Abstract
A rail section that failed due to fatigue showed a smooth surface with well-developed conchoidal markings. This indicated successive stages of crack propagation, characteristic of fatigue failure. The crack was one of several which developed in the sections of curved rail which formed the lower roller path on which the superstructure of a walking drag-line excavator slewed. The cracking, which ran horizontally, developed at the junction of the underside of the rail head with the web and originated at surface defects in the form of grooves present on the castings. It was concluded that the cracking was caused by lateral deflection of the rails under in-service loads. The web of a rail would normally be loaded in compression but, should lateral movements occur, then it would be subjected to bending stresses and fatigue cracks could break out in regions where excessive tensile components predominated.

Abstract
Manufacturing process selection is a critical step in plastic product design. The article provides an overview of the functional requirements that a part must fulfil before process selection is attempted. A brief discussion on the effects of individual thermoplastic and thermosetting processes on plastic parts and the material properties is presented. The article presents process effects on molecular orientation. It also illustrates the thinking that goes into the selection of processes for size, shape, and design factors. Finally, the article describes how various processes handle reinforcement.

Abstract
An investigation of wear and failure of babbitt bushes was completed in this study. The results showed that wear at dry sliding of babbitt obtained by plasma spraying was less than that of babbitt in the as-cast state and after a deformation heat treatment. The failure of babbitt bushes was caused by a simultaneous and interrelated exhibition of fatigue and wear processes that depend considerably on cohesion strength between the bush and the bearing base and accumulation of defects on the contact surface between the bush and the shaft.

Abstract
A failed laser mirror and another complete mirror of the same construction were analyzed. The laser mirror consisted of three layers of material brazed together to form channels through which the cooling water flows. Samples were analyzed with light optical and scanning electron microscopy. The corrosion product contained molybdenum and copper with a trace of gold. The base material was analyzed as molybdenum with negligible alloying additions. The primary mode of corrosion attack on the base material appeared to be intergranular, although uniform corrosion was evident also. It was concluded that corrosion attack sufficiently weakened the base material and the brazed joints, allowing catastrophic failure of the mirror due to the pressure of the cooling water. It was recommended that the mirrors be cleaned of all corrosion products present as a result of past service conditions and proof tested. It was recommended that the water system consisting of deionized water and formaldehyde be replaced with water having a low oxygen content and a cathodic inhibitor (oxygen scavenger).

Abstract
A valve-seat retainer spring (made of 0.23 mm thick 17-7 PH stainless steel) from a fuel control on an aircraft engine was found to be broken after 3980 h of service. The two inner tabs were found to be broken off. The part was revealed to be in relative rotation against its contacting member by the radial wear marks on the convex surface. Beach marks indicating that fatigue fracture had been initiated at the convex surface of the washer and had propagated across to the concave surface were revealed by examination of the fractured surfaces of the washer. The cracks were revealed to have originated in the 0.38-mm radius fillet between the tab and the body of the washer. It was interpreted from the analysis of the compound fracture that it was composed of fatigue fractures caused by the formed tab being loaded so as to compress the spring along the axis of its centerline and produce torsional vibrations. It was concluded that the two inner tabs had broken in fatigue as the result of cyclic loading that compressed and torsionally vibrated the spring. The fillets were replaced with slots to minimize stress concentration at the corners as a corrective measure.

A 2 ft. diam 20 ft. long cylinder with a wall thickness of 1 in. used for the transportation of a compressed gas failed by cracking. The cylinder was forged in a low ally steel. The working pressure was 3000 psi and it had been in service for about seven years. A longitudinal crack, about 2 in. long, developed at the approximate mid-length of the vessel, and allowed slow de-pressurization. Subsequent examination by radiography and ultrasonic means indicated the crack was associated with an irregularly shaped, laminar type of defect located within the wall of the vessel. It was concluded that failure of this vessel resulted from the development of a radial crack orientated in the axial direction. This appeared to have originated on the bore surface in a region where the laminar defect closely approached this surface. The defect was introduced during the manufacture of the vessel, probably originating as a secondary pipe in the ingot which was subsequently displaced and forced into the wall of the vessel during the piercing operation.

