Abstract
A bent Ni-Cu Monel 400 alloy tube, which operated as part of a pipeline in a petrochemical distillery, failed by through-thickness cracking. The pipeline was used to carry a stream of gaseous hydrocarbons containing hydrochloric acid (HCl) into a reaction tower. The tower provided a caustic solution (NaOH) to remove HCl from the stream, before the latter was directed to a burner. Metallographic examination showed that the cracks were intergranular and were frequently branched. Although nominal chemical composition of the component was found within the specified range, energy dispersive x-ray analysis (EDXA) indicated significant segregation of sulfur and chlorine along the grain boundaries. Failure was attributed to hypochlorous-acid (HClO)-induced stress-corrosion cracking (SCC). The HClO was formed by the reaction of HCl with atmospheric O 2 that entered the tube during shutdowns and startups. Residual stresses, originating from in situ bend forming of the tube during assembly of the line, provided a driving force for crack growth, and the segregation of sulfur on grain boundaries made the material more susceptible to cracking.

Abstract
Several of the aluminum alloy 6061-T6 drawn seamless tubes (ASTM B 234, 2.5 cm (1.0 in.) OD with wall thickness of 1.7 mm (0.065 in.)) connecting an array of headers to a system of water-cooling pipes failed. The tubes were supplied in the O temper. They were bent to the desired curvature, preheated, then solution treated, water quenched, and then aged for 8 to 10 h. Analysis (visual inspection, slow-bend testing, 65x macrographic analysis, macroetching, spectrographic analysis, hardness tests, microhardness tests, tension tests, and microscopic examination) supported the conclusions that bending of the connector tubes in the annealed condition induced critical strain near the neutral axis of the tube, which resulted in excessive growth of individual grains during the subsequent solution treatment. Recommendations included bending the connector tubes in the T4 temper as early as possible after being quenched from the solution temperature. The tubes should be stored in dry ice after the quench until bending can be done. The tubes should be aged immediately after being formed. Flattening and slow-bend tests should be specified to ensure that the connector tubes had satisfactory ductility.

Abstract
A steam-condensate line (type 316 stainless steel tubing) began leaking after five to six years in service. The line carried steam condensate at 120 deg C (250 deg F) with a two hour heat-up/cool-down cycle. No chemical treatment had been given to either the condensate or the boiler water. To check for chlorides, the inside of the tubing was rinsed with distilled water, and the rinse water was collected in a clean beaker. A few drops of silver nitrate solution were added to the rinse water, which clouded slightly because of the formation of insoluble silver chloride. This and additional investigation (visual inspection, and 250x micrograph etched with aqua regia) supported the conclusion that the tubing failed by chloride SCC. Chlorides in the steam condensate also caused corrosion of the inner surface of the tubing. Stress was produced when the tubing was bent during installation. Recommendations included providing water treatment to remove chlorides from the system. Continuous flow should be maintained throughout the entire tubing system to prevent concentration of chlorides. No chloride-containing water should be permitted to remain in the system during shutdown periods, and bending of tubing during installation should be avoided to reduce residual stress.

Abstract
A tubular heat exchanger in a refinery reformer unit leaked after one month of service. The exchanger contained 167 type 304 stainless steel U-bent integral-finned tubes. Cracks in the tube wall were revealed during examination. Hardness of the tube was found to be 30 HRC at the inside surface and up to 40 HRC at the base of the fin midway between the roots which indicated that the fins were cold formed and not subsequently annealed thus susceptible to SCC because of a high residual stress level. It was revealed by metallographic examination that the fracture was predominantly by transgranular branched cracking and had originated from the inside surface. It was concluded that the tubes failed in SCC caused by chlorides in the presence of high residual stresses. The finned tubes were ordered in the annealed condition as a corrective measure.

Abstract
An aftercooler was of conventional design and fitted with brass tubes through which cooling-water circulated. Air at 100 psi pressure was passed over the outsides of the tubes, entering the vessel near to the upper tubeplate on one side and leaving it by a branch adjacent to the lower tubeplate on the opposite side. After a mishap, the paint had been burned off the upper half of the shell. Internally, most of the tubes were found to be twisted or bent. The casing of the pump used to circulate the cooling water was also found to be cracked after the mishap. All the evidence pointed to the probability that a fire had occurred within the vessel. Some months before the failure, one of the tubes situated towards the center of the nest developed a leak. Owing to the difficulty of inserting a replacement tube, the defective one was scaled by means of a length of screwed rod fitted with nuts and washers at each end. This assembly became loose, thereby allowing air under pressure to enter the waterside of the cooler and expel the water, leading to overheating and ultimately to the damage described.

