Abstract
A series of poppet-valve stems fabricated from 17-4 PH (AISI type 630) stainless steel failed prematurely in service during the development of a large combustion assembly. The poppet valves were part of a scavenging system that evacuated the assembly after each combustion cycle. The function of the valve is to open and close a port; thus, the valve is subjected to both impact and tensile loading. Analysis (visual inspection, hardness testing, and stress analysis) supported the conclusions that the valve stems were impact loaded to stresses in excess of their yield strength. That they failed in the threaded portion also suggests a stress-concentration effect. Recommendations included changing the material spec to a higher-strength material with greater impact strength. In this case, it was recommended that the stems, despite any possible design changes, be manufactured from an alloy such as PH 13-8Mo, which can be processed to a yield strength of 1379 MPa (200 ksi), with impact energies of the order of 81 J (60 ft·lbf) at room temperature.

Abstract
After about two years in service, a 303 stainless steel valve in contact with a carbonated soft drink in a vending machine occasionally dispensed a discolored drink with a sulfide odor. According to the laboratory at the bottling plant, the soft drink in question was strongly acidic, containing citric and phosphoric acids and having a pH of 2.4 to 2.5. Investigation (visual inspection, chemical analysis, immersion testing in the soft drink, and 100x unetched micrographs) supported the conclusion that the failure was caused by the size and distribution of sulfide stringers in the alloy used in the valve. Manganese sulfide stringers in the valve were exposed at end-grain surfaces in contact with the beverage. The stringers, which were anodic to the surrounding metal, were subject to corrosion, producing a hydrogen sulfide concentration in the immediately adjacent liquid. Recommendations included changing the valve material to type 304 stainless steel.

Abstract
A liquid hydrogen main fuel control valve for a rocket engine failed by fracture of the Ti-5Al-2.5Sn body during the last of a series of static engine test firings. Fractographic, metallurgical, and stress analyses determined that a combination of fatigue and unexpected aqueous stress-corrosion cracking initiated and propagated the crack which caused failure. The failure analysis approach and its results are described to illustrate how fractography and fracture mechanics, together with a knowledge of the crack initiation and propagation mechanisms of the valve material under various stress states and environments, helped investigators to trace the cause of failure.

Abstract
An external tank pressure/vent valve regulates the external tank fuel feed system, which transfers fuel under pressure to the internal tanks of the aircraft. A dual-position valve was found to be sticking at the intermediate positions. Also, service air check valves located on the incoming lines contained poppets that were being stuck in a closed or partially closed position because of suspected corrosion product. Residue taken from the check valve poppet and from the dual-position valve was chemically analyzed. Chloride was present in both samples. It was suspected that moisture entering the service air lines left a chloride-containing compound upon evaporation within the air check valves and pressure/vent assembly. This compound subsequently reacted with the anodized, dichromate sealed check valve housing to lock the check valve poppets in a closed or partially closed position, decreasing the actual pressure being supplied to the pressure/vent valve. It was recommended that an inspection be conducted to ensure that the service air check valves are operating properly prior to removal and servicing of the pressure/vent valve assembly. It was also recommended that dry-film lubricant be checked to ensure that it meets specifications for the pressure/vent valve assembly.

Abstract
One of three valves in a dry automatic sprinkler system tripped accidentally, thus activating the sprinklers. Maintenance records showed that the three valves had been in service less than two years. The valve consisted of a cast copper alloy clapper plate that was held closed by a pivoted malleable iron latch. The latch and top surface of the clapper plate were usually in a sanitary-water environment (stabilized, chlorinated well water with a pH of 7.3) under stagnant conditions. Process make-up water that had been clarified, filtered, softened, and chlorinated and had a pH of 9.8 was occasionally used in the system. Analysis (visual inspection and 250x micrograph) supported the conclusions that failure of the latch was caused by plastic deformation from extensive loss of metal by galvanic corrosion and the sudden loading related to the tripping of the valve. Failure in some regions of the contact area was by ductile (transgranular) fracture. Recommendations included changing the latch material from malleable iron to silicon bronze (C87300). The use of silicon bronze prevents corrosion or galvanic attack and proper adjustment of the latch maintains an adequate contact area.

