Abstract
The onset of leakage adjacent to two butt welds in a 2 in. bore pipe was traced to the development of fine cracks. The pipe carried 40% sodium hydroxide solution. The actual temperature was not known, but the pipeline was steam traced at a pressure of 30 psi, equivalent to a temperature of 130 deg C (266 deg F). Magnetic crack detection revealed circumferential crack-like indications situated a short distance from the butt weld. Cracking originated on the bore surfaces of the tube and was of an intergranular nature reminiscent of caustic cracking in steam boilers. The strength of the solution of caustic soda and possibly the temperature also were in the range known to produce stress-corrosion cracking of mild steels in the presence of stresses of sufficient magnitude. In this instance the location of the cracking suggested that residual stresses from welding, which approach yield point magnitude, were responsible. As all other welds were suspect, the remedy was to remove the joints and to reweld followed by local stress relief.

Abstract
Schedule 80 low-carbon steel pipes used to transfer kraft liquor in a Kamyr continuous pulp digester failed within 18 months after installation. Visual and metallographic examinations established that the cracking initiated on the internal surfaces of the equalizer pipes in the welds and heat-affected zones (HAZs). Fracture/crack morphology was brittle and primarily intergranular and deposits at crack tips were primarily iron oxides with significant amounts of sodium compounds. On these bases, the cracking was characterized as intergranular stress-corrosion cracking (IGSCC). Corrosion-related deterioration was not found, indicating that the material was generally suitable for the intended service. High residual tensile stresses in the welds and HAZS, resulting from field welding under highly constrained conditions using inadequate weld procedures, were the most probable cause of the failures. Minimizing residual stresses through use of welding procedures that include appropriate preweld and interpass temperatures and postweld stress relief heat treatment at 650 deg C (1200 deg F) was recommended to prevent further failures.

Abstract
An E-Brite /Ferralium explosively bonded tube sheet in a nitric acid condenser was removed from service because of corrosion. Visual and metallographic examination of tube sheet samples revealed severe cracking in the heat-affected zone between the outer tubes and the weld joining the tube sheet to the floating skirt. Cracks penetrated deep into the tube sheet, and occasionally into the tube walls. The microstructures of both alloys and of the weld appeared normal. Intergranular corrosion characteristic of end-grain attack was apparent. A low dead spot at the skirt / tube sheet joint allowed the Nox to condense and subsequently reboil. This, coupled with repeated repair welding in the area, reduced resistance to acid attack. Intergranular corrosion continued until failure. Recommendations included changing operating parameter inlet to prevent HNO3 condensation outside the inlet and replacement of the floating skirt with virgin material (i.e., material unaffected by weld repairs).

Abstract
Two AISI type 316 stainless steel dished ends failed through the formation of intergranular stress-corrosion cracks (IGSCC) within a few months of service. The dished ends failed in the straight portions near the circumferential welds that joined the ends to the cylindrical portions of the vessel. Both dished ends were manufactured from the same batch and were supplied by the same manufacturer One of the dished ends had been exposed to sodium at 550 deg C (1020 deg F) for 500 h before failure due to sodium leakage was detected. The other dished end was used to fabricate a second vessel that was kept in storage for 1 year Clear evidence of sensitization was found in areas where IGSCC occurred. Sensitization was extensive in the dished end that had been exposed to sodium at high temperature, and it occurred in a narrow band similar to that typical of weld decay in the dished end that had been kept in storage. Solution annealing was recommended to relieve residual stress, thereby reducing the probability of failure. It was also recommended that the carbon content of the steel be lowered, i.e., that a 316L grade be used.

Abstract
The source of cracking in the circumferential weld seam in a JIS-SM50B carbon-manganese steel pipe used in a CO2 absorber was investigated, the absorber had been in service for 18 years. The seam had been weld-repaired twice, and the repair welds had been locally stress relieved. Longitudinal seams in the same vessel, which had been stress relieved in a furnace, showed no tendency toward cracking. The solution passing through the vessel contained CO2-CO-H20, KHCO, and Cl− ions. Nondestructive testing revealed that the cracks originated in the heat-affected zone and propagated into the base metal and weld. Severe branching of the cracks characteristic of stress-corrosion cracking was observed. Microexamination revealed that crack propagation was transgranular further supporting the possibility of stress-corrosion cracking. Simulation tests carried out in the vessel confirmed this mode of cracking. It was recommended that weld seams be furnace heat treated at a temperature of 600 to 640 deg C (1110 to 1180 deg F) for a minimum of 1 h per inch of section thickness.

