Abstract
A failure of an aboveground storage tank occurred due to external corrosion of the tank floor. The liquid asphalt tank operated at elevated temperatures (approximately 177 deg C, or 350 deg F) and had been in service for six years. Cathodic protection (rectifiers) had been installed since start-up of the tank operation. It was noted, however, that some operational problems with the rectifier may have interrupted its protection. Investigation (visual inspection, on-site examination and testing, EDS analysis of scale deposits, and MIC testing of the soil) supported the conclusion that corrosion may have been caused by an interruption in cathodic protection. The effectiveness of cathodic protection on established microbial deposits is questionable. Recommendations included ultrasonically testing the tank floor and replacing portions based on the remaining wall thickness. Doubling the wall thickness of the floor plates was also recommended.

Abstract
A group of control valves that regulate production in a field of sour gas wellheads performed satisfactorily for three years before pits and cracks were detected during an inspection. One of the valves was examined using chemical and microstructural analysis to determine the cause of failure and provide preventive measures. The valve body was made of A216-WCC cast carbon steel. Its inner surface was covered with cracks stemming from surface pits. Investigators concluded that the failure was caused by a combination of hydrogen-induced corrosion cracking and sulfide stress-corrosion cracking. Based on test data and cost, A217-WC9 cast Cr–Mo steel would be a better alloy for the application.

Abstract
Examination of a 7075-T6 aluminum alloy pylon strut revealed cracks in two locations on the ears of the strut. Because the part was still intact, the cracks had to be forced open so that the fractures could be examined. Scanning electron microscopy (SEM) of the opened cracks showed that the crack surfaces were covered with a mud crack pattern suggestive of stress-corrosion cracking (SCC). The T6 temper is susceptible to SCC. It was concluded that cracking of the strut could have been aggravated by the hard landing experienced by the aircraft. The strut, however, contained stress-corrosion cracks which were present before the landing. It was recommended that an inspection for SCC be made of all pylon struts with a similar service life.

Abstract
Corrosion is the deterioration of a material by a reaction of that material with its environment. The realization that corrosion control can be profitable has been acknowledged repeatedly by industry, typically following costly business interruptions. This article describes the electrochemical nature of corrosion and provides the typical analysis of environmental- and corrosion-related failures. It presents common methods of testing of laboratory corrosion and discusses the processes involved in the prevention of environmental- and corrosion-related failures of metals and nonmetals.

Abstract
A cadmium-plated 4340 Ni-Cr-Mo steel ballast elbow assembly was submitted for failure analysis to determine the element or radical present in an oxidation product found inside the elbow assembly. Energy-dispersive x-ray analysis in the SEM showed that iron was the predominant species, presumably in an oxide form. The inside surface had the appearance of typical corrosion products. Hardness measurements indicated that the 4340 steel was heat treated to a strength of approximately 862 MPa (125 ksi). It was concluded that the oxide detected on the ballast elbow was iron oxide. The possibility that the corrosion products would eventually create a blockage of the affected hole was great considering the small hole diameter (4.2 mm, or 0.165 in.). It was recommended that a quick fix to stop the corrosion would be to apply a corrosion inhibitor inside the hole. This, however, would cause the possibility of inhibitor buildup and the eventual clogging of the hole. A change in the manufacturing process to include a cadmium plating on the hole inside surface was recommended. This was to be accomplished in accordance with MIL specification QQ-P-416, Type II, Class 1. A material change to 300-series stainless steel was also recommended.

Abstract
Hydrogen-assisted stress-corrosion cracking failure occurred in four AISI 4137 chromium molybdenum steel bolts having a hardness of 42 HRC. The normal service temperature (400 deg C, or 750 deg F) was too high for hydrogen embrittlement but, the bolts were subjected also to extended shutdown periods at ambient temperatures. The corrosive environment contained trace hydrogen chloride and acetic acid vapors as well as calcium chloride if leaks occurred. The exact service life was unknown. The bolt surfaces showed extensive corrosion deposits. Cracks had initiated at both the thread roots and the fillet under the bolt head. Multiple, branched cracking was present in a longitudinal section through the failed end of one bolt, typical of hydrogen-assisted SCC in hardened steels. Chlorides were detected within the cracks and on the fracture surface. The failed bolts were replaced with 17-4 PH stainless steel bolts (Condition H 1150M) having a hardness of 22 HRC.

