Abstract
A roadarm for a tracked vehicle failed during preproduction vehicle testing. The arm was a weldment of two cored low-alloy steel sand castings specified to ASTM A 148, grade 120–95. A maximum carbon content of 0.32% was specified. The welding procedure called for degreasing and gas metal arc welding; neither preheating nor postheating was specified. The filler metal was E70S-6 continuous consumable wire with a copper coating to protect it from atmospheric oxidation while on the reel. Analysis of the two castings revealed that the carbon content was higher than specified, ranging from 0.40 to 0.44%. The fracture occurred in the HAZ , where quenching by the surrounding metal had produced a hardness of 55 HRC. Some roadarms of similar carbon content and welded by the same procedure had not failed because they had been tempered during a hot-straightening operation. Brittle fracture of the roadarm was caused by a combination of too high a carbon equivalent in the castings and the lack of preheating and postheating during the welding procedure. A pre-heat and tempering after welding were added to the welding procedure.

Abstract
Nodular cast iron crankshafts and their main-bearing inserts were causing premature failures in engines within the first 1600 km (1000 mi) of operation. The failures were indicated by internal noise, operation at low pressure, and total seizing. Concurrent with the incidence of engine field failures was a manufacturing problem: the inability to maintain a similar microfinish on the cope and drag sides of a cast main-bearing journal. Investigation supported the conclusion that the root cause of the failure was carbon flotation due to the crankshafts involved in the failures showing a higher-than-normal carbon content and/or carbon equivalent. Larger and more numerous cope side graphite nodules broke open, causing ferrite caps or burrs. They then became the mechanism of failure by breaking down the oil film and eroding the beating material. A byproduct was heat, which assisted the failure. Recommendations included establishing closer control of chemical composition and foundry casting practices to alleviate the carbon-flotation form of segregation. Additionally, some nonmetallurgical practices in journal-finishing techniques were suggested to ensure optimal surface finish.

Abstract
A section of cast iron water main pipe contained a hole approximately 6.4 x 3.8 cm (2.5 x 1.5 in.). The pipe was laid in clay type soil. Examination revealed severe pitting around the hole and at the opposite side of the outside diam. A macroscopic examination of a pipe section at the hole area showed that the porosity extended a considerable distance into the pipe wall. Metallographic examination revealed a graphite structure distribution expected in centrifugally cast iron with a hypoeutectic carbon equivalent. Chemical analyses of a nonporous sample had a composition typical of cast iron pipe. Chemical analyses of the porous region had a substantial increase in carbon, silicon, phosphorus, and sulfur. The porous appearance and the composition of the soft porous residue confirmed graphitic corrosion. The selective leaching of iron leaves a residue rich in carbon, silicon, and phosphorus. The high sulfur content is attributed to ferrous sulfide from a sulfate reducing bacteria frequently associated with clay soils. Reinforced coal tar protective coating was recommended.

Abstract
Door-closer cylinder castings manufactured of class 30 gray iron were breaking during machining. The manufacturing source reported that a random sampling of castings from this lot had hardnesses from 180 to 210 HRB. Based on the color of the components, heat treatment of these castings was suspected. Metallurgical examination on two representative castings supported the conclusions that the cracks in these gray iron door closers that were present either before or during the heat treatment were attributed to a substandard microstructure of the wrong type of graphite combined with excessive ferrite. This anomalous structure is caused by shortcomings in the foundry practice of chemical composition, solidification, and inoculation control. Judging from the microstructure, the strength of the material was lower than desired for class 30 gray iron, and the suspected heat treatment further reduced the strength. Recommendations included that the chemistry and inoculation should be controlled to produce type A graphite structure. The chemistry control should aim for a carbon equivalent close to 4.3% to achieve adequate fluidity for thin sections and to alleviate gas defects.

Abstract
A spherical carbon steel fixed-catalyst bed reactor, fabricated from French steel A42C-3S, approximately equivalent to ASTM A201 grade B, failed after 20 years of service while in a standby condition. The unit was found to contain primarily hydrogen at the time of failure. The vessel had a type 304 stainless steel shroud around the catalyst bed as protection against the overheating that was possible if the gas bypassed the bed through the refractory material. The failure was observed to have begun at the toe of the shroud-support ring weld. The ring was found to have a number of small cracks at the root of the weld. The cleavage mode of fracture was confirmed by SEM. The presence of extensive secondary cracking and twinning (Neumann bands) where the fracture followed the line of the shroud-support ring was revealed by metallography. It was revealed by refinery maintenance records that the ring had been removed for hydrotest and welded without any postweld heat treatment. The final cause of failure was concluded to be cracking that developed during the installation of the new shroud ring. Stress-relief heat treatments were recommended to be performed to reduce residual-stress levels after welding.

