Abstract
On 23 Dec 1997, a portion of the main ore conveyor at a large mine collapsed onto a highway and shut down mine operations. The conveyor structure that collapsed was supported by a steel truss spanning 185 ft. Truss failure occurred just as the conveyor transport rate was increased to 8,260 tph. Under this total loading, which was only slightly above the regular operating condition, a poorly designed and fabricated transition joint in the west lower chord failed, thereby overloading other key structural members and causing the entire truss to collapse. Another contributing cause of the collapse was the transition joint welds, where the fracture originated. They were made with undersized fillet welds, 20% smaller than specified on the original fabrication drawing. Because of the poorly designed joint detail and the deficient welds, both of which concentrated stress and strain in the low ductility direction of the transition joint plate, lamellar tearing of plate material occurred at the boxed I-beam fillet weld attachment. Brittle fracture of this joint precipitated global collapse of the truss structure.

Abstract
In a copper hot water system, a bent pipe was soldered into a straight pipe with twice the diameter. The neighborhood of the soldered joint was covered with corrosion product predominantly blue-green in color, presumably carbonates. When these corrosion products were scratched off it was seen that the copper beneath this layer had not suffered noticeable attack. The object of the examination was the localized deep cavities located almost symmetrically to both sides of the inserted end of the narrower tube on the internal wall of the wider tube which had in one place been eaten right through. The symmetrical location on each side of the point of insertion of the narrower pipe and the localized sharp delineation of the attack indicated erosion due to the formation of turbulence. By avoiding sharp transitions and abrupt changes in cross section it is possible to design the pipe work so that localized turbulence is obviated. Degassing and cleansing of the water also would reduce the danger of erosion particularly in the case of softened water, which takes up oxygen and carbon dioxide very readily thus becoming particularly aggressive.

Abstract
A 208 cm (82 in.) ID steel aqueduct (ASTM A572, grade 42, type 2 steel) fractured circumferentially at two points 152 m (500 ft) apart in a section above ground. A year later, another fracture occurred in a buried section 6.4 km (4 mi) away. Both pipes fractured during Jan at similar temperatures and pressures. The pipe had a 24 mm wall thickness, and the hydrostatic head was 331 m (1085 ft). The air temperature was approximately -13 deg C (9 deg F), the water temperature approximately 0.6 deg C (33 deg F), and the steel temperature approximately -4 deg C (25 deg F). The pipe had been shop-fabricated in 12 m (40 ft) lengths, then shop welded into 24 m (80 ft) lengths. Field assembly was with bell-and-spigot joints. Investigation (visual inspection and Charpy V-notch testing) supported the conclusion that brittle fracture of the aqueduct pipe was attributed to a combination of stress concentrations at the toes of the fillet welds due to poor welding technique, including shop welds made without preheat, and a brittle condition of the steel at winter temperatures. Recommendations included revised welding techniques, installation of expansion joints, and the use of steel plate rolled from fully killed ingots.

Abstract
The right landing gear on a twin-turboprop transport aircraft collapsed during landing. Preliminary examination indicated that the failure occurred at a steel-to-aluminum (7014) pinned drag-strut connection due to fracture of the lower set of drag-strut attachment lugs at the lower end of the oleo cylinder housing. Two lug fractures that were determined to be the primary fractures were analyzed. Results of various examinations indicated that stress-corrosion cracking associated with the origins of the principal fractures in the connection was the cause of failure. It was recommended that the design be modified to avoid dissimilar metal combinations of high corrosion potential.

Abstract
An exhaust diffuser assembly failed prematurely in service. The failure occurred near the intake end of the assembly and involved fracture in the diffuser cone (Corten), diffuser in take flange (type 310 stainless steel), diffuser exit flange (type 405 stainless steel), expansion bellows (Inconel 600), and bellows intake flange (Corten). Individual segments of the failed subassemblies were examined using various methods. The analysis indicated that the weld joint in the diffuser intake flange (type 310 stainless steel to Corten steel) contained diffusion-zone solidification cracks. The joints had been produced using the mechanized gas-metal arc welding process. Cracking was attributed to improper control of welding parameters, and failure was attributed to weld defects.

