Abstract
Graphitization, the formation of graphite nodules in carbon and low alloy steels, contributes to many failures in high-temperature environments. Three such failures in power-generating systems were analyzed to demonstrate the unpredictable nature of this failure mechanism and its effect on material properties and structures. In general, the more randomly distributed the nodules, the less effect they have on structural integrity. In the cases examined, the nodules were found to be organized in planar arrays, indicating they might have an effect on material properties. Closer inspection, however, revealed that the magnitude of the effect depends on the relative orientation of the planar arrangement and principle tensile stress. For normal orientation, the effect of embrittlement tends to be most severe. Conversely, when the orientation is parallel, the nodules have little or no effect. The cases examined show that knowledge is incomplete in regard to graphitization, and the prediction of its occurrence is not yet possible.

Abstract
A reversible four-way carbon steel flap valve in a thermal power station failed after 7 years of service. The flap had been fabricated by welding two carbon steel plates to both sides of a carbon steel forging. The valve was used for reversing the flow direction of seawater in the cooling system of a condenser. Visual examination of the flap showed crystalline fracture, indicating a brittle failure. Metallographic examination, chemical analyses, and tensile and impact testing indicated that the failure was caused by the notch sensitivity of the forging material, which resulted in low toughness. It was recommended that fully killed carbon steel with a fine-grain microstructure be used. Redesign of the flap to remove the step in the forging that acted as a notch was also recommended.

Abstract
Handles welded to the top cover plate of a chemical-plant downcomer broke at the welds when the handles were used to lift the cover. The handles were fabricated of low-carbon steel rod; the cover was of type 502 stainless steel plate. The attachment welds were made with type 347 stainless steel filler metal to form a fillet between the handle and the cover. The structure was found to contain a zone of brittle martensite in the portion of the weld adjacent to the low-carbon steel handle; fracture had occurred in this zone. The brittle martensite layer in the weld was the result of using too large a welding rod and too much heat input, melting of the low-carbon steel handle, which diluted the austenitic stainless steel filler metal and formed martensitic steel in the weld zone. Because it was impractical to preheat and postheat the type 502 stainless steel cover plate, the low-carbon steel handle was welded to low-carbon steel plate, using low-carbon steel electrodes. This plate was then welded to the type 502 stainless steel plate with type 310 stainless steel electrodes. This design produced a large weld section over which the load was distributed.

Abstract
The corner of a welded sheet construction made from austenitic corrosion-resistant chromium-nickel steel showed corrosive attack of the outer sheet. This attack was most severe at the points subjected to the greatest heat during welding. Particularly large amounts of weld metal had been applied. Microscopic examination showed grain disintegration was promoted by the thickness of the weld bead and the amount of heat required to produce it. If nonstabilized austenitic sheet is to be used in the future, one of the particularly low-carbon steels, X2 CrNi 18 9 or X2 CrNiMo 18 10, is recommended.

Abstract
This article introduces some of the general sources of heat treating problems with particular emphasis on problems caused by the actual heat treating process and the significant thermal and transformation stresses within a heat treated part. It addresses the design and material factors that cause a part to fail during heat treatment. The article discusses the problems associated with heating and furnaces, quenching media, quenching stresses, hardenability, tempering, carburizing, carbonitriding, and nitriding as well as potential stainless steel problems and problems associated with nonferrous heat treatments. The processes involved in cold working of certain ferrous and nonferrous alloys are also covered.

Abstract
Welded steel storage vessels used to hold mildly alkaline solution were produced in exactly the same manner from deep-drawn aluminum-killed SAE 1006 low-carbon steel sheet. After the cylindrical shell was drawn, a top low-carbon steel closure was welded to the inside diameter. The containers were then filled with the slightly alkaline solution, pressurized, and allowed to stand under ambient conditions. A small number, less than 1%, were returned because they began to leak in service. Inspection revealed general corrosion and pitting on the inner surfaces. However, other tanks that experienced the same service conditions developed no corrosion. Corrosion was linked to forming defects that provided sites for localized corrosion, and to lack of steam drying after cleaning, which increased susceptibility to general corrosion.

Abstract
Gross wastage and embrittlement were observed in plain carbon steel desuperheaters in five new Naval power plants. The gross wastage could be duplicated in laboratory bomb tests using sodium hydroxide solutions and was concluded to be caused by free caustic concentrated by high heat flux. The embrittlement was shown to be caused by the flow of corrosion generated hydrogen which converted the cementite to methane which nucleated voids in the steel. A thermodynamic estimate indicated that a small amount of chromium would stabilize the carbides against decomposition by hydrogen in this temperature range, and laboratory tests with 2-14% Cr steel verified this.