Abstract
Two failures of AP15A grade J-55 electric resistance welded (ERW) tubing in as our gas environment were investigated. The first failure occurred after 112 days of service. Replacement pipe failed 2 days later. Surface examination of the failed tubing indicated that fracture initiated at the outside surface. Metallographic analysis showed that the fracture originated in the upturned fibers adjacent to the ERW bond line. Cross sections of the weld were removed from three random locations in the test sample. At each location, the up turned fibers of the weld zone contained bands of hard-appearing microstructure. Hardness measurements confirmed these observations. The cracks followed these bands. It was concluded that the tubing failed from sulfide stress cracking, which resulted from bands of susceptible microstructure in the ERW zone. The banded microstructure in the pipe suggested that chemical segregation contributed to the hard areas. Postweld normalized heat treatment apparently did not sufficiently reduce the hardness of these areas.

Abstract
A type 316 stainless steel pipe reducer section failed in service of bleached pulp stock transfer within 2 years in a pulp and paper mill. The reducer section fractured in the heat-affected zone of the flange-to-pipe weld on the flange side. The pipe reducer section consisted of 250 and 200 mm (10 and 8 in.) diam flanges welded to a tapered pipe section. The tapered pipe section was 3.3 mm (0.13 in.) thick type 316 stainless steel sheet, and the flanges were 5 mm (0.2 in.) thick CF8M (type 316) stainless steel castings. Visual and metallographic analysis indicated that the fracture was caused by intergranular corrosion/stress-corrosion cracks that initiated from the external surface of the pipe reducer section. Contributory factors were the sensitized condition of the flange and the concentration of corrosive elements from the bleach stock plant environment on the external surface. In the absence of the sensitized condition of the flange, the service of the pipe reducer section was acceptable. A type 316L stainless steel reducer section was recommended to replace the 316 component because of its superior resistance to sensitization.

Abstract
Silver solid-state bonded components containing uranium failed under zero or low applied load several years after manufacture. The final operation in their manufacture was a proof loading that applied a sustained tensile stress to the bond, which all components passed. The components comprised circular cylinders fabricated by plating a thin layer of silver on each of the contact surfaces (uranium and stainless steel) and pressing the parts together at elevated temperature to solid-state bond the two silver surfaces. The manufacturing process produced a high level of residual stress at the bond. The failures appeared to be predominantly located between the silver layer and the uranium substrate. Normal fracture location of specimens taken from similar components was at the silver/silver bond interface. Laboratory testing revealed that the uranium/silver joint was susceptible to premature failure by stress-corrosion cracking under sustained loading if the atmosphere was saturated with water vapor.

Abstract
Several nickel-base superalloy (UNS N06600) welded heat-exchanger tubes used in processing black liquor in a kraft paper mill failed prematurely. Leaking occurred through the tube walls at levels near the bottom tube sheet. The tubes had been installed as replacements for type 304 stainless steel tubes. Visual and stereoscopic examination revealed three types of corrosion on the inside surfaces of the tubes: uniform attack, deeper localized corrosive attack, and accelerated uniform attack. Metallographic analysis indicated that pronounced dissimilar-metal corrosion had occurred in the base metal immediately adjacent to the weld seam. The corrosion was attributed to exposure to nitric acid cleaning solution and was accelerated by galvanic differences between the tubes and a stainless steel tube sheet and between the base metal of the tubes and their dendritic weld seams. A change to type 304 stainless steel tubing made without dendritic weld seams was recommended.

Abstract
Corrosion failure occurred in a titanium clad tubesheet because of a corrosive tube-side gas-liquid mixture leaking through fatigue cracks in the seal welds at tube-to-tubesheet joints. The tubesheet was a carbon steel plate clad with titanium on the tube side face. The seal weld cracks were initiated by cyclic stress imposed by exchanger tubes. The gas-liquid mixture passed through cracks under tube-side pressure, resulting in severe corrosion of the steel backing plate. The failure started with the loosening of the expanded tube-to-tubesheet joints. Loose joints allowed the exchanger tubes to impose load on seal welds and the shell side cooling water entered the crevice between the tubesheet and the tubes. The cooling water in the crevice caused galvanic reaction and embrittlement of seal welds. Brittle crack opening and crack propagation in seal welds occurred due to the cyclic stress imposed by the tubes. The cyclic stress arised from the thermal cycling of the heat exchanger. The possible effects of material properties on the failure of the tubesheet are discussed.