Abstract
Two tanks made of AISI type 304 stainless steel exhibited cracking in the heat-affected zone (HAZ) of the weld that joined the dished end and the shell. The dished ends had been produced by cold deformation. Hardness measurement and simulation tests showed that the deformation was equivalent to a 30% reduction in thickness. Residual stresses were measured at about 135 MPa (20 ksi). The HAZ was found to be sensitized. The tanks had been stored in a coastal atmosphere for about 4 years before installation. The failure was attributed to intergranular stress-corrosion cracking in a sensitized HAZ due to chloride from the environment. Use of low-carbon type AISI 304L was recommended. Minimization of fit-up stresses and covering with polyethylene sheets during storage were also suggested.

Abstract
During the construction of a large-diam pipeline, several girth welds had to be cut out as a result of radiographic interpretation. The pipeline was constructed of 910 mm (36 in.) diam x 13 mm (0.5 in.) wall thickness grade X448 (x65) line pipe. The girth welds were fabricated using standard vertical down stove pipe-welding procedures with E7010 cellulosic electrodes. The crack started partially as a result of incomplete fusion on the pipe side wall, which in turn was a result of misalignment of the two pipes. The crack was typical of hydrogen cracking. Girth welds can be made using cellulosic electrodes. For high-risk girth welds, an increase in preheat and/or a reduction in the local stress by controlling lift height or depositing the hot pass locally before lifting may be required.

Abstract
During a refit of a twenty-year-old Naval destroyer, two cracks were found on the inside of the killed carbon-manganese steel hull plate at the forward end of the boiler room. The cracks coincided with the location of the top and bottom plates of the bilge keel. Metallurgical examination of sections cut from the cracked area identified lamellar tearing as the principle cause of the cracking. This was surprising in 6 mm thick hull plates. Corrosion fatigue and general corrosion also contributed to hull plate perforation. Although it is probable that more lamellar tears exist near the bilge keel in other ships and may be a nuisance in the future, the hull integrity of the ships is not threatened and major repairs are not needed.

Abstract
The genesis of failure of 6.1 mm thick electric resistance welded API 5L X-46 pipes during pretesting at a pressure equivalent to 90% of specified minimum yield strength was investigated. Cracks were found to initiate on the outer surface of the pipes in the fusion zone and propagate along the through-thickness direction. The presence of extensive decarburization and formation of a soft ferrite band within the fusion zone may have contributed to the nucleation of the cracks. Crack propagation was aided by the presence of exogenous inclusions entrapped within the fusion zone. Analysis of these inclusions confirmed the presence of Fe, Si, Ca, and O, indicating slag entrapment to be the most probable culprit.

Abstract
An intermediate shaft (3 in. diam), part of a camshaft drive on a large diesel engine, broke after two weeks of service. Failure occurred at the end of the taper portion adjacent to the screwed thread. The irregular saw-tooth form of fracture was characteristic of failure from torsional fatigue. A second shaft carried as spare gear was fitted and failure took place in a similar manner in about the same period of time. Examination revealed that the tapered portion of the Fe-0.6C carbon steel shaft had been built up by welding prior to final machining. A detailed check by the engine-builder established that the manufacture of these two shafts had been subcontracted. It was ascertained that the taper portions had been machined to an incorrect angle and then subsequently built-up and remachined to the correct taper. The reduction in fatigue endurance following welding was due to heat-affected zone cracking, residual stresses, the lower fatigue strength of the weld deposited metal, and weld defects.

Abstract
During the final shop welding of a large armature for a direct-current motor (4475 kW, or 6000 hp), a loud bang was heard, and the welding operation stopped. When the weld was cold, nondestructive evaluation revealed a large crack adjacent to the root weld. Investigation showed the main crack had propagated parallel to the fusion boundary along the subcritical HAZ and was associated with long stringers of type II manganese sulfide (MnS) inclusions. This supported the conclusion that the weld failed by lamellar tearing as a result of the high rotational strain induced at the root of the weld caused by the weld design, weld sequence, and thermal effects. Recommendations included removing the old weldment to a depth beyond the crack and replacing this with a softer weld metal layer before making the main weld onto the softer layer.

Abstract
Six cracked A36 steel gusset plates that formed part of the roof trusses of a large manufacturing facility were discovered during a routine final inspection of a new building construction. Two different-size plates from different locations in the building were removed and tested. It was determined that the gusset plates failed in the heat-affected zone via an intergranular microcracking mode due to hydrogen-assisted underbead and toe-weld cracking. Proper nondestructive testing techniques for magnetic particle and radiographic inspection of the plate-weld gusset areas were recommended.