Abstract
A failure analysis case study on railroad rails is presented. The work, performed under the sponsorship of the Department of Transportation, addresses the problem of shell and detail fracture formation in standard rails. Fractographic and metallographic results coupled with hardness and residual stress measurements are presented. These results suggest that the shell fractures form on the plane of maximum residual tensile stresses. The formation of the shells is aided by the presence of defects in the material in these planes of maximum residual stress. The detail fracture forms as a perturbation from the shell crack under cyclic loading and is constrained to develop as an embedded flaw in the early stages of growth because the crack is impeded at the gage side and surface of the rail head by compressive longitudinal stresses.

Abstract
Flow forming technology has emerged as a promising, economical metal forming technology due to its ability to provide high strength, high precision, thin walled tubes with excellent surface finish. This paper presents experimental observations of defects developed during flow forming of high strength SAE 4130 steel tubes. The major defects observed are fish scaling, premature burst, diametral growth, microcracks, and macrocracks. This paper analyzes the defects and arrives at the causative factors contributing to the various failure modes.

Abstract
Life testing of cyclic loaded, miniature extension springs made of 17-7 PH stainless steel wire and AISI 302 Condition B stainless steel wire has shown end hook configuration to be a major source of weakness. To avoid cracking and subsequent fatigue failure, it was found that stress concentration depended on end hook bend sharpness. Also, interference fits are to be avoided in the end hooks of small springs. Additionally, a need for careful consideration of the stress-corrosion properties of candidate materials for spring applications has been demonstrated by stress-corrosion test results for 17-7 PH CH900 and for Custom 455 CH850 stainless steels. Laboratory testing of these two materials in the form of compression springs confirmed the superiority of the 17-7 PH over Custom 455.

Abstract
A needle bearing from a filling and seating machine for milk cartons became unusable due to corrosion and fracture of a ring after only four weeks of operation of the machine in a Finnish milk packing plant. These bearings were subject to corrosion by water condensates in this type of environment because of constant temperature changes, and they normally are replaced after eight months. The bearings were lubricated by a molybdenum sulfide paste. Judging by their structure the needles probably consisted of ball bearing steel. They showed corroded initial cracks of the pitting type, i.e., shear-fatigue fractures due to excessive surface pressure. The needles too were overstressed by compression. It seemed that the higher pressure necessary for the pressing of thicker paper accelerated the corrosion, which lead to the crack initiations of the parts and possibly also to impaired lubrication. The machine manufacturer therefore switched to bearings with shells of a complex bronze.

Abstract
A valve spring made of 4.1 mm diam wire, designed to withstand 10,000,000 stress cycles, fractured after only 2,000,000 cycles. The surface displayed impressions which indicated it had been treated by shot blasting. The spring has broken in two places. Fracture 1 was a torsional fatigue fracture which has started from a lobe-like surface defect and not, as is usual, from a point on the most highly stressed inner surface. Fracture 2, on the other hand, was a bending fatigue fracture with a starting point on both the inner and the outer surface of the spiral. The objective of the shot blasting, to put the surface into a state of even compressive internal stress, which must first be overcome during subsequent bending and torsional loading before the boundary zone comes under tensile stress, was therefore not realized in this case. On the contrary, the shot blasting led to a state of internal stress which favored fracture of the spring.

Abstract
Superelastic nitinol wires that fractured under various conditions were examined under a scanning electron microscope in order to characterize the fracture surfaces, produce reference data, and compare the findings with prior published work. The study revealed that nitinol fracture modes and morphologies are generally consistent with those of ductile metals, such as austenitic stainless steel, with one exception: Nitinol exhibits a unique damage mechanism under high bending strain, where damage occurs at the compression side of tight bends or kinks while the tensile side is unaffected. The damage begins as slip line formation due to plastic deformation, which progresses to cracking at high strain levels. The cracks appear to initiate from slip lines and extend in shear (mode II) manner.