After 10 to 20 months of service, the carbon steel hoppers on three trucks used to transport bulk ammonium nitrate prills developed extensive cracking in the upper walls. The prills were discharged from the steel hoppers using air superchargers that generated an unloading pressure of approximately 11 kPa (7 psi). Each hopper truck held from 9,100 to 11,800 kg (10 to 13 tons) of prills when fully loaded and handled approximately 90,700 kg (100 tons) per month. The walls of the hoppers were made of 2.7 mm (0.105 in.) thick flat-rolled carbon steel sheet of structural quality, conforming to ASTM A 245 (obsolete specification replaced by A 570 and A 611). Investigation (visual inspection and 100x micrographs polished and etched with nital) supported the conclusion that failure of the hoppers was the result of intergranular SCC of the sheet-steel walls because of contact with a highly concentrated ammonium nitrate solution. Recommendations included the cost-effective solution of applying a three-coat epoxy-type coating with a total dry thickness of 0.3 mm (0.013 in.) to the interior surfaces of the hoppers.

Abstract
A number of failures involving carbon and alloy steels were analyzed to assess the effects of inclusions and their influence on mechanical properties. Inclusions, including brittle oxides and more ductile manganese sulfides (MnS), affect fatigue endurance limit, fatigue crack propagation rates, fracture toughness, notch toughness, and transverse tensile properties, and do so in an anisotropic manner with respect to rolling direction. Significant property anisotropy has been documented in the failures investigated, providing evidence that designers failed to account for it. Typical fracture morphologies observed in such cases and metallographic appearances of MnS-containing materials are illustrated.

A riveted 0.25% carbon steel oil-storage tank in Oklahoma was dismantled and reassembled in Minnesota by welding to form a storage tank for soybean oil. An opening was cut in the side of the tank to admit a front-end loader. A frame of heavy angle iron was welded to the tank and drilled for bolting on a heavy steel plate. The tank was filled to a record height. In mid-Jan the temperature dropped to -31 deg C (-23 deg F), with high winds. The tank split open and collapsed. The welding used the shielded metal arc process with E6010 electrodes, which could lead to weld porosity, hydrogen embrittlement, or both. At subzero temperatures, the steel was below its ductile-to-brittle transition temperature. These circumstances suggest a brittle condition. Steps to avoid this type of failure: For cold conditions, the steel plate should have a low carbon content and a high manganese-to-sulfur ratio and be in a normalized condition, low-hydrogen electrodes and welding practices should be used, all corners should be generously radiused, the welds should be inspected and ground or dressed to minimize stress concentrations, postweld heating is advisable, and radiographic and penetrant inspection tests should be performed.

Abstract
A steel socket pipe conduit NW 150 cracked open during pressure testing next to the weld seam almost along the entire circumference. The crack occurred in part in the penetration notch and in part immediately adjacent to it. While the uncracked pipe showed the light etch shading of a low-carbon steel in which the zone heated during welding was delineated only slightly next to the seam, the other pipe was etched much darker, i.e., higher in carbon, and the heated zone appeared to stand out darkly against the basic material. The overlapping weld was defect-free and dense. The uncracked pipe consisted of soft steel that obviously was made for this purpose, while the cracked pipe consisted of a strongly-hardenable steel which contained not only more carbon and manganese than customary but also a considerable amount of chromium. Therefore, the damage was caused by a mix-up of materials that allowed an unsuitable steel to be used for the weldment.

A segment of a stainless steel clad bottom cone of an acid sulfite pulping batch digester failed from severe corrosion loss. The digester was fabricated of 19 mm ( 3 4 in.) low-carbon steel with 3.8 mm (0.15 in.) type 317L stainless steel cladding. The manufacturing method for the cladding was unknown. Visual and metallographic analyses indicated that the failure was from transgranular stress-corrosion cracking (TGSCC), which caused extensive cracking and spalling of the cladding and was localized in a segment of the bottom cone. The remainder of the digester cladding was unaffected. The TGSCC was attributed to high, locked-in residual stresses from the cladding process. It was recommended that the bottom cone replacement segment be stress relieved prior to installation.

Abstract
Several type 304L stainless steel dished ends used in the fabrication of cylindrical vessels developed extensive cracking during storage. All of the dished ends had been procured from a single manufacturer and belonged to the same batch. When examined visually, several rust marks were observed, indicating contamination by rusted carbon steel particles. Liquid penetrant testing was used to determine the extent of the cracks, and in situ metallographic analysis was performed over the cracked region. The morphology of the cracks was indicative of transgranular stress-corrosion cracking (TGSCC). Conditions promoting the occurrence of the TGSCC included significant tensile stresses on the inside of the dished ends, the presence of surface contamination by iron due to poor handling practice using carbon steel implements, and storage in a coastal environment with an average temperature of 25 to 32 deg C (77 to 90 deg F), an average humidity ranging from 70 to 80%, and an atmospheric NaCl content ranging from 8 to 45 mg/m2 /day. Recommendations preventing further occurrence of the situation were strict avoidance of the use of carbon steel handling implements, strict avoidance of cleaning practices that cause long-term exposure to chlorine-containing cleaning fluid, and solution annealing of the dished ends at 1050 deg C (1920 deg F) for 1 h followed by water quenching to relieve residual stresses.

