Abstract
Microstructure, corrosion, and fracture morphologies of prestressed steel wires that failed in service on concrete siphons at the Central Arizona Project (CAP) are discussed. The CAP conveys water for municipal, industrial, and agricultural use through a system of canals, tunnels, and siphons from Lake Havasu to just south of Tucson, AZ. Six siphons were made from prestressed concrete pipe units 6.4 m (21 ft) in diam and 7.7 m long, making them the largest circular precast structures ever built. The pipe was manufactured on site and consisted of a 495-mm thick concrete core, wrapped with ASTM A648 steel prestressing wire. All of the CAP failures evaluated were attributed to corrosion. Longitudinal splits reduced the service life of the pipe significantly by facilitating corrosion and introducing sharp cracks into the microstructure of the wire. A few failures were attributed to general corrosion, where the cross section of the wire is reduced until the strength of the wire is exceeded. Most of the failures evaluated were attributed to stress-corrosion cracking.

Abstract
On 21 April 1995, the contents of a large blender (6 cu m) reacted and caused an explosion that killed and injured a number of workers at a plant in Lodi, NJ. A mixture of sodium hydrosulfite and aluminum powder was being mixed at the time of the accident. This report focuses on evaluations of the blender to determine if material or mechanical failures were the cause of the accident. The results indicate that the mixing vessel was metallurgically sound and did not contribute to the initiation of the failure. However, the vessel was not designed for mixing chemicals that must be isolated from water and excessive heat. Water leaking into the vessel through a graphite seal may have initiated the reactions that caused the accident.

Abstract
When a large LPG low-carbon steel storage tank was put into service for the first time and filled beyond the proof testing level, a brittle fracture crack initiated at a fillet weld between a stiffener ring and the wall. The crack propagated to a length of 5.5 m and arrested. Analysis showed that the plates satisfied the criteria of BS 4741. It was concluded that the cause of crack initiation was the lack of a mouse hole at the junction between the stiffening ring and the wall of the tank. The tank was repaired and put back in service. When it was filled beyond the proof test level, again a brittle crack was initiated at a horizontal weld defect and propagated vertically, destroying the tank and the liquefaction plant. The initiation site was a thumbnail elliptical crack in a horizontal weld, having a depth of 1.5 mm, and a length of 4.5 mm. This showed that as late the mid-1970s, misunderstanding of brittle fracture led to the wrong design and construction of an LPG storage tank. Abstract
This article considers two mechanisms of cavitation failure: those for ductile materials and those for brittle materials. It examines the different stages of cavitation erosion. The article explains various cavitation failures including cavitation in bearings, centrifugal pumps, and gearboxes. It provides information on the cavitation resistance of materials and other prevention parameters. The article describes two American Society for Testing and Materials (ASTM) standards for the evaluation of erosion and cavitation, namely, ASTM Standard G 32 and ASTM Standard G 73. It concludes with a discussion on correlations between laboratory results and service.

This paper summarizes several cases of metallurgical failure analysis of surgical implants conducted at the Laboratory of Failure Analysis of IPT, in Brazil. Investigation revealed that most of the samples were not in accordance with ISO standards and presented evidence of corrosion assisted fracture. Additionally, some components were found to contain fabrication/processing defects that contributed to premature failure. The implant of nonbiocompatible materials results in immeasurable damage to patients as well as losses for the public investment. It is proposed that local sanitary regulation agencies create mechanisms to avoid commercialization of surgical implants that are not in accordance with standards and adopt the practice of retrieval analysis of failed implants. This would protect the public health by identifying and preventing the main causes of failure in surgical implants.

Abstract
Friction and wear are important when considering the operation and efficiency of components and mechanical systems. Among the different types and mechanisms of wear, adhesive wear is very serious. Adhesion results in a high coefficient of friction as well as in serious damage to the contacting surfaces. In extreme cases, it may lead to complete prevention of sliding; as such, adhesive wear represents one of the fundamental causes of failure for most metal sliding contacts, accounting for approximately 70% of typical component failures. This article discusses the mechanism and failure modes of adhesive wear including scoring, scuffing, seizure, and galling, and describes the processes involved in classic laboratory-type and standardized tests for the evaluation of adhesive wear. It includes information on standardized galling tests, twist compression, slider-on-flat-surface, load-scanning, and scratch tests. After a discussion on gear scuffing, information on the material-dependent adhesive wear and factors preventing adhesive wear is provided.

Abstract
A hollow, splined alloy steel aircraft shaft (machined from an AMS 6415 steel forging – approximately the same composition as 4340 steel – then quenched and tempered to a hardness of 44.5 to 49 HRC) cracked in service after more than 10,000 h of flight time. The inner surface of the hollow shaft was exposed to hydraulic oil at temperatures of 0 to 80 deg C (30 to 180 deg F). Analysis (visual inspection, 15-30x low magnification examination, 4x light fractograph, chemical analysis, hardness testing) supported the conclusions that the shaft cracked in a region subjected to severe static radial, cyclic torsional, and cyclic bending loads. Cracking originated at corrosion pits on the smoothly finished surface and propagated as multiple small corrosion-fatigue cracks from separate nuclei. The originally noncorrosive environment (hydraulic oil) became corrosive in service because of the introduction of water into the oil. Recommendations included taking additional precautions in operation and maintenance to prevent the use of oil containing any water through filling spouts or air vents. Also, polishing to remove pitting corrosion (but staying within specified dimensional tolerances) was recommended as a standard maintenance procedure for shafts with long service lives.