Abstract
A number of AISI 347 stainless steel bellows intended for use in the control rod drive mechanism of a fast breeder reactor were found to be leaking before being placed in service. The bellows, which had been in storage for one year in a seacoast environment, exhibited a leak rate on the order of 1 x 10−7 cu cm/s (6 x 10−8 cu in./s). Optical metallography revealed numerous pits and cracks on the surfaces of the bellow convolutes, which had been welded to one another using an autogenous gas tungsten arc welding process. Microhardness measurements indicated that the bellows had not been adequately stress relieved. It was recommended that a complete stress-relieving treatment be applied to the formed bellows. Improvement of storage conditions to avoid direct and prolonged contact of the bellows with the humid, chloride-containing environment was also recommended.

Abstract
A steel galvanizing vat measuring 3 x 1.2 x 1.2 m (10 x 4 x 4 ft) and made of 19 mm thick carbon steel plate (ASTM A285, grade B)) at a shipbuilding and ship-repair facility failed after only three months of service. To verify suspected failure cause, two T joints were made in 12.5 mm thick ASTM A285, grade B, steel plate. One joint was welded using the semiautomatic submerged arc process with one pass on each side. A second joint was welded manually by the shielded metal arc process using E6010 welding rod and four passes on each side. The silicon content of the shielded metal arc weld was 0.54%, whereas that of the submerged arc weld was 0.86%. After being weighed, the specimens were submerged in molten zinc for 850 h. Analysis (visual inspection, chemical analysis, 100x 2% nital-etched micrographs) supported the conclusions that the vat failed due to molten-zinc corrosion along elongated ferrite bands, possibly because silicon was dissolved in the ferrite and thus made it more susceptible to attack by the molten zinc. Recommendations included rewelding the vat using the manual shielded metal arc process with at least four passes on each side.

Abstract
Several 304H stainless steel superheater tubes fractured in stressed areas within hours of a severe caustic upset in the boiler feedwater system. Tests performed on a longitudinal weld joint, which connected two adjacent tubes in the tertiary superheater bank, confirmed caustic-induced stress-corrosion cracking, promoted by the presence of residual welding stresses. Improved maintenance of check valves and routine inspection of critical monitoring systems (conductivity alarms, sodium analyzers, etc.) were recommended to help avoid future occurrences of severe boiler feedwater contamination. Additional recommendations were to eliminate these short longitudinal weld joints by using a bracket assembly joint between the tubes, use a post-weld heat treatment to relieve residual welding stress or select a more stress-corrosion cracking resistant alloy for this particular application.

Abstract
A type 321 stainless steel downcomer expansion joint that handled process gases was found to be leaking approximately 2 to 3 weeks after installation. The expansion joint was the second such coupling placed in the plant after failure of the original bellows. The failed joint was disassembled and examined to determine the cause of failure. Energy-dispersive x-ray analysis revealed significant peaks for chlorine and phosphorus, indicating failure by chloride stress-corrosion cracking (SCC). Cracks in the liner and bellows exhibited a branched pattern also typical of SCC. Cracks through the inner liner initiated on the outer surface of the liner and propagated inward, whereas cracks in the bellows originated on the inner surface and propagated outward. Stress-corrosion cracking of the assembly was caused by chloride contaminants trapped inside the bellows following hydrostatic testing. Checking the test fluid for chloride and removing all fluids after hydrostatic testing were recommended to prevent further failure.

