Abstract
The dished ends of a heavy water/helium storage tank manufactured from 8 mm (0.3 in.) thick type 304 stainless plate leaked during hydrotesting. Repeated attempts at repair welding did not alleviate the problem. Examination of samples from one dished end revealed that the cracking was confined to the heat affected zone (HAZ) surrounding circumferential welds and, to a lesser extent, radial welds that were part of the original construction. Most of the cracks initiated and propagated from the inside surface of the dished ends. Microstructures of the base metal, HAZ, and weld metal indicated severe sensitization in the HAZ due to high heat input during welding. An intergranular corrosion test confirmed the observations. The severe sensitization was coupled with residual stresses and exposure of the assembly to a coastal atmosphere during storage prior to installation. This combination of factors resulted in failure by stress-corrosion cracking. Implementation of a new repair procedure was recommended. Repairs were successfully made using the new procedure, and all cracks in the weld repair zones were eliminated.

Abstract
Nickel anodes failed in several electrolysis cells in a heavy-water upgrading plant. Dismantling of a cell revealed gouging and the presence of loosely attached black porous masses on the anode. The carbon steel top, plate was severely corroded. An appreciable quantity of black powder was also present on the bottom or the cell. SEM/EDX studies of the outer and inner surfaces of the gouged anode showed the presence of iron globules at the interface between the gouged and the unattacked anode. The chemical composition of the black powder was determined to be primarily iron. Cell malfunction was attributed to the accelerated dissolution of the carbon steel anode top, dislodgment of grains from the material, and subsequent closing of the small annular space between the anode and the cathode by debris from the anode top. Cladding of the carbon steel top with a corrosion-resistant material, such as nickel, nickel-base alloy, or stainless steel, was recommended.

Abstract
A detailed failure analysis was conducted on an ammonia refrigerant condenser tube component that failed catastrophically during its initial hours of operation. Evidence collected clearly demonstrated that the weld between a pipe and a dished end contained a sharp unfused region at its root (lack of penetration). Component failure had started from this weld defect. The hydrogen absorbed during welding facilitated crack initiation from this weld defect during storage of the component after welding. Poor weld toughness at the low operating temperature facilitated crack growth during startup, culminating in catastrophic failure as soon as the crack exceeded critical length.

Abstract
The presence of secondary, branching intergranular stress-corrosion cracking in a type 440C stainless bearing caused the analyst to overlook the real culprit, which was a mechanically-initiated, primary transgranular crack that propagated through the steel's hard chromium carbide. Failure was actually caused by overload. Had the original conclusion been accepted, a relatively exotic alloy would have been specified. In another case, brass heat exchanger tube failure was automatically attributed to attack by an acidic cleaner, and a decision was made to stop using the solution. A more thorough analysis showed failure was caused by tube vibration. In a third case, a type 304 stainless steel bellows in a test loop was thought to have failed because of chloride stress corrosion. The report concluded with a recommendation that carbon steel be used as an alternative bellows material. Caustic, not chloride, stress corrosion was the culprit. Had material substitutions been made on the original premise of countering chloride stress corrosion, most of the loop's highly stressed components would have eventually failed.

Abstract
An I-beam of IS-226 specification—I-section dimensions of 450 x l50 x 10 mm (17.7 x 5.9 x 0.4 in.) and a length of 12.41 m (40.7ft)—was flame cut into two section in an open yard near these a coast under normal weather conditions. After approximately 112h, the shorter section of he I-beam split catastrophically along the entire length through the web. Detailed investigation revealed segregation of high levels of carbon, sulfur and phosphorus in the middle of the web and high residual stresses attributed to rolling during fabrication. Flame cutting caused a change in the distribution of the residual stresses, which, aided by low fracture toughness due to the poor quality of the beam, resulted in failure. It was recommended that segregation be avoided in cast ingots used for I-beam manufacture by implementing a better quality-control procedure.

Abstract
Extensive cracking was found in a batch of die-cast ZAMAK 3 solenoid valve seats during commissioning of the system in which they were installed. Scanning electron microscopic and chemical analyses conducted on one of the failed valve seats showed that the composition of the alloy was different from that specified. The presence of excess aluminum and lead impurities that had segregated to the grain boundaries, coupled with an inadequate amount of magnesium, resulted in intergranular corrosion and subsequent intergranular failure. Corrosion was accelerated by storage in a humid environment in a coastal area. It was recommended that proper chemical analysis of the zinc-aluminum alloy be carried out as a quality control procedure.