Abstract
In a copper hot water system, a bent pipe was soldered into a straight pipe with twice the diameter. The neighborhood of the soldered joint was covered with corrosion product predominantly blue-green in color, presumably carbonates. When these corrosion products were scratched off it was seen that the copper beneath this layer had not suffered noticeable attack. The object of the examination was the localized deep cavities located almost symmetrically to both sides of the inserted end of the narrower tube on the internal wall of the wider tube which had in one place been eaten right through. The symmetrical location on each side of the point of insertion of the narrower pipe and the localized sharp delineation of the attack indicated erosion due to the formation of turbulence. By avoiding sharp transitions and abrupt changes in cross section it is possible to design the pipe work so that localized turbulence is obviated. Degassing and cleansing of the water also would reduce the danger of erosion particularly in the case of softened water, which takes up oxygen and carbon dioxide very readily thus becoming particularly aggressive.

A seamless hot-drawn boiler tube NW 300 of 318 mm OD and 9 mm wall thickness made of steel 15Mo3 was bent with sand filling after preheating allegedly to 1000 deg C. In the process it had cracked repeatedly in the drawn fiber. The composition corresponded to specifications, but exceptionally high copper content was noticeable. Microstructural examination showed the damage was due to overheating and burning during preheating and bending. Furthermore, crack formation was promoted by precipitation of metallic copper that had penetrated into the austenitic grain boundaries under the influence of tensile stresses that arose during bending. This phenomenon is known as “solder brittleness.”

Abstract
The drive-shaft hanger bearings failed after 300 to 400 h in service. The shaft, supported by labyrinth-sealed single row radial ball bearings of ABEC-1 tolerances, was made of aluminum 2024-T3 tubing (2.5 cm diam and 1.2 mm wall thickness). The bearings were lubricated with a paste-type mineral-oil lubricant (containing molybdenum disulfide and polytetrafluoroethylene particles) or grease conforming to MIL-G-81322 (containing thickening agent and synthetic hydrocarbons) and had two-piece spot-welded retainers. On visual examination, the balls were observed to be embedded in the inner-ring raceway which had been softened by the elevated temperatures reached during the failure. Broken retainers and worn and bent out of shape seals were found. Penetration of gritty particles, water and other corrosive agents and leakage of lubricant out of the bearing permitted by the worn seals was observed. It was concluded that overheating was caused by lubricant flow was permitted by wear of the labyrinth seals. Positive rubbing seals and MIL-G-81322 grease lubricant were found to have longer life than those with the labyrinth seals and mineral-oil-paste lubricant on testing under simulated environmental conditions and were installed as a corrective measure. Importance of dirt free supply and drainage of oil was discussed.

Abstract
On 21 April 1995, the contents of a large blender (6 cu m) reacted and caused an explosion that killed and injured a number of workers at a plant in Lodi, NJ. A mixture of sodium hydrosulfite and aluminum powder was being mixed at the time of the accident. This report focuses on evaluations of the blender to determine if material or mechanical failures were the cause of the accident. The results indicate that the mixing vessel was metallurgically sound and did not contribute to the initiation of the failure. However, the vessel was not designed for mixing chemicals that must be isolated from water and excessive heat. Water leaking into the vessel through a graphite seal may have initiated the reactions that caused the accident.

Abstract
The wires used in a wet precipitator for cleaning the gases coming off a basic oxygen furnace failed. The system consisted of six precipitators, three separate dual units, each composed of four zones. Each zone contained rows of wires (cold drawn AISI 1008 carbon steel) suspended between parallel collector plates. It was determined that the 1008 wires failed because of corrosion fatigue. It was decided to replace all of the wires in the two zones with the highest rates of failure with cold-drawn type 304 austenitic stainless steel wire. These expensive wires, however, failed after a week by transgranular SCC. Annealed type 430 ferritic stainless steel was subsequently suggested to prevent further failures.

Abstract
Coiled tubing used in drilling operations failed at the halfway point of its estimated fatigue life. The failure was found to be transverse to the tubing axis. Visual examination revealed a flat fracture surface extending 13 mm with the rest of the fracture showing shear lips indicative of tensile overload. The flat portion of the fracture surface was typical of fatigue cracking. Fatigue striations were revealed by SEM. Corrosion pitting on the tubing ID from which the fatigue crack had propagated were observed on closer examination. The corrosion pitting was speculated to have occurred when the tubing was idle and fluids accumulated at the bottom of the tubing wraps. The coiled tubing was concluded to have failed prematurely due to low-cycle fatigue initiated at corrosion pitting sites. Corrosive attack on the coiled tubing was recommended to be reduced by completely removing fluids or modifying the fluids in the tubing or purging by flowing dry nitrogen to dry it out.