Abstract
A storage tank had been in service at a petrochemical plant for 13 years when inspectors discovered cracks adjacent to weld joints and in the base plate near the foundation. The tank was made from AISI 304 stainless steel and held styrene monomer, a derivative of benzene. The cracks were subsequently welded over with 308 stainless steel filler wire and the base plate was replaced with new material. Soon after, the tank began leaking along the weld bead, triggering a full-scale investigation; spectroscopy, optical and scanning electron microscopy, fractography, SEM-EDS analysis, and microhardness, tensile, and impact testing. The results revealed transgranular cracks in the HAZ and base plate, likely initiated by stresses developed during welding and the presence of chloride from seawater used in the plant. It was also found that the repair weld was improperly done, nor did it include a postweld heat treatment to remove weld sensitization and minimize residual stresses.

A welded joint between lengths of 4 in. OD x 13 SWG copper pipe which formed part of a cold-water main failed by cracking over one-third of the circumference. Microscopic examination of the filler metal showed that it had a structure corresponding to a brass of the 60:40 type commonly used for bronze welding. Failure resulted from dezincification of the joint material from the internal side of the tube. Also, a selective attack on the beta phase had occurred. It was evident that the loss in mechanical strength arising from the corrosion had resulted in the development of cracking in service. The filler metal used was not resistant to the conditions to which it was exposed. Copper welding rods as per BS 1077 or a Cu-Ag-P brazing alloy as recommended in BS 699, would have been preferable.

Abstract
During a refit of a twenty-year-old Naval destroyer, two cracks were found on the inside of the killed carbon-manganese steel hull plate at the forward end of the boiler room. The cracks coincided with the location of the top and bottom plates of the bilge keel. Metallurgical examination of sections cut from the cracked area identified lamellar tearing as the principle cause of the cracking. This was surprising in 6 mm thick hull plates. Corrosion fatigue and general corrosion also contributed to hull plate perforation. Although it is probable that more lamellar tears exist near the bilge keel in other ships and may be a nuisance in the future, the hull integrity of the ships is not threatened and major repairs are not needed.

Abstract
The dished ends of a heavy water/helium storage tank manufactured from 8 mm (0.3 in.) thick type 304 stainless plate leaked during hydrotesting. Repeated attempts at repair welding did not alleviate the problem. Examination of samples from one dished end revealed that the cracking was confined to the heat affected zone (HAZ) surrounding circumferential welds and, to a lesser extent, radial welds that were part of the original construction. Most of the cracks initiated and propagated from the inside surface of the dished ends. Microstructures of the base metal, HAZ, and weld metal indicated severe sensitization in the HAZ due to high heat input during welding. An intergranular corrosion test confirmed the observations. The severe sensitization was coupled with residual stresses and exposure of the assembly to a coastal atmosphere during storage prior to installation. This combination of factors resulted in failure by stress-corrosion cracking. Implementation of a new repair procedure was recommended. Abstract
Beveled weld-joint V-sections were fabricated to connect inlet and outlet sections of tubes in a type 347 stainless steel heat exchanger for a nitric acid concentrator. Each V-section was permanently marked with the tube numbers by a small electric-arc pencil. After one to two years of service, multiple leaks were observed in the heat-exchanger tubes. Investigation supported the conclusion that the corrosion occurred at two general locations: the stop point of the welds used to connect the inlet and outlet legs of the heat exchanger, and the stop points on the identifying numerals. Recommendations included replaced the material with type 304L stainless steel.

Abstract
A nozzle in a wastewater vaporizer began leaking after approximately three years of service with acetic and formic acid wastewaters at 105 deg C (225 deg F) and 414 kPa (60 psig). The shell of the vessel was weld fabricated from 6.4 mm (0.25 in.) E-Brite stainless steel plate and measured 1.5 m (58 in.) in diameter and 8.5 m (28 ft) in length. Investigation (visual inspection, chemical analysis, radiography, dye-penetrant inspection, and hydrostatic testing of all E-Brite welds, 4x images, 100x/200x images electrolytically etched with 10% oxalic acid, and V-notch Charpy testing) supported the conclusion that failure of the nozzle weld was the result of intergranular corrosion caused by the pickup of interstitial elements and subsequent precipitation of chromium carbides and nitrides. Carbon pickup was believed to have been caused by inadequate joint cleaning prior to welding. The increase in the weld nitrogen level was a direct result of inadequate argon gas shielding of the molten weld puddle. Two areas of inadequate shielding were identified: improper gas flow rate for a 19 mm (0.75 in.) diam gas lens nozzle, and contamination of the manifold gas system. Recommendations included changes in the cleaning and welding process.