Abstract
A paper drier head manufactured from gray cast iron was removed from service as a result of NDE detection of crack-like surface discontinuities. This component was subjected to internal steam pressure to provide heat energy for drying. Investigation (visual inspection, chemical analysis, mechanical testing, as-polished 54x magnification, etched with nital 33x/54x/215x/230x magnification) supported the conclusions that the NDE indications were the consequence of a cold-shut condition in the casting. The cold shut served as a stress-concentration site and was therefore a potential source of crack initiation. The combination of low material strength and a casting defect was a potential source of unexpected fracture during service, because the component was under pressure from steam. Recommendations included removing other dryer heads exhibiting similar discontinuities and/or material quality from service.

Abstract
The repeated failure of a welded ASTM A283 grade D pipe that was part of a 6 km (4 mi) line drawing and conducting river water to a water treatment plant was investigated. Failure analysis was conducted on sections of pipe from the third failure. Visual, macrofractographic, SEM fractographic, metallographic, chemical, and mechanical property (tension and impact toughness) analyses were conducted. On the basis of the tests and observations, it was concluded that the failure was the combined result of poor notch toughness (impact) properties of the steel, high stresses in the joint area, a possible stress raiser at the intersection of the spiral weld and girth weld, and sudden impact loading, probably due to water hammer. Use of a semi- or fully killed steel with a minimum Charpy V-notch impact value of 20 J (15 ft·lbf) at 0 deg C (32 deg F) was recommended for future water lines. Certified test results from the steel mill, procedure qualification tests of the welding, and design changes to reduce water hammer were also recommended.

Abstract
The information provided in this article is intended for those individuals who want to determine why a casting component failed to perform its intended purpose. It is also intended to provide insights for potential casting applications so that the likelihood of failure to perform the intended function is decreased. The article addresses factors that may cause failures in castings for each metal type, starting with gray iron and progressing to ductile iron, steel, aluminum, and copper-base alloys. It describes the general root causes of failure attributed to the casting material, production method, and/or design. The article also addresses conditions related to the casting process but not specific to any metal group, including misruns, pour shorts, broken cores, and foundry expertise. The discussion in each casting metal group includes factors concerning defects that can occur specific to the metal group and progress from melting to solidification, casting processing, and finally how the removal of the mold material can affect performance.

Hardenability evaluation is typically applied to heat treatment process control, but can also augment standard metallurgical failure analysis techniques for steel components. A comprehensive understanding of steel hardenability is an essential complement to the skills of the metallurgical failure analyst. The empirical information supplied by hardenability analysis can provide additional processing and service insight to the investigator. The intent of this paper is to describe some applications of steel thermal response concepts in failure analysis, and several case studies are included to illustrate these applications.

Abstract
One of the connecting rods of a vertical, four-cylinder engine with a cylinder diameter of 5 in. failed by fatigue cracking just below the gudgeon-pin boss. Failure took place in line with the lower edge of a deposit of weld metal. The fracture surface was smooth, conchoidal, and characteristic of that resulting from fatigue. The origin of the major crack was associated with a crescent-shaped area immediately below the weld deposit. This showed brittle fracture characteristics and appeared to be an initial crack that occurred at the time of welding and from which the fatigue crack subsequently developed. The rod was made from a medium carbon or low-alloy steel in the hardened and fully tempered condition. Evidence indicated that, following modification to the oil feed system, the rod that broke was returned to service with fine cracks present immediately below the weld deposit, which served as the starting points of the fatigue cracks. Following this accident, the remaining three rods (which had been modified in a similar manner) were replaced as a precautionary measure.

Abstract
The crankshaft of a compressor fractured through the web remote from the driving end after about three years of service. The fracture ran diagonally across the web into the crankpin. It passed through the centers of two screwed plugs inserted into the web from opposite faces approximately in line with the crankpin center line. The fracture was of the fatigue type, slowly developing cracks having started from opposite sides of each tapped hole and crept across the section. Microstructure of the crankshaft indicated the material was a plain carbon steel, the carbon content being of the order of 0.3%. The failure resulted principally from the stress-raising effects of the screw holes combined with the cracks in the welds. If the screw holes had been left unfilled or if some form of mechanical locking had been used if plugged, failure would have been postponed if not averted.

Abstract
This article focuses on the general root causes of failure attributed to the casting process, casting material, and design with examples. The casting processes discussed include gravity die casting, pressure die casting, semisolid casting, squeeze casting, and centrifugal casting. Cast iron, gray cast iron, malleable irons, ductile iron, low-alloy steel castings, austenitic steels, corrosion-resistant castings, and cast aluminum alloys are the materials discussed. The article describes the general types of discontinuities or imperfections for traditional casting with sand molds. It presents the international classification of common casting defects in a tabular form.
