Abstract
The failure of a high-speed pinion shaft from a marine diesel engine was investigated. The shaft, which had been in service for more than 30 years, failed shortly after the bearings were replaced. Examination of the shaft revealed cyclic fatigue, with a substantial distribution of nonmetallic inclusions near the fracture initiation site. Fracture mechanics analysis indicated that, if stresses acting on the shaft were induced only by normal service loads, there was little likelihood that the inclusions served as failure initiation sites. Further examination of the bearing elements revealed an abnormal wear pattern, consistent with the application of elevated bending loads. The root cause of failure was determined to be an increase in service stresses after bearing replacement along with the presence of nonmetallic inclusions in the shaft.

Abstract
A high-speed pinion gear shaft, part of a system that compresses natural gas, was analyzed to determine why it failed. An abnormal wear pattern was observed on the shaft surface beneath the inner race of the support bearings. Material from the shaft had transferred to the bearing races, creating an imbalance (enough to cause noise and fumes) that operators noted two days before the failure. Macrofeatures of the fracture surface resembled those of fatigue, but electron microscopy revealed brittle, mostly intergranular fracture. Classic fatigue features such as striations were not found. To resolve the discrepancy, investigators created and tested uniaxial fatigue samples, and the microfeatures were nearly identical to those found on the failed shaft. The root cause of failure was determined to be fatigue, and it was concluded that cracks on the pinion shaft beneath the bearings led to the transfer of material.

Two intermediate impeller drive gears (made of AMS 6263 steel, gas carburized, hardened, and tempered) exhibited evidence of pitting and abnormal wear after production tests in test-stand engines. The gears were examined for hardness, case depth, and microstructure of case and core. It was found that gear 1 had a lower hardness than specified while the case hardness of gear 2 was found to be within limits. Both the pitting and the wear pattern were revealed to be more severe on gear 1 than on gear 2. Surface-contact fatigue (pitting) of gear 1 (cause of lower carbon content of the carburized case and hence lower hardness) was found to be the reason for failure. It was recommended that the depth of the carburized case on impeller drive gears be increased from 0.4 to 0.6 mm to 0.6 to 0.9 mm to improve load-carrying potential and wear resistance. A minimum case-hardness requirement was set at 81 HRA.

An AISI 4320 H transfer gear shaft that was part of a transmission sustained severe surface damage after 12 h of dynamometer testing at various gearing and torque loads. The damage was characterized by generalized wear and spalling. Examination of a cross section of the shaft that intersected undamaged, burnished, and surface-spalled zones revealed no anomalies in the chemistry, microstructure, or hardness that could have caused the damage. The physical evidence suggested that the operable mechanism was contact fatigue caused by misalignment of the shaft in the assembly.

Abstract
A carbon steel piping cross-tee assembly which conveyed hydrogen sulfide (H7S) process gas at 150 to 275 deg C (300 to 585 deg F) with a maximum allowable operating pressure of 3 MPa (450 psig) ruptured at the toe of one of the welds at the cross after several years of service. The failure was initially thought to be the result of thermal fatigue, and the internal surfaces exhibited the “elephant hide” pattern characteristic of thermal fatigue. However metallographic failure analysis found that this pattern was the result of corrosion rather than thermal fatigue. Corrosion caused failure at this location because the weld was abnormally thin as fabricated. Thus, failure resulted from inadequate deposition of weld metal and subsequent wall thinning from internal corrosion. It was recommended that the cross-tee be replaced with a like component, with more careful attention to weld quality.

Abstract
A cast countershaft pinion on a car puller for a blast furnace broke after one month of service; expected life was 12 months. The pinion was specified to be made of 1045 steel heat treated to a hardness of 245 HRB. The pinion steel was analyzed and was a satisfactory alternative to 1045 steel. The pinion was annealed before flame or induction hardening of the teeth to a surface hardness of 363 HRB and a core hardness of 197 HRB. The broken pinion had a tooth which had failed by fatigue fracture through the tooth root because of the low strength from incomplete surface hardening of the tooth surfaces. Contributing factors included uneven loading because of misalignment and stress concentrations in the tooth roots caused by tool marks. Greater strength was provided by oil quenching and tempering the replacement pinions to a hardness of 255 to 302 HRB. Machining of the tooth roots was revised to eliminate all tool marks. Surface hardening was applied to all tooth surfaces, including the root. Proper alignment of the pinion was ensured by carefully checking the meshing of the teeth at startup.

A ball bearing in a military jet engine sustained heavy damage and was analyzed to determine the cause. Almost all of the balls and a portion of the outer race were found to be flaking, but there were no signs of damage on the inner race and cage. Tests (chemistry, hardness, and microstructure) indicated that the bearing materials met the specification requirements. However, closer inspection revealed areas of discoloration, or nonuniform contact marks, on the ID surface of the inner ring. The unusual wear pattern suggested that the bearing was not properly mounted, thus subjecting it to uneven or eccentric loading. This explains the preferential nature of the flaking on the outer race and points to an assembly error as the root cause of failure.

