548), Enterobacter sp. (FJ973549, FJ973550), and Enterococcus sp. (FJ973551), were found in scale collected from heat exchanger tubes taken out of service at a gas turbine power station. The corrosive effect of each type of bacteria on mild steel was investigated using electrochemical (polarization and impedance) techniques, and the biogenic calcium scale formations analyzed by XRD. It was shown that the bacteria contribute directly to the formation of calcium carbonate, a critical factor in the buildup of scale and pitting corrosion on heat exchanger tubes.

Abstract
A straight-tube cooler type heat exchanger had been in service for about ten years serving a coal pulverizer in Georgia. Non-potable cooling water from a local lake passed through the inner surfaces of the copper tubing and was cooling the hot oil that surrounded the outer diametral surfaces. Several of the heat exchangers used in the same application at the plant had experienced a severe reduction in efficiency in the past few years. One heat exchanger reportedly experienced some form of leakage following discovery of oil contaminating the cooling water. This heat exchanger was the subject of a failure investigation to determine the cause and location of the leaks. Corrosion products primarily contained copper oxide, as would be expected from a copper tubing. The product also exhibited the presence of a significant amount of iron oxides. Metallographic cross sectioning of the tubes and microscopic analysis revealed several large and small well rounded corrosion pits present at the inner diametral surfaces. The cause of corrosion was attributed to corrosive waters that were not only corroding the copper, but were corroding steel pipes upstream from the tubing.

Abstract
A number of failures involving carbon and alloy steels were analyzed to assess the effects of inclusions and their influence on mechanical properties. Inclusions, including brittle oxides and more ductile manganese sulfides (MnS), affect fatigue endurance limit, fatigue crack propagation rates, fracture toughness, notch toughness, and transverse tensile properties, and do so in an anisotropic manner with respect to rolling direction. Significant property anisotropy has been documented in the failures investigated, providing evidence that designers failed to account for it. Typical fracture morphologies observed in such cases and metallographic appearances of MnS-containing materials are illustrated.

Abstract
Housings (being tested as part of a material conversion) from an electrical appliance failed during an engineering evaluation. They had been injection molded from a commercial polycarbonate/PET blend. Parts produced from the previous material, a nylon 6/6 resin, had consistently passed the testing regimen. Grease was applied liberally within the housing assembly during production. Investigation included visual inspection, 24x SEM images, micro-FTIR in the ATR mode, and analysis using DSC. No signs of material contamination were found, but the thermograms showed a crystallization of the PET resin. The grease present within the housing assembly, analyzed using micro-FTIR, was composed of a hydrocarbon-based oil, a phthalate-based oil, lithium stearate, and an amide-based additive. The conclusion was that the appliance housings failed through environmental stress cracking caused by a phthalate-based oil that was not compatible with the PC portion of the resin blend. Thus, the resin conversion was the root cause of the failures. Additionally, during the injection molding process the molded parts had been undercrystallized, reducing their mechanical strength. More importantly, the resin had been degraded, producing a reduction in the molecular weight and reducing both the mechanical integrity and chemical-resistance properties of the parts.

Abstract
A chemical storage vessel failed while in service. The failure occurred as cracking through the vessel wall, resulting in leakage of the fluid. The tank had been molded from a high-density polyethylene (HDPE) resin. The material held within the vessel was an aromatic hydrocarbon-based solvent. Investigation (visual inspection, stereomicroscopic examination, 20x/100x SEM images, micro-FTIR in the ATR mode, and analysis using DSC and TGA) supported the conclusion that the chemical storage vessel failed via a creep mechanism associated with the exertion of relatively low stresses. The source of the stress was thought to be molded-in residual stresses associated with uneven shrinkage. This was suggested by obvious distortion evident on cutting the vessel. Relatively high specific gravity and the elevated heat of fusion indicated that the material had a high level of crystallinity. In general, increased levels of crystallinity result in higher levels of molded-in stress and the corresponding warpage. The significant reduction in the modulus of the HDPE material, which accompanied the saturation of the resin with the aromatic hydrocarbon-based solvent, substantially decreased the creep resistance of the material and accelerated the failure.

The specified elongation of 10% could not be achieved in several hollow pinion gear shafts made of cast Cr-Mo steel GS 35 Cr-Mo 5 3 that were heat treated to a strength of 90 kp/sq mm. The steel was melted in a basic 3 ton arc furnace and deoxidized in the furnace and in the pan with a total of 7 kg aluminum. Fracture of a tensile specimen occurred with low elongation and, apparently, also with low reduction of area. In some places it was coarse grained conchoidal. It was found that the exceptionally low elongation of the cast specimens was due to excessive deoxidation by aluminum.

Abstract
A fire in a storage yard engulfed several propane delivery trucks, causing one of them to explode. A series of elevated-temperature stress-rupture tears developed along the top of the truck-mounted tank as it was heated by the fire. Unstable fracture then occurred suddenly along the length of the tank and around both end caps, following the girth welds that connect them to the center portion of the tank. The remaining contents of the tank were suddenly released, aerosolized, and combusted, creating a powerful boiling liquid expanding vapor explosion (BLEVE). Based on the metallography of the tank pieces, the approximate tank temperature at the onset of explosion was determined. Metallurgical analysis provided additional insights as well as a framework for making tanks less susceptible to this destructive failure mechanism.

Abstract
When bulging occurred in mortar tubes made of British I steel during elevated-temperature test firing, a test program was formulated to evaluate the high-temperature properties (at 540 to 650 deg C, or 1000 to 1200 deg F) of the British I steel and of several alternative alloys including a maraging steel (18% Ni, grade 250), a vanadium-modified 4337 gun steel (4337V), H19 tool steel, and high-temperature alloys Rene 41, Inconel 718, and Udimet 630. All the alloys evaluated had been used in mortar tubes previously or were known to meet the estimated minimum yield strength. The alloys fall in this order of decreasing strengths: Udimet 630, Inconel 718, Rene 41, H19 tool steel, British I steel, 4337V gun steel, and maraging steel. When cycled between room temperature and 540 to 650 deg C (1000 to 1200 deg F), only Udimet 630, Inconel 718, and Rene 41 retained yield strengths higher than the minimum. Also, these three alloys maintained high strengths over the tested range, whereas the others decreased in yield strength as cycling progressed. Analysis showed Inconel 718 was considered best suited for 81-mm mortar tubes, and widespread industrial use ensured its availability.
