Abstract
Failure occurred in two TOW flight missile cases in unrelated launchings. After an extensive investigation, it was concluded that stress corrosion was the most likely cause of failure. Subsequent to this conclusion, inner wall softening was observed in an unfired TOW flight missile case. Questions arose as to how the softening occurred and whether or not it had contributed to the failures. This report contains the results of a study which resolved that inner wall softening could not have been present in the failed missile cases.

Abstract
The crosslinked epoxy resin encapsulant protecting an electromagnetic valve coil failed during long-term storage and was examined to determine the cause. The investigation included fault-tree analysis, FTIR and EDX spectroscopy, and differential scanning calorimetry with thermogravimetric analysis. Based on test data, the epoxy resin had not been properly cured and was hydrolyzed in its compromised state because of humidity. Hence, the depolymerized material gradually softened to the point where the effect of creep caused it to flow, ultimately causing the failure.

Abstract
A type 316L stainless steel “Jewett nail” hip implant failed after 2 months of service. Fracture occurred through the first of five screw holes in the plate section. Microscopic examination of mating fracture surfaces showed that failure had initiated at the outside (convex) surface of the plate and proceeded through its thickness. The fracture morphology was characteristic of fatigue. A beveled area on the inside surface of the plate indicated that the implant had been fractured for some time prior to removal. Metallographic examination of samples cut from the plate section revealed a series of hidden repair welds on the inside surface of the plate in the vicinity of the fracture. Comparison of the microstructure in the area of the fracture with that in an area away from the weld indicated that the repair welding had resulted in the creation of an annealed, softened zone. Manufacturers should never attempt to salvage this type of critical device by welding or any other procedure that might compromise its integrity.

Abstract
To determine the effect of severe service on cast 357 aluminum pistons, a metallurgical evaluation was made of four pistons removed from the engine of the Hawk-Offenhauser car which had been driven by Rich Muther in the first Ontario, California 500 race. The pistons were studied by visual inspection, hardness traverses, radiography, dye penetrant inspection, chemical analysis, macrometallography, optical microscopy, and electron microscopy. The crown of one piston had a rough, crumbly deposit, which was detachable with a knife. Two pistons had remains of carbonaceous deposits. The fourth was severely hammered. It was concluded that the high temperatures developed in this engine created an environment too severe for 357 aluminum. Surfaces were so hot that the low-melting constituent melted. Then, the alloy oxidized rapidly to form Al2O3, an abrasive which further aggravated problems. The temperature in much of the piston was high enough to cause softening by overaging, lowering strength.

Abstract
One of three valves in a dry automatic sprinkler system tripped accidentally, thus activating the sprinklers. Maintenance records showed that the three valves had been in service less than two years. The valve consisted of a cast copper alloy clapper plate that was held closed by a pivoted malleable iron latch. The latch and top surface of the clapper plate were usually in a sanitary-water environment (stabilized, chlorinated well water with a pH of 7.3) under stagnant conditions. Process make-up water that had been clarified, filtered, softened, and chlorinated and had a pH of 9.8 was occasionally used in the system. Analysis (visual inspection and 250x micrograph) supported the conclusions that failure of the latch was caused by plastic deformation from extensive loss of metal by galvanic corrosion and the sudden loading related to the tripping of the valve. Failure in some regions of the contact area was by ductile (transgranular) fracture. Recommendations included changing the latch material from malleable iron to silicon bronze (C87300). The use of silicon bronze prevents corrosion or galvanic attack and proper adjustment of the latch maintains an adequate contact area.

Abstract
The flanged bearing bush carrying the drive shaft of a feed pump suddenly fractured after about two years of service. The chemical composition was normal for high chromium ledeburitic cast steel, which was corrosion and wear resistant as well as refractory. For unknown reasons the rotating shaft came into direct contact with the flange. Mechanical friction caused a rise in temperature on both contact surfaces. This mutual contact lasted long enough for the temperature in the contact zone to exceed 1200 deg C, at which the flange material became softened or molten. As a result, considerable structural changes took place on the inner wall of the flange. Thermal stresses and excessive mechanical loads due to smearing of the flange material then led to fracture of the flange.

Abstract
A natural gas pipeline explosion and subsequent fire significantly altered the pipeline steel microstructure, obscuring in part the primary cause of failure, namely, coating breakdown at a local hard spot in the steel. Chemical analysis was made on pieces cut from the portion of the pipe that did not fracture during the explosion and from piece 5-1 which contained the fracture origin site. Both pieces were found to have 0.30% carbon and 1.2% Mn with sulfur and phosphorus impurities acceptably low. Fracture mechanics analysis used in conjunction with fractographic results confirmed the existence of a very hard spot in the steel prior to the explosion, which was softened significantly in the ensuing fire. This finding allowed the micromechanism leading to fracture to be identified as hydrogen embrittlement resulting from cathodic charging.