Abstract
Four tanks made from type 304L stainless steel were removed from storage. Atmospheric corrosion on the outside of the tanks and pitting and crevice corrosion on the inside were visible. Metallographic examination revealed that the internal corrosion had been caused by crevices related to weld spatter and uneven weld deposit and by service water that had not been drained after hydrostatic testing. External corrosion was attributed to improper passivation. It was recommended that the surfaces be properly passivated and that, before storage, the interiors be rinsed with demineralized water and dried.

Abstract
A cast housing, part of a multi-shaft yoking mechanism, failed during assembly and installation of the equipment in which it was to be used. The housing, or yoke body, was cast from AISI 420 grade ferritic stainless steel. Analysis revealed that the failure was caused by the presence of shrinkage cavities, which lowered the load-bearing capability. The failure occurred at the location where there was an abrupt change in the section thickness. A redesign to provide a smooth contour at the section junction was recommended along with optimization of casting parameters to avoid shrinkage cavities.

Abstract
Leaks developed in 22 admiralty brass condenser tubes. The tubes were part of a condenser that was being used to condense steam from a nuclear power plant and had been in operation for less than 2 years. Analysis identified three types of failure modes: stress-corrosion cracking, corrosion under deposit (pitting and crevice), and dezincification. Fractures were transgranular and typical of stress-corrosion cracking. The primary cause of the corrosion deposit was low-flow conditions in those parts of the condenser where failure occurred. Maintenance of proper flow conditions was recommended.

Abstract
A copper condenser dashpot in a refrigeration plant failed prematurely. The dashpot was a long tubular component with a cup brazed at each end. Stereomicroscopic examination of the fracture surface at low magnification revealed a typical ductile mode of failure. The failure was attributed to insufficient component thickness, which made the dashpot unable to withstand internal operating pressure, and to extensive annealing in the heat-affected zones of the brazed joints. It was recommended that the condenser dashpot design take into account the annealing effects of brazing. Hydrostatic testing at a pressure times greater than the maximum operating pressure prior to placing the component in service was also suggested.

Abstract
AM350 stainless steel bellows used in the control rod drive mechanism of a fast breeder reactor failed after 1000 h of service in sodium at 550 deg C (1020 deg F). Helium leak testing indicated that leaks had occurred at various regions of the welded joints between the convolutes in the bellows. The weld failure was attributed to poor quality assurance during fabrication, which resulted in cracklike openings at the fusion zone. The openings extended during tensile loading. Use of proper welding procedures and quality control measures were recommended to prevent future failures.

Abstract
Two AISI type 316 stainless steel dished ends failed through the formation of intergranular stress-corrosion cracks (IGSCC) within a few months of service. The dished ends failed in the straight portions near the circumferential welds that joined the ends to the cylindrical portions of the vessel. Both dished ends were manufactured from the same batch and were supplied by the same manufacturer One of the dished ends had been exposed to sodium at 550 deg C (1020 deg F) for 500 h before failure due to sodium leakage was detected. The other dished end was used to fabricate a second vessel that was kept in storage for 1 year Clear evidence of sensitization was found in areas where IGSCC occurred. Sensitization was extensive in the dished end that had been exposed to sodium at high temperature, and it occurred in a narrow band similar to that typical of weld decay in the dished end that had been kept in storage. Solution annealing was recommended to relieve residual stress, thereby reducing the probability of failure. It was also recommended that the carbon content of the steel be lowered, i.e., that a 316L grade be used.

Abstract
Several type 304L stainless steel dished ends used in the fabrication of cylindrical vessels developed extensive cracking during storage. All of the dished ends had been procured from a single manufacturer and belonged to the same batch. When examined visually, several rust marks were observed, indicating contamination by rusted carbon steel particles. Liquid penetrant testing was used to determine the extent of the cracks, and in situ metallographic analysis was performed over the cracked region. The morphology of the cracks was indicative of transgranular stress-corrosion cracking (TGSCC). Conditions promoting the occurrence of the TGSCC included significant tensile stresses on the inside of the dished ends, the presence of surface contamination by iron due to poor handling practice using carbon steel implements, and storage in a coastal environment with an average temperature of 25 to 32 deg C (77 to 90 deg F), an average humidity ranging from 70 to 80%, and an atmospheric NaCl content ranging from 8 to 45 mg/m2 /day. Recommendations preventing further occurrence of the situation were strict avoidance of the use of carbon steel handling implements, strict avoidance of cleaning practices that cause long-term exposure to chlorine-containing cleaning fluid, and solution annealing of the dished ends at 1050 deg C (1920 deg F) for 1 h followed by water quenching to relieve residual stresses.