Abstract
During construction of a revolving sky-tower observatory, a 2.4 m (8 ft) diam cylindrical column developed serious circumferential cracks overnight at the 14 m (46 ft) level where two 12 m (40 ft) sections were joined by a girth weld. The temperatures ranged from 12 deg C (53 deg F) to 7 deg C (45 deg F) that night. The column was shop fabricated in 12 m (40 ft) long sections of 19 mm (3/4 in.) thick steel plate of ASTM A36 steel. Crack initiation was caused by high residual stress during girth welding, and the presence of notches formed by the termination of the incomplete welds. Continuation of the cracks was attributed to the brittle condition of the steel when cooled by the night air. A steel with a much lower ductile-to-brittle transition temperature is essential for this type of structure. Other necessary steps include better control of the girth-welding, choice of a more favorable electrode to avoid porosity, careful termination of all welds to avoid formation of notches, and completion of all welds before other sections of the column are erected.

Abstract
Uncoated high-strength alloy steel cap screws retaining a cast aluminum (356.0) diffuser assembly in a centrifugal refrigerant compressor failed in a brittle manner a short time after the system was placed in operation. Evidence obtained during the failure analysis indicated that the failures were the result of hydrogen embrittlement produced by galvanic corrosion and attendant evolution of hydrogen at the dissimilar junction, which was also the site of the highest tensile stress. Suggested measures for minimizing recurrences included use of lower-strength, galvanically-compatible fasteners and appropriately-applied and treated compatible coatings.

Abstract
A backwell tube situated in the combustion chamber of a 100 atm boiler, which had been in service for many years, failed. The temperature of the saturated steam was about 300 deg C. Two pipe sections with attacked areas in the circumferential welding joint were examined for cause of failure. First section showed strong pit or trench-like attack in the welding seam on the inner surface. A bluish-black corrosion product adhered to the pits. The second section showed small blisters at the welding seam. The metallographic examination of the first section showed welding seam was strongly reduced in bulk from the inside and covered with a thick crumbling layer of magnetic iron oxide (Fe3-O4). This was a corrosion product resulting from the operation of the boiler. In addition, it was decarburized from the inside, and interspersed with grain boundary cracks. This form of attack is typical for the decarburization of steel by high-pressure hydrogen. Hence, the defects in the pipe sections were the result of scaling during the operation of the steam boiler. It was recommended to avoid unnecessary overheating during the welding of materials for high-pressure steam boiler operations.

Abstract
A detailed failure analysis was conducted on an ammonia refrigerant condenser tube component that failed catastrophically during its initial hours of operation. Evidence collected clearly demonstrated that the weld between a pipe and a dished end contained a sharp unfused region at its root (lack of penetration). Component failure had started from this weld defect. The hydrogen absorbed during welding facilitated crack initiation from this weld defect during storage of the component after welding. Poor weld toughness at the low operating temperature facilitated crack growth during startup, culminating in catastrophic failure as soon as the crack exceeded critical length.

Abstract
A spindle made of hardenable 13% chromium steel X40 Cr13 (Material No. 1.4034) that was fastened to a superheated steam push rod made of high temperature structural steel 13Cr-Mo44 (Material No. 1.7335) by means of a convex fillet weld, fractured at the first operation of the rod directly next to the weld bead. Investigation showed that the fracture of the superheated steam push rod spindle was caused by hardening and hardening crack formation in the weld seams and adjoining areas. It would have been preferable to avoid welding near the cross sectional transitions altogether in consideration of the crack sensitivity of high hardenability steels. If for some reason this was not possible, then all precautions should have been taken that are applicable to the particular steel, such as preheating, slow cooling and stress relief tempering after welding. The selection of an austenitic additive material should have been considered because it could have equalized stresses due to its high elongation. Most probably, however, a material of lower hardenability should have been selected for the spindle if high operating properties were of paramount importance.