Abstract
In a copper hot water system, a bent pipe was soldered into a straight pipe with twice the diameter. The neighborhood of the soldered joint was covered with corrosion product predominantly blue-green in color, presumably carbonates. When these corrosion products were scratched off it was seen that the copper beneath this layer had not suffered noticeable attack. The object of the examination was the localized deep cavities located almost symmetrically to both sides of the inserted end of the narrower tube on the internal wall of the wider tube which had in one place been eaten right through. The symmetrical location on each side of the point of insertion of the narrower pipe and the localized sharp delineation of the attack indicated erosion due to the formation of turbulence. By avoiding sharp transitions and abrupt changes in cross section it is possible to design the pipe work so that localized turbulence is obviated. Degassing and cleansing of the water also would reduce the danger of erosion particularly in the case of softened water, which takes up oxygen and carbon dioxide very readily thus becoming particularly aggressive.

Abstract
The main shaft in a locomotive turbocharger fractured along with an associated bearing sleeve. Visual and fractographic examination revealed that the shaft fractured at a sharp-edged groove between two journals of different cross-sectional area. The dominant failure mechanism was low-cycle rotation-bending fatigue. The bearing sleeve failed as a result of abrasive and adhesive wear. Detailed metallurgical analysis indicated that the sleeve and its respective journal had been subjected to abnormally high temperatures, increasing the amount of friction between the sleeve, bearing bush, and journal surface. The excessive heat also softened the induction-hardened case on the journal surface, decreasing its fatigue strength. Fatigue crack initiation occurred at the root fillet of the groove because of stress concentration.

Abstract
The drive-shaft hanger bearings failed after 300 to 400 h in service. The shaft, supported by labyrinth-sealed single row radial ball bearings of ABEC-1 tolerances, was made of aluminum 2024-T3 tubing (2.5 cm diam and 1.2 mm wall thickness). The bearings were lubricated with a paste-type mineral-oil lubricant (containing molybdenum disulfide and polytetrafluoroethylene particles) or grease conforming to MIL-G-81322 (containing thickening agent and synthetic hydrocarbons) and had two-piece spot-welded retainers. On visual examination, the balls were observed to be embedded in the inner-ring raceway which had been softened by the elevated temperatures reached during the failure. Broken retainers and worn and bent out of shape seals were found. Penetration of gritty particles, water and other corrosive agents and leakage of lubricant out of the bearing permitted by the worn seals was observed. It was concluded that overheating was caused by lubricant flow was permitted by wear of the labyrinth seals. Positive rubbing seals and MIL-G-81322 grease lubricant were found to have longer life than those with the labyrinth seals and mineral-oil-paste lubricant on testing under simulated environmental conditions and were installed as a corrective measure. Importance of dirt free supply and drainage of oil was discussed.

Factors which may lead to premature roller bearing failure in service include incorrect fitting, excessive pre-load during installation, insufficient or unsuitable lubrication, over-load, impact load vibration, excessive temperature, contamination by abrasive matter, ingress of harmful liquids, and stray electric currents. Most common modes of failure include flaking or pitting (fatigue), cracks or fractures, creep, smearing, wear, softening, indentation, fluting, and corrosion. Abstract
Failure analysis results were employed to identify a better alloy. Chipper knives used in the field to chip logs failed frequently. The knives were made of alloys with a composition of Fe-0.48C-0.30Mn-0.90Si-8.50Cr-1.35Mo-1.20W-0.30V. The development of tougher alloy steel with superior properties was initiated. The nominal composition of Fe-0.50C-0.30Mn-0.40Si-5.00Cr-2.00Mo was developed which achieved the goals of edge retention, resistance to softening under frictional heating, wear resistance, ease of heat treatment, dimensional stability in heat treatment, grindability, and low alloy cost. A chip harvester made from this composition was tested in field with older composition knives. It was found that the new knives outperformed the older knives. The key to the development was interpreted to be careful study of a number of failed knives with different problems used in different types of operations.

Abstract
The fatigue failure of a wire rope used on a skip hoist in an underground mine has been studied as part of the ongoing research by the Bureau of Mines into haulage and materials handling hazards in mines. Macroscopic correlation of individual wire failures with wear patterns, fractography, and microhardness testing were used to gain an understanding of the failure mechanism. Wire failures occurred predominantly at characteristic wear sites between strands. These wear sites are identifiable by a large reduction in diameter; however, reduction in area was not responsible for the location of failure. Fractography revealed multiple crack initiation sites to be located at other less noticeable wear sites or opposite the characteristic wear site. Microhardness testing revealed hardening, and some softening, at wear sites.
