Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.

Abstract
Cavitation damage of diesel engine cylinder liners is due to vibration of the cylinder wall, initiated by slap of the piston under the combined forces of inertia and firing pressure as it passes top dead center. The occurrence on the anti-thrust side may possibly result from bouncing of the piston. The exact mechanism of cavitation damage is not entirely clear. Two schools of thought have developed, one supporting an essentially erosive, and the other an essentially corrosive, mechanism. Measures to prevent, or reduce, cavitation damage should be considered firstly from the aspect of design, attention being given to methods of reducing the amplitude of the liner vibration. Attempts have been made to reduce the severity of attack by attention to the environment. Inhibitors, such as chromates, benzoate/nitrite mixtures, and emulsified oils, have been tried with varying success. Attempts have been made to reduce or prevent cavitation damage by the application of cathodic protection, and this has been found to be effective in certain instances of trouble on propellers.

Practical examples of stress-corrosion cracking (SCC) and methods for its prevention were presented. Cracks in chloride-sensitive austenitic steels were very branched and transcrystalline. Etched cross sections of molybdenum-free samples showed chloride-induced cracks running out of the pitted areas. Alternatively polishing and etching micro-sections for viewing at high magnification made crack detail more visible. Optical and scanning electron micrographs showed cracking in austenitic cast steel and cast iron due to both internal tensile and critical residual stresses; the latter causes flake-like spalling. Measures to prevent SCC include stress reduction, use of austenitic steels or nickel alloys not susceptible to grain boundary attack, use of ferritic chromium steels, surface slag removal, control of temperature and chloride concentration, and cathodic protection.

Abstract
Corrosion is the deterioration of a material by a reaction of that material with its environment. The realization that corrosion control can be profitable has been acknowledged repeatedly by industry, typically following costly business interruptions. This article describes the electrochemical nature of corrosion and provides the typical analysis of environmental- and corrosion-related failures. It presents common methods of testing of laboratory corrosion and discusses the processes involved in the prevention of environmental- and corrosion-related failures of metals and nonmetals.

It describes corrosion failure analysis and discusses corrective and preventive approaches to mitigate corrosion-related failures of metals. These include: change in the environment; change in the alloy or heat treatment; change in design; use of galvanic protection; use of inhibitors; use of nonmetallic coatings and liners; application of metallic coatings; use of surface treatments, thermal spray, or other surface modifications; corrosion monitoring; and preventive maintenance.

Abstract
Single 6.4 mm (0.25 in.) post-tensioning wires failed in a parking garage in the southern portion of the United States. Several failed wires were removed and the lengths were examined for signs of corrosion using SEM metallography. The scans showed localized shallow pitting, and chloride was detected in some of the pits. The test also revealed an initial crack that was probably caused by hydrogen embrittlement. Since no chloride was detected on the fracture surface, and none was detected in the overlying concrete, the corrosion appears to have begun prior to the wires' placement in the concrete.

Abstract
Leaks developed at random locations in aluminum brass condenser tubes within the first year of operation of a steam condenser in a nuclear power plant. One failed tube underwent scanning electron microscopy surface examination and optical microscope metallography. It was determined that the tube failed from crevice corrosion under seawater deposits that had formed on the inner surface. Mechanical cleaning of the condenser tubes every 6 months and installation of intake screens of smaller mesh size were recommended.

Given the extreme conditions to which surgical tools can be exposed, and the potential for misuse, failures are inevitable and systematic methods for analyzing them are necessary to keep them in check.

A segment of a stainless steel clad bottom cone of an acid sulfite pulping batch digester failed from severe corrosion loss. The digester was fabricated of 19 mm ( 3 4 in.) low-carbon steel with 3.8 mm (0.15 in.) type 317L stainless steel cladding. The manufacturing method for the cladding was unknown. Visual and metallographic analyses indicated that the failure was from transgranular stress-corrosion cracking (TGSCC), which caused extensive cracking and spalling of the cladding and was localized in a segment of the bottom cone. The remainder of the digester cladding was unaffected. The TGSCC was attributed to high, locked-in residual stresses from the cladding process. It was recommended that the bottom cone replacement segment be stress relieved prior to installation.

Abstract
A shopping mall in South Carolina was originally constructed in 1988 and a second phase completed in 1989. The HVAC system inside the mall included an open, recirculating condenser water loop that served various fan coil units located within tenant spaces. The system had a recirculating capacity of about 44,000 gal (166,000 L) of water. It consisted primarily of steel pipes fitted with threaded connectors on the 2 in. (46 cm) pipes and bolted flanged couplings on the larger pipes. Seven years following the completion of the mall, corrosion problems were noted at the outer and inner surfaces of the pipe. Visual observations on the inner diametral surfaces revealed that the pipes were, in almost all cases, filled with corrosion products. A significant amount of base metal loss was documented in all of the samples. The cause of the observed corrosion was determined to be a lack of corrosion monitoring and poor water quality. Pipe replacement and a regular water testing program were recommended.

Abstract
A helicopter tail rotor blade spar failed in fatigue, allowing the blade to separate during flight. The 2014-T652 aluminum alloy blade had a hollow spar shank filled with lead wool ballast and a thermoset polymeric seal. A corrosion pit was present at the origin of the fatigue zone and numerous trails of corrosion pits were located on the spar cavity's inner surfaces. The corrosion pitting resulted from the failure of the thermoset seal in the spar shank cavity. The seal failure allowed moisture to enter into the cavity. The moisture then served as an electrolyte for galvanic corrosion between the lead wool ballast and the aluminum spar inner surface. The pitting initiated fatigue cracking which led to the spar failure.

