Abstract
An amusement ride failed when a component in the ride parted, permitting it to fly apart. The ride consisted of a central shaft supporting a spider of three arms, each of which was equipped with an AISI 1040 steel secondary shaft about which a circular platform rotated. The main shaft rotated at about 12 rpm and the platforms at a speed of 20 rpm. The accident occurred when one of the secondary shafts on the amusement ride broke. The point of fracture was adjacent to a weld that attached the shaft to a 16 mm thick plate, which in turn bore the platform support arms. Investigation (visual inspection, 0.4x magnification, and stress analysis) supported the conclusion that a likely cause for the fatigue failure was the combination of residual stresses generated in welding and centrifugal service stresses from operation that were accentuated by areas of stress concentration at the undercut locations. Without the excessive residual stress, the shaft dimensions appeared ample for the service load. Recommendations included applying the fillet weld with more care to avoid undercutting. The residual stresses could be minimized by pre-weld and post-weld heat application.

Abstract
Welded to the top of a dust bin for rigid support, a furnace water-wall tube in a new stationary boiler broke at the welded joint shortly after start-up. The tubes measured 64 mm (2.5 in.) OD by 3.2 mm (0.125 in.) wall thickness and were made of carbon steel to ASME SA-226 specifications. Investigation supported the conclusion that a crevice-like undercut was likely the primary cause of the fracture and that the source of the necessary fluctuating stress was tube vibration inherent in boiler operation. Recommendations included magnetic-particle inspection of the remaining water-wall tubes in the row, replacing the broken tube, and repairing cracks in other tubes by welding.

Abstract
A weld that attached the head to the shell of a preheater containing steam at 1.4 MPa (200 psi) and was used in the manufacture of paper cracked in service. The original joint contained a 6.4 by 50 mm backing ring that had been tack welded to the inside surface of the shell in a position to project beyond the fully beveled top edge of the shell. The projecting edge of the ring fitted against a deep undercut on the inner corner of the rim of the head. The internal 90-deg angle in this undercut was sharp, with almost no fillet. A bevel from the lower edge of the undercut to the outside of the head completed the groove for the circumferential attachment weld. Investigation (visual inspection and actual size views etched in hot 50% hydrochloric acid) supported the conclusion that cracking occurred in the HAZ in the head of the original design, originating in the sharp corner of the undercut, which was an inherent stress raiser. Recommendations included revised joint design to ensure full root penetration.

Abstract
The case and stiffener of an inner-combustion-chamber case assembly failed by completely fracturing circumferentially around the edge of a groove arc weld joining the case and stiffener to the flange. The assembly consisted of a cylindrical stiffener inserted into a cylindrical case that were both welded to a flange. The case, stiffener, flange, and weld deposit were all of nickel-base alloy 718. It was observed that a manual arc weld repair had been made along almost the entire circumference of the original weld. Investigation (visual inspection, 0.5x macrographs, and 10x etched with 2% chromic acid plus HCl views) supported the conclusions that failure was by fatigue from multiple origins caused by welding defects. Ultimate failure was by tensile overload of the sections partly separated by the fatigue cracks. Recommendations included correct fit-up of the case, stiffener, and flange and more skillful welding techniques to avoid undercutting and unfused interfaces.

Abstract
Failure of a case hardened steel shaft incorporated fuel pump in a turbine-powered aircraft resulted in damage to the aircraft. The disassembled pump was found to be dry and free of any contamination. Damage was exhibited on the pressure side of each spline tooth in the impeller and the relatively smooth cavities and undercutting of the flank on this side indicated that the damage was caused by an erosion or abrasion mechanism. A relatively smooth worn area was formed at the center of each tooth due to an abrasive action and an undulating outline with undercutting was observed on the damaged side. Particles of sand, paint, or plastic, fibers from the cartridge, brass, and steel were viewed in the brown residue on the filter cartridge under a low power microscope and later confirmed by chemical analysis. Large amount of iron was identified by application of a magnet. It was concluded that the combined effect of vibration and abrasive wear by sand and metal particles removed from the splines damaged the shaft. Case hardened spline teeth surface was recommended to increase resistance to wear and abrasion.