Copper shortening has been found to occur in the rotor windings of turbo alternators and takes the form of a progressive reduction in the length of the coils leading to distortion of the end windings. The trouble results from the high loading which develops between successive layers of the strip conductor due to centrifugal force. This leads to a high frictional binding force between turns and prevents axial expansion under normal heating in service. Rotor trouble which proved to be due to copper shortening was found in a set rated at 27.5 MW. It was manufactured in 1934 at which time silver-bearing copper was not available. The use of hard-drawn silver-bearing copper for a rewind, in conjunction with special attention to blocking up the end windings, is confidently expected to effect a complete cure.

Abstract
Mechanical testing is an evaluative tool used by the failure analyst to collect data regarding the macro- and micromechanical properties of the materials being examined. This article provides information on a few important considerations regarding mechanical testing that the failure analyst must keep in mind. These considerations include the test location and orientation, the use of raw material certifications, the certifications potentially not representing the hardware, and the determination of valid test results. The article introduces the concepts of various mechanical testing techniques and discusses the advantages and limitations of each technique when used in failure analysis. The focus is on various types of static load testing, hardness testing, and impact testing. The testing types covered include uniaxial tension testing, uniaxial compression testing, bend testing, hardness testing, macroindentation hardness, microindentation hardness, and the impact toughness test.

Abstract
Failed ferrous components were analyzed from a crane that operated on an offshore platform. The crane failed during operation and fell into the sea. The brake spring on the boom hoist was found to have fractured in four places. The spring contained a line defect (seam) that was the source of each crack. The fracture of the oil quenched and tempered (HRC 50 ASTM A229) spring was by stress-corrosion cracking after the crane fell into the sea because fatigue cannot account for the fractures observed. The crane failure was caused by an overload created by the operator catching a free-falling load.

Abstract
In the course of a general overhaul, the crankpins and main journals (3 in. diam) of the crankshaft of a four-cylinder oil engine were built up by metal spraying. Four weeks later, the shaft broke through the pin remote from the flywheel (driving) end. The fracture was of the fatigue type. A creeping crack originated in the fillet at the inside surface of the pin and extended parallel to the plane of the web across practically the entire section before complete rupture occurred. The sprayed metal on the fractured pin had very poor adhesion. The surfaces of the main journals had not been grooved but appeared to have been roughened by shot or grit-blasting prior to spraying and the deposit was more firmly adherent to these surfaces than in the case of the pins. It is doubtful, however, whether the adhesion of sprayed metal to a surface prepared even in this manner would always be satisfactory under severe loading conditions, such as those to which a crankpin is subjected in service.

A compression hip screw is a device designed to hold fractures in the area of the femur in alignment and under compression. A side plate, which is an integral part of the device, is attached by screws to the femur, and it holds the compression screw in position. The device analyzed had broken across the eighth hole (of nine holes) from the end of the plate. The detailed metallurgical failure analysis of the device, including metallography and fractography, is reported here. It was found that the device had adequate metallurgical integrity for the application for which it was intended. It is believed that failure was caused by the lack of a screw in the ninth hole. Evidence is also presented which indicates that the device was bent prior to insertion, and the local plastic deformation may have caused structural changes leading to premature crack initiation.

Abstract
Mechanical springs are used in mechanical components to exert force, provide flexibility, and absorb or store energy. This article provides an overview of the operating conditions of mechanical springs. Common failure mechanisms and processes involved in the examination of spring failures are also discussed. In addition, the article discusses common causes of failures and presents examples of specific spring failures, describes fatigue failures that resulted from these types of material defects, and demonstrates how improper fabrication can result in premature fatigue failure. It also covers failures of shape memory alloy springs and failures caused by corrosion and operating conditions.