Factors which may lead to premature roller bearing failure in service include incorrect fitting, excessive pre-load during installation, insufficient or unsuitable lubrication, over-load, impact load vibration, excessive temperature, contamination by abrasive matter, ingress of harmful liquids, and stray electric currents. Abstract
A flat spring for the main landing gear of a light aircraft failed after safe execution of a hard landing. The spring material was identified by chemical analysis to be 6150 steel. The fracture was found to have occurred near the end of the spring that was inserted through a support member about 25 mm thick and attached to the fuselage by a single bolt. Brinelling (plastic flow and indentation due to excessive localized contact pressure) was observed on the upper surface of the spring where the forward and rear edges of the spring contacted the support member. It was indicated by chevron marks that brittle fracture had started beneath the brinelled area at the forward edge of the upper surface of the spring. The origin of the brittle fracture was found to be a small fatigue crack that had been present for a considerable period of time before final fracture occurred. Fracture of the landing-gear spring was concluded to have been caused by a fatigue crack that resulted from excessive brinelling at the support point. Regular visual examinations to detect evidence of brinelling and wear at the support in aircraft with this configuration of landing-gear spring were recommended.

Abstract
Spalled fragments from the work rolls of a steel bar straightening machine were received for failure analysis. Visual inspection coupled with optical and scanning electron microscopy were used as the principal analytical techniques for the investigation. Fractographic analysis revealed the presence of a characteristic fatigue crack propagation pattern (beach marks) and radial chevron marks indicating the occurrence of final overload through a brittle intergranular fracture. The collected evidence suggests that surface-initiated cracks propagated by fatigue led to spalling, resulting in severe work roll damage as well as machine downtime and increased maintenance costs.

This case study involves two continuously cast steel crankshaft failures. Three parties performed their own failure analyses: (1) the engine manufacturer responsible for component design, specification, and application; (2) the steel supplier and forging supplier responsible for making the steel, forging the shape, and preliminary heat treatment; and (3) a supplier that provided induction hardening, finish machining, and inspection. An independent engineering firm was subsequently involved, but because each party had its own agenda, there was no agreement on the metallurgical source of the failure and thus no continued analysis to pin down and eliminate the root cause.

Abstract
The case study presented in this article details the failure investigation of an M50 alloy steel bearing used in a jet engine gearbox drive assembly. It discusses the investigative steps and analytic tools used to determine the root cause, highlighting the importance of continuous, thorough questioning by the investigating activity. The combined analyses demonstrated that the bearing failed by a single event overload as evidenced by bulk deformation and traces of foreign material on the rolling elements. The anomalous transferred metal found on the rolling elements subsequently led to the discovery of overlooked debris in an engine chip detector, and thus resulted in a review of several maintenance practices.

Gears can fail in many different ways, and except for an increase in noise level and vibration, there is often no indication of difficulty until total failure occurs. This article reviews the major types of gears and the basic principles of gear-tooth contact. It discusses the loading conditions and stresses that effect gear strength and durability. The article provides information on different gear materials, the common types and causes of gear failures, and the procedures employed to analyze them. Finally, it presents a chosen few examples to illustrate a systematic approach to the failure examination.

Rolling-element bearings use rolling elements interposed between two raceways, and relative motion is permitted by the rotation of these elements. This article presents an overview of bearing materials, bearing-load ratings, and an examination of failed bearings. Rolling-element bearings are designed on the principle of rolling contact rather than sliding contact; frictional effects, although low, are not negligible, and lubrication is essential. The article lists the typical characteristics and causes of several types of failures. It describes failure by wear, failure by fretting, failure by corrosion, failure by plastic flow, failure by rolling-contact fatigue, and failure by damage. The article discusses the effects of fabrication practices, heat treatment and hardness of bearing components, and lubrication of rolling-element bearings with a few examples.

Abstract
This article discusses failures in shafts such as connecting rods, which translate rotary motion to linear motion, and in piston rods, which translate the action of fluid power to linear motion. It describes the process of examining a failed shaft to guide the direction of failure investigation and corrective action. Fatigue failures in shafts, such as bending fatigue, torsional fatigue, contact fatigue, and axial fatigue, are reviewed. The article provides information on the brittle fracture, ductile fracture, distortion, and corrosion of shafts. Abrasive wear and adhesive wear of metal parts are also discussed. The article concludes with a discussion on the influence of metallurgical factors and fabrication practices on the fatigue properties of materials, as well as the effects of surface coatings.

This article describes the characteristics of tools and dies and the causes of their failures. It discusses the failure mechanisms in tool and die materials that are important to nearly all manufacturing processes, but is primarily devoted to failures of tool steels used in cold-working and hot-working applications. It reviews problems introduced during mechanical design, materials selection, machining, heat treating, finish grinding, and tool and die operation. The brittle fracture of rehardened high-speed steels is also considered. Finally, failures due to seams or laps, unconsolidated interiors, and carbide segregation and poor carbide morphology are reviewed with illustrations.

Abstract
This article discusses the classification of sliding bearings and describes the major groups of soft metal bearing materials: babbitts, copper-lead bearing alloys, bronze, and aluminum alloys. It provides a discussion on the methods for fluid-film lubrication in bearings. The article presents the variables of interest for a rotating shaft and the load-carrying capacity and surface roughness of bearings. Grooves and depressions are often provided in bearing surfaces to supply or feed lubricant to the load-carrying regions. The article explains the effect of contaminants in bearings and presents the steps for failure analysis of sliding bearings. It also reviews the factors responsible for bearing failure with examples.