Abstract
Tubes in heat exchangers, made of copper alloy C44300 and used for cooling air failed after 5 to six years of service. Air passed over the shell-side surface of the tubes and was cooled by water flowing through the tubes. Water vapor in the air was condensed (pH 4.5) on the tube surfaces during the cooling process. Air flow over the tubes reversed direction every 585 mm as a result of baffling placed in the heat exchangers. An uneven ridgelike thinning and perforation of the tube wall on the leeward side of the tube was revealed by visual examination. Undercut pits on the outer surface of the tube were revealed by metallographic examination of a cross section of the failed area. Impingement attack which led to perforation was revealed by both the ridgelike appearance of the damaged area and the undercut pitting. The heat exchanger was retubed with tubes made of aluminum bronze (copper alloy C61400).

Abstract
While undergoing vibration testing, a type 347 stainless steel inlet header for a fuel-to-air heat exchanger cracked in the header tube adjacent to the weld bead between the tube and header duct. Investigation (visual inspection and liquid penetrant inspection) supported the conclusion that the crack in the header tube was the result of a stress concentration at the toe of the weld joining a doubler collar to the tube. The stress concentration was caused by undercutting from poor welding technique and an unfavorable joint design that did not permit a good fit-up. Recommendations included manufacturing the doubler collar so that it could be placed in intimate contact with the header duct, and a revised weld procedure was recommended to result in a smaller, controlled, homogeneous weld joint with less distortion.

Abstract
AISI type 410 stainless steel tube bundles in a heat exchanger experienced leakage during hydrostatic testing even before being in service. The inside surfaces of the tubes was observed to have been pitted. Chloride-ion pitting was revealed by the undercutting in the cross section of a pit and further confirmed by x-ray spectrometry. It was concluded that the failure was caused by pitting due to chlorides in the water used to flush the tubes before service. The use of brackish water to flush or test stainless steel equipment was recommended to avoid pitting.

Abstract
A pipe in a chip conveyor cracked at the toe of an exterior fillet weld connecting a flange to the pipe. The chip conveyor consisted of several spool sections. Each section was made up of a length of low-alloy steel pipe and two flanges, which were welded to each end. The composition specified for the pipe steel was 0.25C-0.98Mn-3.52Ni-1.34Cr-0.24Mo, which approximates a 9300 steel with high molybdenum. Investigation supported the conclusion that the conveyor pipe failed by brittle fracture, which was attributed to the stresses induced in forcing the circular flange over the elliptical section of the pipe. The toe of the weld and the adjacent undercut were stress raisers that determined the point of major crack origin. Under residual stress, the internal point of incomplete fusion also initiated additional cracks. Recommendations included ensuring a proper fit between an elliptical flange and pipe end to eliminate the cracking.

Abstract
Severe pitting was found on the internal surfaces of SA-210 Grade C waterwall tubing of a coal-fired boiler at a cogeneration facility. Metallographic examination showed the pits to be elliptical, having an undercut morphology with supersurface extensions,. a type of pitting characteristic of acidic attack. Energy-dispersive X-ray spectroscope revealed the presence of chlorine in the pit deposits, indicating that the pitting was promoted by underdeposit chloride attack. The presence of copper in deposits on the internal surface of the tubing may have acted as a secondary factor. Acidic conditions may have formed during a low-pH excursion that reportedly occurred several years prior. To prevent future failures, severely damaged tubing must be replaced. Internal deposit buildup must be removed by chemical cleaning to prevent further pitting. Water quality needs continued monitoring and maintenance to ensure that another low-pH excursion does not occur.

Abstract
A recurring piston shaft failure problem on the billet-loading tray of an extrusion press was investigated. Two shafts fractured within a period of 10 days. The shaft was machined from normalized EN3 (AISI C1022) steel stock without further treatment. Visual, microstructural, chemical, and mechanical (hardness and tensile properties) analyses of failed shaft specimens were conducted. The examinations showed that the shafts had failed by fatigue. It was recommended that a low-alloy steel (e.g., 3% Ni-Cr) in the hardened and tempered condition and subjected to shot-peening surface-hardening treatment be used. The provision of a stop to reduce bending stresses was also recommended.