Abstract
After two weeks of operation, a poppet used in a check valve to control fluid flow and with a maximum operating pressure of 24 MPa (3.5 ksi) failed during operation. Specifications required that the part be made of 1213 or 1215 rephosphorized and resulfurized steel. The poppet was specified to be case hardened to 55 to 60 HRC, with a case depth of 0.6 to 0.9 mm (0.025 to 0.035 in.); the hardness of the mating valve seat was 40 HRC. Analysis showed that the fracture occurred through two 8 mm (0.313 in.) diam holes at the narrowest section of the poppet. The valve continued to operate after it broke, which resulted in extensive loss of metal between the holes. 80x micrograph and 4x macrograph of a 5% nital etched longitudinal section, and chemical analyses showed the poppet did fit 1213 or 1215 specs. However, hardness measurements showed surface hardness was excessive-61 to 65 HRC instead of the specified 55 to 60 HRC. Thus, the poppet failed by brittle fracture, and cracking occurred across nonmetallic inclusions. Recommendation was to redesign the valve with the poppet material changed to 4140 steel, hardened, and tempered to 50 to 55 HRC.

Abstract
A precipitation-hardened stainless steel poppet valve assembly used to shut off the flow of hydrazine fuel to an auxiliary power unit was found to leak. SEM and optical micrographs revealed that the final heat treatment designed for the AM-350 bellows material rendered the AM-355 poppet susceptible to intergranular corrosive attack (IGA) from a decontaminant containing hydroxy-acetic acid. This attack provided pathways for which fluid could leak across the sealing surface in the closed condition. It was concluded that the current design is flight worthy if the poppet valve assembly passes a preflight helium pressure test. However a future design should use the same material for the poppet and bellows so that the final heat treatment will produce an assembly not susceptible to IGA.

Abstract
Failure of a pilot scale test melter resulted from severe overheating of an Inconel 690 (690) jacketed molybdenum electrode. Extreme temperatures were required to melt the glass during this campaign because the feed material contained a very high waste loading. Metallurgical evaluation revealed the presence of an alloy containing nickel and molybdenum in several ingots found on the bottom of the melter and on a drip which had solidified on the electrode sheath. This indicates that a major portion of the electrode assembly was exposed to a temperature of at least 1317 deg C, the nickel/molybdenum eutectic temperature. Small regions on the end of the 690 sheath showed evidence of melting, indicating that this localized region exceeded 1345 deg C, the melting point of 690. In addition to nickel, antimony was found on the grain boundaries of the molybdenum electrode. This also contributed to the failure of the electrode. The source of the antimony was not identified but is believed to have originated from the feed material. Metallurgical evaluation also revealed that nickel had attacked the grain boundaries of the molybdenum/tungsten drain valve. This component did not fail in service; however, intergranular attack led to degradation of the mechanical properties, resulting in the fracture of the drain valve tip during disassembly. Antimony was not observed on this component.

Abstract
The clapper in a 250 mm diam disk valve (made from ASTM A36 steel, stress relieved and cadmium plated) fractured at the welded joint between the clapper and a 20 mm diam support rod (also made of same material). The valve contained a stream of gas consisting of 55% H2S, 39% CO2, 5% H2, and 1% hydrocarbons at 40 deg C and 55 kPa during operation. Voids on the fracture surface and evidence of incomplete weld penetration were revealed by examination. Brittle fracture was indicated by the overall appearance through some fatigue beach marks were observed. Very narrow bands of high hardness were revealed at the edges of the weld metal. It was revealed by chemical analysis of this band that a stainless steel filler metal had been used which produced mixed composition at the weld boundaries. The plating material was revealed to be nickel by chemical analysis. It was concluded that clapper failed by fatigue and brittle fracture because it was welded with an incorrect filler metal. A clapper assembly was welded with a low-carbon steel filler metal, then cadmium plated.