Abstract
A wall section of a carbon steel choke body in gas service at 4400 psig blew out three months after the use of a corrosion inhibitor was stopped. Corrosion damage occurred in ripples, leaving both smoothly polished and unattacked areas. The corrodent in condensate wells was principally carbon dioxide dissolved in water condensed from the gas stream, with organic acids possibly an aggravating factor. A gas analysis showed no other corrosive agents. No metallurgical or fabrication defects were found in the carbon steel part. The mode of attack was corrosion-erosion, caused by the corrosive, high velocity gas flow. The corrosion rate of either the inhibited or uninhibited gas stream was too high for equipment in high pressure gas service. Type 410 (12% Cr) stainless steel was recommended for the choke bodies because other equipment such as valves made of type 410 showed no evidence of corrosion damage after three years' exposure. This change was made five years ago and there have been no failures since.

Abstract
A fan support casting failed unexpectedly while running at 1800 rpm in pulp at 65 deg C (150 deg F). The leading edge of the blade exhibited deep spongy holes leading to reduced section and finally to fracture of the part when the remaining section size was insufficient to support the load. Analysis showed the support casting to be a standard 8620 type composition with a hardness of 311 HRB. The design of the casting was not streamlined. There were several square corners present where great pressure differences could be generated. This was a case of erosion-corrosion with the classic spongy appearance of cavitation. Two changes were proposed: streamlining the part to avoid abrupt changes in fluid flow; and a change in alloy to a more corrosion-resistant material (304 or preferably 316) to increase the tenacity of protective films.

Abstract
Helicopter rotor blade components that included the horizontal hinge pin, the associated nut, and the locking washer were examined. Visual examination of the submitted parts revealed that the hinge pin, fabricated from 4340 steel, was broken and that the fracture face showed a flat beach mark pattern indicative of a preexisting crack. The threaded area of the pin had an embedded thread that did not appear to come from the pin. A chemical analysis was conducted on the embedded thread and on an associated attachment to determine the origin of the thread. Analysis showed that the thread and nut were 4140 steel. Scanning electron fractographic examination of the fracture initiation site strongly suggested that the fracture progressed by fatigue. It was concluded that the failure of the horizontal hinge pin initiated at areas of localized corrosion pits. The pits in turn initiated fatigue cracks, resulting in a failure mode of corrosion fatigue. It was recommended that all of the horizontal hinge pins be inspected. Those pins determined to be satisfactory for further use should be stripped of cadmium, shot peened, and coated with cadmium to a minimum thickness of 0.0127 mm (0.0005 in.).

Abstract
An elbow made from welded steel tube had become leaky along a well-defined line in the axial direction. The entire wall of the tube was corroded, and the longitudinally-welded seam stood out clearly as a result of particularly intensive corrosive attack. The appearance of the corroded surface indicates the action of water with a high oxygen content. The oxygen in the return water must have originated from the ventilation of the open expansion vessel. Because of the corrosion-favoring effect of a crevice, water with a high oxygen-content that was perhaps still warm or even hot found particularly favorable conditions for corrosion in the defective welded seam (crevice corrosion). The tube material itself is perfectly satisfactory and in no way responsible for the failure.

Abstract
Some of the admiralty brass tubes were failing in a heat exchanger. The heat exchanger cooled air by passing river water through the inside of the tubes. The wall thickness of all tubes ranged between 1.19 to 1.27 mm (0.047 to 0.050 in.). General intergranular corrosion occurred at the inside surfaces of the tubes. Transgranular stress-corrosion cracking, probably the result of sulphates under basic conditions, and dezincification occurred also as the result of galvanic corrosion under the deposits in the tubes. Recommendations were to use a closed-loop water system to eliminate sulphates, ammonia, etc., and to run trials on one unit with tubes of other alloys such as 80-20 Cu-Ni or 70-30 Cu-Ni to evaluate their performance prior to any large scale retubing operations.

Abstract
Two hot water reheat coil valves from a heating/ventilating/air-conditioning system failed in service. The values, a 353 copper alloy 19 mm (3/4 in.) valve and a 360 copper alloy 13 mm (1/2 in.) valve, had been failing at an increasing rate. The failures were confined to the stems and seats. Visual examination revealed severe localized metal loss in the form of deep grooves with smooth and wavy surfaces. Metallographic analysis of the grooved areas revealed uniform metal loss. No evidence of intergranular or selective attack indicating erosion-corrosion was observed, Recommendations included use of a higher-copper brass, cupronickel, or Monel for the valve seats and stems and operation of the valves in either the fully opened or closed position.