Abstract
High failure rates in the drive shafts of 40 newly acquired articulated buses was investigated. The drive shafts were fabricated from a low-carbon (0.45%) steel similar to AISI 5046. Investigators examined all 40 buses, discovering six different drive shaft designs across the fleet. All of the failures, a total of 14, were of the same type of design, which according to finite-element analysis, produces a significantly higher level of stress. SEM examination of the fracture surface of one of the failed drive shafts revealed fatigue striations near the OD and ductile dimpling near the ID, evidence of high-cycle fatigue. Based on the failure rate and fatigue life predictions, it was recommended to discontinue the use of drive shafts with the inferior design.

Abstract
A recuperator used for preheating the combustion air for a rolling mill furnace failed after a relatively short service time because of leakage of the pipes in the colder part. The 6 % chrome steel pipes used for the warmer part connected by means of welding with austenitic electrodes to the unalloyed mild steel pipe of larger diam. Visual inspection showed corrosion and deep, trench-like erosion over the entire circumference of the seam on the side of the thicker mild steel pipe. Examination using the V2-A solution for picral etch showed the microstructure of the unalloyed pipe had become coarse-grained and acicular, and the microstructure of the welding seam had become predominantly martensitic as a result of the mixing of the weld metal with the fused pipe material. The chrome steel pipe had become partially transformed to martensite or bainite at the transition to the weld. Thus, the failure occurred due to typical contact corrosion wherein the alloyed welding seam represented the less noble electrode. The martensitic structure may have contributed to the failure as well. Due to the typical nature of the failure, no recommendations were made.

Abstract
Three bearing bosses from the cover of scrap shears were sent in for examination. They had torn off the base plate to which they had been welded by fillet welds all around. Two of these were examined. They showed entirely the same symptoms. The bosses had broken away on three sides along the welds. The cleaved fractures in the burned notches propagated partially above and partially below several incipient cracks which may have been fatigue fractures. Metallographic sections showed that the fractures had occurred either at the burned notches near the transition from the weld to the sheet, or else they ran in the sheet material next to the weld. The quality of the welds could not be judged because the opposite fracture pieces to which they adhered had not been sent in. It was concluded that the breakaway of these bosses was at least favored by overheating and hardening.

Abstract
A nozzle in a wastewater vaporizer began leaking after approximately three years of service with acetic and formic acid wastewaters at 105 deg C (225 deg F) and 414 kPa (60 psig). The shell of the vessel was weld fabricated from 6.4 mm (0.25 in.) E-Brite stainless steel plate and measured 1.5 m (58 in.) in diameter and 8.5 m (28 ft) in length. Investigation (visual inspection, chemical analysis, radiography, dye-penetrant inspection, and hydrostatic testing of all E-Brite welds, 4x images, 100x/200x images electrolytically etched with 10% oxalic acid, and V-notch Charpy testing) supported the conclusion that failure of the nozzle weld was the result of intergranular corrosion caused by the pickup of interstitial elements and subsequent precipitation of chromium carbides and nitrides. Carbon pickup was believed to have been caused by inadequate joint cleaning prior to welding. The increase in the weld nitrogen level was a direct result of inadequate argon gas shielding of the molten weld puddle. Two areas of inadequate shielding were identified: improper gas flow rate for a 19 mm (0.75 in.) diam gas lens nozzle, and contamination of the manifold gas system. Recommendations included changes in the cleaning and welding process.

Abstract
Several wires in aluminum conductor cables fractured within 5 to 8 years of, service in Alaskan tundra. The cables were comprised of 19-wire strands; the wires were aluminum alloy 6201-T81. Visual and metallographic examinations of the cold-upset pressure weld joints in the wires established that the fractures were caused by fatigue loading attributable to wind/thermal factors at the joints. The grain flow at the joints was transverse to the wire axis, rendering the notches of the joints sensitive to fatigue loading. An additional contributory factor was intergranular corrosion, which assisted fatigue crack initiation/propagation. The failure was attributed to the departure of conductor quality from the requirements of ASTM B 398 and B 399, which specify that “no joints shall be made during final drawing or in the finished wire” and that the joints should not be closer than 15 m (50 ft). The failed cable did not meet either criterion. It was recommended that the replacement cable be inspected for strict compliance to ASTM requirements.