Abstract
Fragments of screen bars which as structural elements of a condenser had come into contact with cooling water from the mouth of a river were received. The screen bars were made of stainless austenitic Cr-Ni-Mo steel X 5 Cr-Ni-Mo18 10 (Material No. 1.4401). The bars were fractured repeatedly. The ruptures did not occur exclusively or even preferentially at the loops, but just as frequently at locations between them. The mistake made in this case was annealing the steel at a temperature in the critical region. This was probably done to relieve stresses that originated during cold-forming and led to damage by stress corrosion. This would have been the correct method for a ferritic steel, but not austenitic steel, which requires the special heat treatment indicated. When an anneal in the critical region is unavoidable and the indicated additional treatment is impossible or difficult, a type of steel has to be chosen which is resistant to intergranular corrosion.

Abstract
Aluminum alloy BS.1476-HE.15 by virtue of its high strength and low density finds application in the form of bars or sections for cranes, bridges, and other such structures where a reduction in dead weight load and inertia stresses is advantageous. Bars and sections in H.15 alloy are mostly produced by extrusion. Some material processed this way has been prone to exfoliation corrosion. Extended aging for 24 h at a temperature of 185 deg C (365 deg F) virtually suppresses the tendency for exfoliation corrosion to develop. Also, the use of a sprayed coating, either of aluminum or Al-1Zn alloy, was effective in halting and preventing this form of attack. While alarming, the appearance of exfoliation corrosion provides a valuable warning to the engineer or inspector before a severe weakening of the particular sections has occurred.

Abstract
The drive-shaft hanger bearings failed after 300 to 400 h in service. The shaft, supported by labyrinth-sealed single row radial ball bearings of ABEC-1 tolerances, was made of aluminum 2024-T3 tubing (2.5 cm diam and 1.2 mm wall thickness). The bearings were lubricated with a paste-type mineral-oil lubricant (containing molybdenum disulfide and polytetrafluoroethylene particles) or grease conforming to MIL-G-81322 (containing thickening agent and synthetic hydrocarbons) and had two-piece spot-welded retainers. On visual examination, the balls were observed to be embedded in the inner-ring raceway which had been softened by the elevated temperatures reached during the failure. Broken retainers and worn and bent out of shape seals were found. Penetration of gritty particles, water and other corrosive agents and leakage of lubricant out of the bearing permitted by the worn seals was observed. It was concluded that overheating was caused by lubricant flow was permitted by wear of the labyrinth seals. Positive rubbing seals and MIL-G-81322 grease lubricant were found to have longer life than those with the labyrinth seals and mineral-oil-paste lubricant on testing under simulated environmental conditions and were installed as a corrective measure. Importance of dirt free supply and drainage of oil was discussed.

Abstract
The wires used in a wet precipitator for cleaning the gases coming off a basic oxygen furnace failed. The system consisted of six precipitators, three separate dual units, each composed of four zones. Each zone contained rows of wires (cold drawn AISI 1008 carbon steel) suspended between parallel collector plates. It was determined that the 1008 wires failed because of corrosion fatigue. It was decided to replace all of the wires in the two zones with the highest rates of failure with cold-drawn type 304 austenitic stainless steel wire. These expensive wires, however, failed after a week by transgranular SCC. Annealed type 430 ferritic stainless steel was subsequently suggested to prevent further failures.

Abstract
A type 321 stainless steel downcomer expansion joint that handled process gases was found to be leaking approximately 2 to 3 weeks after installation. The expansion joint was the second such coupling placed in the plant after failure of the original bellows. The failed joint was disassembled and examined to determine the cause of failure. Energy-dispersive x-ray analysis revealed significant peaks for chlorine and phosphorus, indicating failure by chloride stress-corrosion cracking (SCC). Cracks in the liner and bellows exhibited a branched pattern also typical of SCC. Cracks through the inner liner initiated on the outer surface of the liner and propagated inward, whereas cracks in the bellows originated on the inner surface and propagated outward. Stress-corrosion cracking of the assembly was caused by chloride contaminants trapped inside the bellows following hydrostatic testing. Checking the test fluid for chloride and removing all fluids after hydrostatic testing were recommended to prevent further failure.

Abstract
The US Army Research Laboratory performed a failure investigation on a broken main landing gear mount from an AH-64 Apache attack helicopter. A component had failed in flight, and initially prevented the helicopter from safely landing. In order to avoid a catastrophe, the pilot had to perform a low hover maneuver to the maintenance facility, where ground crews assembled concrete blocks at the appropriate height to allow the aircraft to safely touch down. The failed part was fabricated from maraging 300 grade steel (2,068 MPa [300 ksi] ultimate tensile strength), and was subjected to visual inspection/light optical microscopy, metallography, electron microscopy, energy dispersive spectroscopy, chemical analysis, and mechanical testing. It was observed that the vacuum cadmium coating adjacent to the fracture plane had worn off and corroded in service, thus allowing pitting corrosion to occur. The failure was hydrogen-assisted and was attributed to stress corrosion cracking (SCC) and/or corrosion fatigue (CF). Contributing to the failure was the fact that the material grain size was approximately double the required size, most likely caused from higher than nominal temperatures during thermal treatment. These large grains offered less resistance to fatigue and SCC. In addition, evidence of titanium-carbo-nitrides was detected at the grain boundaries of this material that was prohibited according to the governing specification. This phase is formed at higher thermal treatment temperatures (consistent with the large grains) and tends to embrittle the alloy. It is possible that this phase may have contributed to the intergranular attack. Recommendations were offered with respect to the use of a dry film lubricant over the cadmium coated region, and the possibility of choosing an alternative material with a lower notch sensitivity. In addition, the temperature at which this alloy is treated must be monitored to prevent coarse grain growth. As a result of this investigation and in an effort to eliminate future failures, ARL assisted in developing a cadmium brush plating procedure, and qualified two Army maintenance facilities for field repair of these components.