Abstract
A reversible four-way carbon steel flap valve in a thermal power station failed after 7 years of service. The flap had been fabricated by welding two carbon steel plates to both sides of a carbon steel forging. The valve was used for reversing the flow direction of seawater in the cooling system of a condenser. Visual examination of the flap showed crystalline fracture, indicating a brittle failure. Metallographic examination, chemical analyses, and tensile and impact testing indicated that the failure was caused by the notch sensitivity of the forging material, which resulted in low toughness. It was recommended that fully killed carbon steel with a fine-grain microstructure be used. Redesign of the flap to remove the step in the forging that acted as a notch was also recommended.

Abstract
The engine of an automobile lost power and compression and emitted an uneven exhaust sound after several thousand miles of operation. When the engine was dismantled, it was found that the outer spring on one of the exhaust valves was too short to function properly. The short steel spring and an outer spring (both of patented and drawn high-carbon steel wire) taken from another cylinder in the same engine were examined in the laboratory to determine why one had distorted and the other had not. Investigation (visual inspection, microstructure examination, and hardness testing) supported the conclusion that the engine malfunctioned because one of the exhaust-valve springs had taken a 25% set in service. Relaxation in the spring material occurred because of the combined effect of improper microstructure (proeutectoid ferrite) plus a relatively high operating temperature. Recommendations included using quenched-and-tempered steel instead of patented and cold-drawn steel or using a more expensive chromium-vanadium alloy steel instead of plain carbon steel; the chromium-vanadium steel would also need to be quenched and tempered.

Abstract
Cracks on the outer surface near a hanger lug were revealed by visual inspection of a type 316 stainless steel main steam line of a major utility boiler system. Cracking was found to have initiated at the outside of the pipe wall or immediately beneath the surface. The microstructure of the failed pipe was found to consist of a matrix precipitate array (M23C6) and large s-phase particles in the grain boundaries. A portable grinding tool was used to prepare the surface and followed by swab etching. All material upstream of the boiler stop valve was revealed to have oriented the cracking normally or nearly so to the main hoop stress direction. Residual-stress measurements were made using a hole-drilling technique and strain gage rosettes. Large tensile axial residual stresses were measured at nearly every location investigated with a large residual hoop stress was found for locations before the stop valve. It was concluded using thermal stress analysis done using numerical methods and software identified as CREPLACYL that one or more severe thermal downshocks might cause the damage pattern that was found. The root cause of the failure was identified to be thermal fatigue, with associated creep relaxation.

Abstract
An experimental high-temperature rotary valve was found stuck due to growth and distortion after approximately 100 h. Gas temperatures were suspected to have been high due to overfueled conditions. Both the rotor and housing in which it was stuck were annealed ferritic ductile iron similar to ASTM A395. Visual examination of the rotor revealed unusually heavy oxidation and thermal fatigue cracking along the edge of the gas passage. Material properties, including microstructure, composition, and hardness, of both the rotor and housing were evaluated to determine the cause of failure. The microstructure of the rotor was examined in three regions. The shaft material, the heavy section next to the gas passage and the thin edge of the rotor adjacent to the gas passage. The excessive gas temperatures were responsible for the expansion and distortion that prevented rotation of the rotor. Actual operating temperatures exceeded those intended for this application. The presence of transformation products in the brake-rotor edge indicated that the lower critical temperature had been exceeded during operation.

The crosslinked epoxy resin encapsulant protecting an electromagnetic valve coil failed during long-term storage and was examined to determine the cause. The investigation included fault-tree analysis, FTIR and EDX spectroscopy, and differential scanning calorimetry with thermogravimetric analysis. Based on test data, the epoxy resin had not been properly cured and was hydrolyzed in its compromised state because of humidity. Hence, the depolymerized material gradually softened to the point where the effect of creep caused it to flow, ultimately causing the failure.

Abstract
The cover of a feed cock fitted to an economic boiler suddenly blew off and the plug lifted sufficiently to permit the escape of water. It was found that all four of the studs securing the cover had fractured. In each case fracture had occurred at the end of one of the screwed portions adjacent to the shank. The fractures were of a short nature, with no evidence of progressive cracking by fatigue, nor was there any sign of stretching prior to failure. The fractured faces and the shanks of the studs were of rusted appearance. Microscopic examination of the material showed it to be an austenitic nickel-chromium stainless steel, stabilized by titanium and of the free machining type. Multiple transgranular cracking characteristic of failure from stress-corrosion cracking, was present to an extensive degree. It was considered probable that there had been slight leakage of water from the valve over a period and evaporation resulted in a solution which favored failure from stress-corrosion cracking. If corrosion resistant studs were desired, those of bronze or Monel metal are to be preferred.