Abstract
Pressure testing of a batch of AISI type 316L stainless steel thermowells intended for use in a nuclear power-plant resulted in the identification of leakage at the tips in 20% of the parts. Radiography at the tip region of representative thermowells showed linear indications along the axes. SEM examination revealed the presence of longitudinally oriented nonmetallic inclusions that were partly retained and partly dislodged. Electron-dispersive x-ray analysis indicated that the inclusions were composed of CaO. Based on the overall chemistry of the inclusion sites, the source of the CaO was determined to be slag entrapment during the steel making process. It was recommended that the thermowell blanks be ultrasonically tested prior to machining and that the design be modified to make internal pressurization possible.

Abstract
Leaks developed at random locations in aluminum brass condenser tubes within the first year of operation of a steam condenser in a nuclear power plant. One failed tube underwent scanning electron microscopy surface examination and optical microscope metallography. It was determined that the tube failed from crevice corrosion under seawater deposits that had formed on the inner surface. Mechanical cleaning of the condenser tubes every 6 months and installation of intake screens of smaller mesh size were recommended.

Abstract
A section of type 304 stainless steel pipe from a stand by system used for emergency injection of cooling water to a nuclear reactor failed during precommissioning. Leaking occurred in only one spot. Liquid penetrant testing revealed a narrow circumferential crack. Metallographic examination of the cracked area indicated stress-corrosion cracking, which had originated at rusted areas that had formed on longitudinal scratch marks on the outer surface of the pipe. The material was free from sensitization, and there was no significant amount of cold work. It was recommended that the stainless steel be kept rust free.

Abstract
Two tanks made of AISI type 304 stainless steel exhibited cracking in the heat-affected zone (HAZ) of the weld that joined the dished end and the shell. The dished ends had been produced by cold deformation. Hardness measurement and simulation tests showed that the deformation was equivalent to a 30% reduction in thickness. Residual stresses were measured at about 135 MPa (20 ksi). The HAZ was found to be sensitized. The tanks had been stored in a coastal atmosphere for about 4 years before installation. The failure was attributed to intergranular stress-corrosion cracking in a sensitized HAZ due to chloride from the environment. Use of low-carbon type AISI 304L was recommended. Minimization of fit-up stresses and covering with polyethylene sheets during storage were also suggested.

Abstract
Upon arrival at the erection site, an AISI type 316L stainless steel tank intended for storage of fast breeder test reactor coolant (liquid sodium) exhibited cracks on its shell at two of four shell/nozzle fillet-welded joint regions. The tank had been transported from the manufacturer to the erection site by road, a distance of about 800 km (500 mi). During transport, the nozzles were kept at an angle of 45 deg to the vertical because of low clearance heights in road tunnels. The two damaged joints were unsupported at their ends inside the vessel, unlike the two uncracked nozzles. Surface examination showed ratchet marks at the edges of the fracture surface, indicating that loading was of the rotating bending type. Electron fractography using the two-stage replica method revealed striation marks characteristic of fatigue fracture. The striations indicated that the cracks had advanced on many “mini-fronts,” also indicative of nonuniform loading such as rotating bending. It was recommended that a support be added at the inside end of the nozzles to rigidly connect with the shell. In addition to avoiding transport problems, this design modification would reduce fatigue loading that occurs in service due to vibration of the nozzles during filling and draining of the tank.

Abstract
A number of AISI 347 stainless steel bellows intended for use in the control rod drive mechanism of a fast breeder reactor were found to be leaking before being placed in service. The bellows, which had been in storage for one year in a seacoast environment, exhibited a leak rate on the order of 1 x 10−7 cu cm/s (6 x 10−8 cu in./s). Optical metallography revealed numerous pits and cracks on the surfaces of the bellow convolutes, which had been welded to one another using an autogenous gas tungsten arc welding process. Microhardness measurements indicated that the bellows had not been adequately stress relieved. It was recommended that a complete stress-relieving treatment be applied to the formed bellows. Improvement of storage conditions to avoid direct and prolonged contact of the bellows with the humid, chloride-containing environment was also recommended.

Abstract
Repeated failures of high-pressure ball valves were reported in a chemical plant. The ball valves were made of AFNOR Z30C13 martensitic stainless steel. Initial examination of the valves showed that failure occurred in a weld at the ball/stem junction end of austenitic stainless steel sleeves that had been welded to the valve stem at both ends. Metallographic examination showed that a crack had been introduced into the weld by improper weld heat treatment. Stress concentration at the weld location resulting from an abrupt change in cross section facilitated easy propagation of the crack during operation. Proper weld heat treatment was recommended, along with avoidance of abrupt change in cross section near the weld. Due penetrant testing at the ball stem junction before and after heat treatment was also suggested.