Abstract
A supplementary axle, which was used as an extension to a highway-trailer tractor to increase its load-bearing capacity, failed in service. The rolled steel channel extensions that secured the axle assembly to the tractor main-frame I-beams fractured transversely, with the crack in each instance initiating at a weld that joined the edge of the lower flange to the support bracket casting. The cracks propagated through the flange on each side until the effective cross-sectional area had been reduced sufficiently to bring about sudden and complete fracture of the remaining web and upper flange. Fatigue fracture was caused by a combination of high bending stresses in the bottom flanges of the channels due to the heavy load being carried, concentration of stresses due to the rapid change in section modulus of the channel at its point of attachment to the support-bracket casting, and brittleness of the high-hardness HAZ of the weld associated with the abnormally high carbon content in the central part of the channel. Welding of channel edges contributed to harmful gradients in section moduli and should be avoided in future assemblies.

Abstract
Schedule 80 low-carbon steel pipes used to transfer kraft liquor in a Kamyr continuous pulp digester failed within 18 months after installation. Visual and metallographic examinations established that the cracking initiated on the internal surfaces of the equalizer pipes in the welds and heat-affected zones (HAZs). Fracture/crack morphology was brittle and primarily intergranular and deposits at crack tips were primarily iron oxides with significant amounts of sodium compounds. On these bases, the cracking was characterized as intergranular stress-corrosion cracking (IGSCC). Corrosion-related deterioration was not found, indicating that the material was generally suitable for the intended service. High residual tensile stresses in the welds and HAZS, resulting from field welding under highly constrained conditions using inadequate weld procedures, were the most probable cause of the failures. Minimizing residual stresses through use of welding procedures that include appropriate preweld and interpass temperatures and postweld stress relief heat treatment at 650 deg C (1200 deg F) was recommended to prevent further failures.

A throttle arm of an aircraft engine fractured and caused loss of engine control. The broken part consisted of a 6.4-mm (1/4-in.) diam medium-carbon steel rod with a thread to fit a knurled brass nut that was inserted into the throttle knob. The threaded rod had been welded to the throttle-linkage bar by an assembly-weld deposit made on the rod adjacent to the threaded portion. The fracture surface exhibited a coarse-grain brittle texture with an initiating crack at a thread root. The throttle-arm failed by brittle fracture because of the presence of cracks at the thread roots that were within the HAZ of the adjacent weld deposit. The heat of welding had generated a coarse-grain structure with a weak grain-boundary network of ferrite that had not been corrected by postweld heat treatment. The combination of the cracks and this unfavorable microstructure provided a weakened condition that resulted in catastrophic, brittle fracture under normal applied loads. The design was altered to eliminate the weld adjacent to the threaded portion of the rod.

Abstract
A pipe in the lateral wall of a boiler powering an aircraft carrier flat-top boat failed during a test at sea. The pipe was made from ASTM 192 steel, an adequate material for the application. Microstructural analysis along with equipment operating records provided valuable insight into what caused the pipe to rupture. Although the pipe had been replaced just 50 h before the accident, the analysis revealed incrustations and corrosion pits on the inner walls and oxidation on the outer walls. Microstructural changes were also observed, indicating that the steel was exposed to high temperatures. The combined effect of pitting, incrustations, and phase transformations caused the pipe to rupture.

Abstract
Examples of metallic inclusions in steels of various types are presented. The structure of an inclusion in an annealed Fe-1C-1.5Cr steel consisted of ferrite with lamellar pearlite. The carbon content of the inclusion was therefore considerably lower than that of the chromium steel and was adapted to the latter by diffusion only at the periphery of the inclusion. In another section of a hardened piece of the same chromium steel, the steel in this case had a structure of martensite with hypereutectic carbide, while the inclusions consisted of a very fine laminated eutectoid of the lower pearlite range (Troostite). In a pipe of 18-8 austenitic stainless steel a weakly magnetizable spot of limited size was found. This inclusion too was probably more alloy-deficient than the austenitic steel, similar to the ones described above. All three cases were casting defects.

This study examines the role of calcium-precipitating bacteria (CPB) in heat exchanger tube failures. Several types of bacteria, including Serratia sp. (FJ973548), Enterobacter sp. (FJ973549, FJ973550), and Enterococcus sp. (FJ973551), were found in scale collected from heat exchanger tubes taken out of service at a gas turbine power station. The corrosive effect of each type of bacteria on mild steel was investigated using electrochemical (polarization and impedance) techniques, and the biogenic calcium scale formations analyzed by XRD.