Abstract
This report covers case histories of failures in fixed-wing light aeroplane and helicopter components. A crankshaft of AISI 4340 Ni-Cr-Mo alloy steel, heat treated and nitrided all over, failed in bending fatigue. The nitrided layer was ground too rapidly causing excessive heat generation which induced grinding cracks and grinding burn. Tensional stresses resulting from grinding developed in a thin surface layer. On another crankshaft, chromium plating introduced undesirable residual tensile stresses. Such plating is an unsatisfactory finish for crankshafts of aircraft engines. Aircraft engine manufacturers and aeronautical standards require magnetic particle inspection to detect grinding cracks after reconditioning. Abstract
Socket head cap screws used in a naval application were failing in service due to delayed fracture. The standard ASTM A 574 screws were zinc plated and dichromate coated. Investigation (visual inspection, 1187 SEM images, chemical analysis, and tension testing) of both the failed screws and two unused, exemplar fasteners from the same lot supported the conclusion that the cap screws appear to have failed due to hydrogen embrittlement, as revealed by delayed cracking and intergranular fracture morphology. Static brittle overload fracture occurred due to the tension preload, and prior hydrogen charging that occurred during manufacturing. The probable source of charging was the electroplating, although postplating baking was reportedly performed as well. Recommendations included examining the manufacturing process in detail.

Abstract
Linear indications on the outer surface of a cross in a piping system were revealed by dye-penetrant examination. The cross was specified to be SA403 type WP 304 stainless steel. The cross had been subjected to induction-heating stress improvement. The linear indications on the cross were located in wide bands running circumferentially below the cross-to-cap weld and above the cap-to-discharge-pipe weld. The material was found to conform to the requirements both in terms of hardness and strength. Intergranular cracks filled with oxide were observed on metallographic analysis of a sectioned and oxalic acid etched sample. The grain size was found to exceed the ASTM standard. No indications of sensitization were observed during testing with practice A of ASTM A 262. Definitive evidence of contaminants to support SCC as the failure mechanism was not disclosed during analysis. It was concluded that overheating or burning of the forging, which classically results in large grain size, intergranular fractures, and fine oxide particles dispersed throughout the grains was the possible reason for the failure.

Abstract
A 1-in. diam pump spindle fractured within the length covered by the boss of the impeller which was attached to the spindle by means of an axial screw. The pump had been in use in a chemical plant handling mixtures of organic liquids and dilute sulfuric acid having a pH value of 2 to 4 at temperatures of 80 to 90 deg C (176 to 194 deg F). The fracture was unusual in that it was of a fibrous nature, the fibers-which were orientated radially-were readily detachable. The surface of the spindle adjacent to the fracture had an etched appearance and the mode of cracking in this region suggested that failure resulted from an intergranular attack. Subsequent microscope examination confirmed the generally intergranular mode of failure. A macro-etched section near the fracture revealed a radial arrangement of columnar crystals, indicating that the spindle was a cast and not a wrought product as had been presumed. Spectroscope examination showed this particular composition (Fe-23Cr-18Ni-1.8Mo-1.2Si) did not conform to a standard specification and is apparently a proprietary alloy. Abstract
A fan support casting failed unexpectedly while running at 1800 rpm in pulp at 65 deg C (150 deg F). The leading edge of the blade exhibited deep spongy holes leading to reduced section and finally to fracture of the part when the remaining section size was insufficient to support the load. Analysis showed the support casting to be a standard 8620 type composition with a hardness of 311 HRB. The design of the casting was not streamlined. There were several square corners present where great pressure differences could be generated. This was a case of erosion-corrosion with the classic spongy appearance of cavitation. Two changes were proposed: streamlining the part to avoid abrupt changes in fluid flow; and a change in alloy to a more corrosion-resistant material (304 or preferably 316) to increase the tenacity of protective films.

Abstract
Cracks, with no other damage, were observed in a niobium alloy (Nb-106) part when it was pulled from several months of protective storage for assembly into a rocket nozzle. SEM views showed the cracks to be intergranular, with contaminant particles on a large number of the grain facets. EDX analysis showed they consisted of niobium and fluorine. Plastic replicas, prepared by standard TEM techniques, were analyzed with selected-area electron diffraction, showing a pattern match for niobium tetrafluoride. Auger analyses showed electron spectra containing peaks representing carbon, oxygen, nitrogen, fluorine, and chlorine. Investigation into the processing history of the part showed the tenacious oxide film formed by the affinity of niobium for oxygen - even when heat treated in a vacuum – was removed with a combination of strong acids: nitric, hydrochloric, hydrofluoric, and lactic, resulting in the contaminants found on the surface. Thus, residues of the cleaning acid on the part had caused SCC during storage, with the tensile stresses necessary to generate SCC assumed to have been residual stresses from the heat treatment. Recommendation was made that more stringent cleaning procedures to remove any trace of the cleaning acids be used.
