Abstract
An explosion occurred in a portion of a horizontal, U-shaped expansion loop in a steam main approximately 10-in. diam which had been operating at 400 psi for six years. Steam conditions varied from 538 deg C (450 deg F) saturated to 343 deg C (650 deg F) superheated. Fracture occurred longitudinally through the upper wall over a length of approximately 68 in. The sample received for examination was ultrasonically tested, which indicated a band of internal defects extending 1 in. in from the edge. Subsequently, the portion of the pipe embodying the other side of the rupture was obtained for examination. Transverse sections through this and the mating portion already received, followed by magnetic crack detection, revealed the presence of defective zones. Subsequent ultrasonic examination of other sections of the steam main indicated suspect areas in a number of lengths of pipe. These defects were basically laminations of a similar form to those which resulted in the failure of the portion of pipe.

Abstract
Two leaks were discovered at a sulfur recovery unit in a refinery. The leaks were at pipe-to-elbow welds in a 152 mm (6 in.) (NPS 6) diam line, operating in lean amine service at 50 deg C (120 deg F) and 2.9 MPa (425 psig). Thickness measurements indicated negligible loss of metal, and the leaks were clamped. A year later, 15 additional leaks were discovered, again at pipe-to-elbow welds in lean amine lines. Further nondestructive testing located other cracks, giving a total of 35. These lines had been in service for approximately eight years. Investigation (visual inspection, hardness testing, and micrographic cross-sections) supported the conclusion that the failure was caused by lean amine SCC. It was considered unlikely that these pipe welds had received such a postweld heat treatment, although it is industry practice to postweld stress relieve piping and pressure vessels in lean amine service if the temperature is expected to be above 95 deg C (200 deg F). Recommendations included inspecting all welds using shear wave ultrasonic testing and postweld heat treating all welds in lean amine service.

Abstract
The gun mount used in two types of self-propelled artillery consists of an oil-filled recoil cylinder and a sand-cast (MIL-I-11466, grade D7003) ductile-iron piston that connects to the gun tube through a threaded rod. The piston contains several orifices through which oil is forced as a means of absorbing recoil energy. During operation, the piston is stressed in tension, pulled by oil pressure on one end and the opposing force of the gun tube on the other. The casting specification stipulated that the graphite be substantially nodular and that metallographic test results be provided for each lot. Investigation (visual inspection, fatigue testing, 0.25x/0.35x/50x magnifications, 2% nital etched 60x/65x magnifications, and SEM views) showed that most of the service fractures occurred in pistons containing vermicular graphite. Recommendations included ultrasonic testing of pistons already in the field to identify and reject those containing vermicular graphite. In addition, metallographic control standards were suggested for future production lots.

Abstract
In 1979, during a routine bridge inspection, a fatigue crack was discovered in the top flange plate of one tie girder in a tied arch bridge crossing the Mississippi River. Metallographic analysis indicated a banding or segregation problem in the middle of the plate, where the carbon content was twice what it should have been. Based on this and results of ultrasonic testing, which revealed that the banding occurred in 24-ft lengths, it was decided to close the bridge and replace the defective steel. The steel used in the construction of this bridge was specified as ASTM A441, commonly used in structural applications. Testing showed an increase in hardness and weight percent carbon and manganese in the banded region. Further testing revealed that the area containing the segregation and coarse grain structure had a lower than expected toughness and a transition temperature 90 deg F higher than specified by the ASTM standards. The fatigue crack growth rate through this area was much faster than expected. All of these property changes resulted from increased carbon levels, higher yield strength, and larger than normal grain size.

Abstract
A failure of an aboveground storage tank occurred due to external corrosion of the tank floor. The liquid asphalt tank operated at elevated temperatures (approximately 177 deg C, or 350 deg F) and had been in service for six years. Cathodic protection (rectifiers) had been installed since start-up of the tank operation. It was noted, however, that some operational problems with the rectifier may have interrupted its protection. Investigation (visual inspection, on-site examination and testing, EDS analysis of scale deposits, and MIC testing of the soil) supported the conclusion that corrosion may have been caused by an interruption in cathodic protection. The effectiveness of cathodic protection on established microbial deposits is questionable. Recommendations included ultrasonically testing the tank floor and replacing portions based on the remaining wall thickness. Doubling the wall thickness of the floor plates was also recommended.

Abstract
A system of carbon steel headers, handling superheated water of 188 deg C (370 deg F) at 2 MPa (300 psi) for automobile-tire curing presses, developed a number of leaks within about four months after two to three years of leak-free service. All the leaks were in shielded metal arc butt welds joining 200 mm (8 in.) diam 90 deg elbows and pipe to 200 mm (8 in.) diam welding-neck flanges. A flange-elbow-flange assembly and a flange-pipe assembly that had leaked were removed for examination. Investigation (visual inspection, hardness testing, chemical analysis, magnetic-particle testing, radiographic inspection, and 2% nital etched 1.7x views) showed varying IDs on the assemblies and supported the conclusions that the failures of the butt welds were the result of fatigue cracks caused by cyclic thermal stresses that initiated at stress-concentrating notches at the toes of the interior fillet welds on the surfaces of the flanges. Recommendations included using ultrasonic testing to identify the appropriate joints and then replacing them. Special attention to accuracy of fit-up in the replacement joints was also recommended to achieve smooth, notch-free contours on the interior surfaces.