Abstract
A main steam pipe was found to be leaking due to a large circumferential crack in a pipe-to-fitting weld in one of two steam leads between the superheater outlet nozzles and the turbine stop valves (a line made of SA335-P22 material). The main crack surface was found to be rough, oriented about normal to the outside surface, and had a dark oxidized appearance. The cracking was found to be predominantly intergranular. Distinct shiny bands that etched slower than the remainder of the sample at the top of each individual weld bead were revealed by microscopic examination. These bands were found contain small cracks and microvoids. A mechanism of intergranular creep rupture at elevated temperature was identified as a result of a series of stress-rupture and tensile tests. It was revealed by the crack shape that cracking initiated on the pipe exterior, then propagated inward and in the circumferential direction in response to a bending moment load. It was concluded that the primary cause of failure was the occurrence of bending stresses that exceeded the stress levels predicted by design calculations and that were higher than the maximum allowable primary membrane stress.

A superheater in a generator produced 80 t/h of steam at 400 deg C and 41 kPa. Failure took place at the connection from the collector to the vent line used during start up. The material of construction was carbon steel, and the unit had 240,000 h of operation at the time of failure, with 99 shutdowns. Widespread cracking on the inside was apparent, the most severe cracking being some distance from the nozzle connection in a downstream direction. Widespread cracking and pitting were observed also at the connections to the safety valve and soot blower. Pitting was most apparent on the downstream sides of the openings in the shell. In all the damaged areas the mechanism of failure involved surface pitting and subsequent SCC. This failure showed the problems that can develop where there are long lines in which condensation may occur and return periodically to a superheater or other hot component. In this particular case, control of dissolved solids in the boiler feedwater may have been inadequate.

Examination of the header of the third superheater of a boiler producing 150 t/h of steam at 525 deg C and 118 kPa, disclosed extensive internal cracking at the connection to the tube joining this to a safety valve. Cracking was observed within the tube and in the thickness of the shell wall itself. The boiler had been in operation for approximately 160,000 h and was shut down for inspection when the cracking was detected. The material involved was 2.25 Cr, 1 Mo steel, and the unit had been subjected to 115 shutdowns. Initiation of the cracks was attributed to thermal shock, caused by the periodic return of condensate along the long connecting line (some 9 m long). Propagation of the cracks was due to thermal cycling, together with periodic pressure cycles, producing growth by low cycle fatigue. This was aided by corrosion within the cracks and by the wedging action caused by corrosion deposits at their tips. The failure suggests control of dissolved solids in the boiler feedwater may have been inadequate.

Abstract
The bottom flange of a vertical pipe coupled to an isolating valve in a steam supply line to a turbine failed. Steam pressure was 1,500 psi and the temperature 416 deg C (780 deg F). Multiple cracking occurred in the bore of the flange. A quarter-segment was cut out and examined. The cracks were located in the part of the flange that formed a continuation of the pipe bore. The majority of them originated at the end of the flange bore and extended axially along the pipe and radially across the flange face. Magnetic crack detection revealed a further number of cracks in the weld deposit. While the fracture in the weld metal was of the ductile type exhibiting a fine fibrous appearance, that in the flange material was of the cleavage type. Microscopic examination revealed that the cracks were blunt-ended fissures of the type characteristic of corrosion-fatigue. It was concluded that cracking was due to corrosion-fatigue, which arose from the combined effect of a fluctuating tensile stress in the presence of a mildly corrosive environment.

Abstract
Diagnosis of environmentally induced failures is greatly facilitated by metallographic analysis. As an example, a failure analysis of ASTM A574 material grade bolts is presented. The bolts served as bonnet screws in underground valves and failed due to stress-corrosion cracking. Metallographic methods were used to diagnose and provide solutions for the service failure. Included are photos showing crack propagation morphology and fracture surface appearance.