Abstract
Several type 304L (UNS S30400) stainless steel seamless tubes in a high-pressure synthesis gas cooler condensing ammonia in a fertilizer plant leaked in an unexpectedly short time. Representative samples of the tubes were subjected to chemical analysis, hardness tests, and optical microscopy examination. The tests revealed that the tubes conformed to specification. Crack morphology indicated stress-corrosion cracking by chlorides present in the cooling water. Use of a duplex stainless steel (for example, UNS S32304 S31803) as a tube material was recommended.

Abstract
Both rods in a Harrington rod cervical stent failed after a short time in service. Metallurgical analysis revealed a significant number of notches as well as enlarged grain size in one of the two rods, rough shallow-cracked surfaces along the bend profiles, possible signs of corrosion, and fractures (on both rods) near indentations imparted by retaining clamps. The observations suggest that surface roughness and bending defects initiated cracking that led to the fatigue failure of the compromised rod, followed some time later by the overload fracture of the second rod.

Abstract
A process vessel heating coil, consisting of several 3 ft diam turns, was supplied with steam at 400 psi and a temperature of 343 deg C (650 deg F). At bi-weekly intervals well water was introduced to effect rapid cooling of the contents. After about eight months, leakage developed from a circumferential crack on the underside of the uppermost turn. Shorter cracks were found at a similar location on the bottom turn, and further leakage occurred at pinhole perforations adjacent to the crack in the top turn and near to a butt-weld in the coil. Microscopic examination revealed that the cracks were predominantly of the intergranular variety. In addition, transgranular cracks were present. Material was an austenitic stainless steel of the type specified but the absence of columbium and titanium in significant amounts showed that it was not stabilized against intergranular carbide precipitation. The transgranular cracks indicated that failure was due partly to stress-corrosion. It was concluded that the chlorides provided the main corrodent for both the stress and intercrystalline-corrosion cracking.

Abstract
A new crane failed during the overload test following erection. A test load of 5 tons at the end of the jib (rated capacity 4 tons) was in the process of being slewed at the time of this failure. Inspection revealed that the collapse had resulted from the opening out of one eye of the rimming steel tie-bar of the main jib at the lower splice. This permitted the pin to pass through and allowed the jib to fall. Examination subsequently revealed that brittle fracture of two of the corner angles of the tower head assembly had also occurred. Had the tie-bar material been of satisfactory quality and/or, if the end that failed had been flamecut instead of sheared, then the damage resulting from the excessive overload would have been limited to yielding of the material in the region of the pin-joint. Such yielding on an overload test further indicated that the scantlings of the pin-joints were inadequate. Two other crane failures showed that failure resulted from the use of rimming steel, and embrittlement of the material was evident.

Abstract
An 850 ft. long steam main working at a pressure of 120 psi ruptured. Two lengths of pipe were submitted for examination, one containing the rupture and the other from an unaffected part removed to facilitate repair. The rupture surfaces were smooth, suggesting that the failure had taken place either at a weld in the pipe or at a discontinuity in the material. Microscopic examination through the joints at the ends of the rupture confirmed that the pipe had been made from strip and the edges lap-welded. The second case concerned an 8 in. diam pipe in which a longitudinal defect was discovered running the entire length. Examination of a section through the pipe containing the defect, showed this to be a scarf-welded pipe, only about half the section of the scarf having been satisfactorily welded together. It was concluded that both pipes had been defective at the time of manufacture and that service conditions had served to extend the defects.

Abstract
A compression hip screw is a device designed to hold fractures in the area of the femur in alignment and under compression. A side plate, which is an integral part of the device, is attached by screws to the femur, and it holds the compression screw in position. The device analyzed had broken across the eighth hole (of nine holes) from the end of the plate. The detailed metallurgical failure analysis of the device, including metallography and fractography, is reported here. It was found that the device had adequate metallurgical integrity for the application for which it was intended. It is believed that failure was caused by the lack of a screw in the ninth hole. Evidence is also presented which indicates that the device was bent prior to insertion, and the local plastic deformation may have caused structural changes leading to premature crack initiation.

Abstract
The curved parts of exit pigtails made of wrought Incoloy 800H tubing used in steam reforming furnaces failed by performance after a period of service shorter than that predicted by the designers. Examination of a set of tubes consisting of both curved (perforated) and straight parts revealed that the cracks initiated at the outer surface by a combined mechanism of creep and intergranular embrittlement. A smaller grain size resulting from cold bending fabrication procedures for the curved parts was responsible for accelerating the embrittlement. It was recommended that hot bending be used for fabrication of the curved parts. A change of alloy to a low-alloy chromium-molybdenum allay to protect against heat was also suggested.