Abstract
A welded natural gas line of 400 mm OD and 9 mm wall thickness made of unalloyed steel with 0.22C had to be removed from service after four months because of a pipe burst. Metallographic examination showed the pipe section located next to the gas entrance was permeated by cracks or blisters almost over its entire perimeter in agreement with the ultrasonic test results. Only the weld seam and a strip on each side of it were crack-free. Based on this investigation, the pipeline was taken out of service and reconstructed. To avoid such failures in the future, two preventative measures may be considered. One is to desulfurize the gas. Based on tests, however, the desulfurization would have to be carried very far to be successful. The second possibility is to dry the gas to such an extent as to prevent condensate, and this corrosion, from forming no matter how low winter temperatures may drop. This measure was ultimately recommended, deemed more effective and cheaper.

Abstract
Pressure testing of a batch of AISI type 316L stainless steel thermowells intended for use in a nuclear power-plant resulted in the identification of leakage at the tips in 20% of the parts. Radiography at the tip region of representative thermowells showed linear indications along the axes. SEM examination revealed the presence of longitudinally oriented nonmetallic inclusions that were partly retained and partly dislodged. Electron-dispersive x-ray analysis indicated that the inclusions were composed of CaO. Based on the overall chemistry of the inclusion sites, the source of the CaO was determined to be slag entrapment during the steel making process. It was recommended that the thermowell blanks be ultrasonically tested prior to machining and that the design be modified to make internal pressurization possible.

Abstract
A bull gear from a coal pulverizer at a utility failed by rolling-contact fatigue as the result of continual overloading of the gear and a nonuniform, case-hardened surface of the gear teeth. The gear consisted of an AISI 4140 Cr-Mo steel gear ring that was shrunk fit and pinned onto a cast iron hub. The wear and pitting pattern in the addendum area of the gear teeth indicated that either the gear or pinion was out of alignment. Beach marks observed on the fractured surface of the gear indicated that fatigue was the cause of the gear failure. Similar gears should be inspected carefully for signs of cracking or misalignment. Ultrasonic testing is recommended for detection of subsurface cracks, while magnetic particle testing will detect surface cracking. Visual inspection can be used to determine the teeth contact pattern.

Octagonal cast ingots weighing 6.5 tons and made of unalloyed heat treated steel CK 45 according to DIN 17200, and crankshafts forged from these ingots showed internal separations during ultrasonic testing. To determine the cause of defect, an ingot slice and a crank arm were examined metallographically. Investigation showed this was a case where flaky forgings were made from cast ingots with primary grain boundary cracks. This parallelity supports the often expressed opinion that both occurrences have the same origin, i.e. that hydrogen precipitation was the driving force in the formation of primary grain boundary cracks in cast ingots.

Abstract
This paper describes the investigation of a corrosion failure of bottom plates on an aboveground tank used for the storage of potable water. The tank was internally inspected for the first time after six years of service. Paint blisters and rust spots were observed on the bottom plates and first to third course shell plates. Sand blasting and repainting of the bottom plates and first course shell plates was to be used as a remedial measure. However, during the sand blasting, holes and deep pitting were observed on the bottom plates. On-site visual inspection, magnetic flux leakage (MFL) inspection, ultrasonic testing (UT), and evaluation of the external cathodic protection (CP) system were used in the failure analysis. The corrosion products were analyzed using energy-dispersive X-ray analysis (EDAX). The failure is attributed to the ingress of water and its impoundment under the tank bottom along the periphery inside the ring wall and failure of water side epoxy coating. Various measures to prevent such failures in the future are recommended.

Abstract
Waterwall tube failure samples removed from a coal- and oil-fired boiler in service for 12 years exhibited localized underdeposit corrosion and hydrogen damage. EDS and XRD revealed that bulk internal deposits collected from the tubes contained metallic copper which can accelerate corrosion through galvanic effects and can promote hydrogen damage. Ultrasonic testing was recommended to locate tubes with severe gouging and corrosion, which are suspect locations for hydrogen damage. The source of the copper should be identified and future chemical cleaning of the boiler should address its presence in the waterwall tubes.

Abstract
The large steel ring produced for a nuclear application from a billet of 8822 steel was inspected. The large billet was first forged into a doughnut preform in a large press, and then formed into the ring by ring rolling. A straight-beam ultrasonic inspection was instituted and calibrated using the back-surface-reflection method to determine whether adequate ultrasonic penetration was available. Areas of indications were noted at approximately midheight and adjacent to the bore area. An axial angle-beam inspection from the outside was performed, mainly in the area of indications to reveal detectable indications. The indications were not considered serious enough to reject the forgings. A few small indications in the areas tested were revealed by magnetic particle inspection. The area was conditioned by grinding and polishing to obtain an additional inspection at a greater depth from the inside surface. A much more severe condition was revealed after the test. The indications were classified as areas of chemical segregation and nonmetallic inclusions. The ring was considered unsatisfactory for the application and replacement of the defective ring from an acceptable billet was the most economical solution.