Abstract
Four nose wheels fabricated from 2014-T6 aluminum alloy and cold worked at the flange were examined. Visual examination showed that the failure started in the tube well area on the wheel with serial number 31. The failure initiated in the flange fillet on wheels with serial numbers 67, 217, and 250. Scanning electron microscopy (SEM) examination of the fractures showed that failure initiated by SCC or a corrosion pit on all failures examined. The failures then progressed by fatigue. Dye penetrant testing revealed no additional flaws on the wheels that had failed in the flange area. There was, however, one flaw area in the flange of the wheel that failed in the tube well. This flaw resembled a corrosion pit. It was concluded that failure of nose wheels 67, 217, and 250 was caused by cracking due to SCC or pitting. The failures progressed by fatigue. Because failure occurred in the same general area on all three wheels, these locations are suspect as being underdesigned. It was recommended that the nose wheel be redesigned and additional service data be accumulated to understand the contributing factors that resulted in wheel failure.

Abstract
A failed laser mirror and another complete mirror of the same construction were analyzed. The laser mirror consisted of three layers of material brazed together to form channels through which the cooling water flows. Samples were analyzed with light optical and scanning electron microscopy. The corrosion product contained molybdenum and copper with a trace of gold. The base material was analyzed as molybdenum with negligible alloying additions. The primary mode of corrosion attack on the base material appeared to be intergranular, although uniform corrosion was evident also. It was concluded that corrosion attack sufficiently weakened the base material and the brazed joints, allowing catastrophic failure of the mirror due to the pressure of the cooling water. It was recommended that the mirrors be cleaned of all corrosion products present as a result of past service conditions and proof tested. It was recommended that the water system consisting of deionized water and formaldehyde be replaced with water having a low oxygen content and a cathodic inhibitor (oxygen scavenger).

Abstract
One of five underground drain lines intended to carry a highly acidic effluent from a chemical-processing plant to distant holding tanks failed in just a few months. Each line was made of 304L stainless steel pipe 73 mm (2 in.) in diam with a 5 mm (0.203 in.) wall thickness. Lengths of pipe were joined by shielded metal arc welding. Soundness of the welded joints was determined by water back-pressure testing after several lengths of pipe had been installed and joined. Before completion of the pipeline, a pressure drop was observed during back-pressure testing. An extreme depression in the backfill revealed the site of failure. Analysis (visual inspection, electrical conductivity, and soil analysis) supported the conclusions that the failure had resulted from galvanic corrosion at a point where the corrosivity of the soil was substantially greater than the average, resulting in a voltage decrease near the point of failure of about 1.3 to 1.7 V. Recommendations included that the pipelines be asphalt coated and enclosed in a concrete trough with a concrete cover. Also, magnesium anodes, connected electrically to each line, should be installed at periodic intervals along their entire length to provide cathodic protection.

Abstract
Fretting and fretting corrosion at the contact area between the screw hole of a type 316LR stainless steel bone plate and the corresponding screw head was studied. The attack on the 316LR stainless steel was only shallow. Mechanical grinding and polishing structures were exhibited by a large portion of the contact area. Fine corrosion pits in the periphery were observed and intense mechanical material transfer that can take place during fretting was revealed. Smearing of material layers over each other during wear was observed and attack by pitting corrosion was interpreted to be possible.

Abstract
Three wing flap hinge bearings were received by the laboratory for analysis. The bearings were fabricated from chromium-plated type 440C martensitic stainless steel. The intergranular fracture pattern seen in the electron fractographs, coupled with the corrosion pits observed on the inner diam of the bearings, strongly suggested that failure initiated by pitting and progressed by SCC or hydrogen embrittlement from the plating operation. It was recommended that the extent of the flap hinge bearing cracking problem be determined by using nondestructive inspection because it is possible to crack hardened type 440C during the chromium plating process. An inspection for pitting on the bearing inner diam was also recommended. It was suggested that electroless nickel be used as a coating for the entire bearing. A review of the chromium plating and baking sequence was recommended also to ensure that a source of hydrogen is not introduced during the plating operation.

Abstract
Cerclage wire, which was used with two screws and washers for a tension band in a corrective internal fixation, was found broken at several points and corroded after nine months in service. The material was examined using energy-dispersive x-ray analysis and determined not to be in compliance with standards (type 304 stainless steel without molybdenum). The screws and washers were found to be made of remelted implant-quality type 316L stainless steel and were intact. Signs of sensitization, characterized by chromium carbide precipitates at the grain boundaries, were revealed by the microstructure. Intercrystalline corrosion with pitted grains was indicated by SEM fractography. Improper heat treatment of the steel was interpreted to have led to intercrystalline corrosion and implant separation.

Abstract
A large portion of the four-hole Lane plate disintegrated and consisted mainly of corrosion products after remaining in the body for 26 years. Transformation structures and carbides were exhibited by the plate which was made from chromium steel. Minimal corrosion was exhibited by the soft austenitic 304 stainless steel used to make the screws. The corrosion products of the plate were revealed by microprobe analysis to impregnate the surrounding tissues. Improper material selection was concluded to be the reason for the general corrosion behavior.
