Abstract
Pipes made of low-carbon Thomas steel had been welded longitudinally employing the carbon-arc process with bare electrode wire made for argon-shielded arc welding. Difficulties were encountered during the cutting of threads because of the presence of hard spots. Microstructural examination showed welding conditions were such that a carburizing atmosphere developed, which led to an increase in carbon content and hardening at certain locations such as terminal bells and lap joints. This explained the processing difficulties during the threading operation.

Abstract
An outer fan-duct assembly of titanium alloy Ti-5Al-2.5Sn (AMS 4910) for a gas-turbine fan section cracked 75 mm (3 in.) circumferentially through a repair weld in an arc weld in the front flange-duct segment. Examination of the crack with a binocular microscope revealed no evidence of fatigue. A blue etch-anodize inspection showed the presence of an alpha case along the edges of the repair weld. The alpha case, a brittle oxide-enriched layer, forms when welds are inadequately shielded from the atmosphere during deposition. The brittleness of this layer caused transgranular cracks to form and propagate in tension under the thermal stresses created by the repair-weld heat input. The crack resulted from contamination and embrittlement of a repair weld that had received inadequate gas shielding. Thermal stresses cracked the oxide-rich layer that formed. The gas-shielding accessories of the welding torch were overhauled to ensure that leak-in or entrainment of air was eliminated. Also, the purity of the shielding-gas supplies was rechecked to make certain that these had not become contaminated.

Abstract
A steel galvanizing vat measuring 3 x 1.2 x 1.2 m (10 x 4 x 4 ft) and made of 19 mm thick carbon steel plate (ASTM A285, grade B)) at a shipbuilding and ship-repair facility failed after only three months of service. To verify suspected failure cause, two T joints were made in 12.5 mm thick ASTM A285, grade B, steel plate. One joint was welded using the semiautomatic submerged arc process with one pass on each side. A second joint was welded manually by the shielded metal arc process using E6010 welding rod and four passes on each side. The silicon content of the shielded metal arc weld was 0.54%, whereas that of the submerged arc weld was 0.86%. After being weighed, the specimens were submerged in molten zinc for 850 h. Analysis (visual inspection, chemical analysis, 100x 2% nital-etched micrographs) supported the conclusions that the vat failed due to molten-zinc corrosion along elongated ferrite bands, possibly because silicon was dissolved in the ferrite and thus made it more susceptible to attack by the molten zinc. Recommendations included rewelding the vat using the manual shielded metal arc process with at least four passes on each side.

Abstract
A Ti-6Al-4V alloy pressure vessel failed during a proof-pressure test, fracturing along the center girth weld. The girth joints were welded with the automatic gas tungsten arc process utilizing an auxiliary trailing shield attached to the welding torch to provide inert-gas shielding for the exterior surface of the weld. A segmented backup ring with a gas channel was used inside the vessel to shield the weld root. The pressure vessel failed due to contamination of the fusion zone by oxygen, which resulted when the gas shielding the root face of the weld was diluted by air that leaked into the gas channel. Thermal stresses cracked the embrittled weld, exposing the crack surfaces to oxidation before cooling. One of these cracks caused a stress concentration so severe that failure of the vessel wall during the proof test was inevitable. A sealing system at the split-line region of the segmented backup ring was provided, and a fine-mesh stainless steel screen diffuser was incorporated in the channel section of the backup ring to prevent air from leaking in. A titanium alloy color chart was furnished to permit correlation of weld-zone discoloration with the degree of atmospheric contamination.

Abstract
Several of the welds in a hoist carriage tram-rail assembly fabricated by shielded metal arc welding the leg of a large T-section 1020 steel beam to the leg of a smaller T-section 1050 steel rail failed in one portion of the assembly. Four weld cracks and several indefinite indications were found by magnetic-particle inspection. The cracks were revealed by metallographic examination to have originated in the HAZs in the rail section. Cracks in welds and in HAZs resulting from arcing the electrode adjacent to the weld and weld spatter were also revealed. The tram-rail assembly was concluded to have failed by fatigue cracking in HAZs. The fatigue cracking was initiated and propagated by vibration of the tram rail by movement of the hoist carriage on the rail. As a corrective measure, welding procedures were improved and the replacement rail assemblies were preheated and postheated.

Abstract
A nuclear steam-generator vessel constructed of 100-mm thick SA302, grade B, steel was found to have a small leak. The leak originated in the circumferential closure weld joining the transition cone to the upper shell. The welds had been fabricated from the outside by the submerged arc process with a backing strip. The backing was back gouged off, and the weld was completed from the inside with E8018-C3 electrodes by the shielded metal arc process. Striations of the type normally associated with progressive or fatigue-type failures including beach marks that allowed tracing the origin of the fracture to the pits on the inner surface of the vessel were revealed. Copper deposits with zinc were revealed by EDS examination of discolorations. Pitting was revealed to have been caused by poor oxygen control in the steam generators and release of chloride into the steam generators. It was concluded by series of controlled crack-propagation-rate stress-corrosion tests that A302, grade B, steel was susceptible to transgranular stress-corrosion attack in constant extension rate testing with as low as 1 ppm chloride present. It was recommended to maintain the coolant environment low in oxygen and chloride. Copper ions in solution should be eliminated or minimized.

Abstract
A riveted 0.25% carbon steel oil-storage tank in Oklahoma was dismantled and reassembled in Minnesota by welding to form a storage tank for soybean oil. An opening was cut in the side of the tank to admit a front-end loader. A frame of heavy angle iron was welded to the tank and drilled for bolting on a heavy steel plate. The tank was filled to a record height. In mid-Jan the temperature dropped to -31 deg C (-23 deg F), with high winds. The tank split open and collapsed. The welding used the shielded metal arc process with E6010 electrodes, which could lead to weld porosity, hydrogen embrittlement, or both. At subzero temperatures, the steel was below its ductile-to-brittle transition temperature. These circumstances suggest a brittle condition. Steps to avoid this type of failure: For cold conditions, the steel plate should have a low carbon content and a high manganese-to-sulfur ratio and be in a normalized condition, low-hydrogen electrodes and welding practices should be used, all corners should be generously radiused, the welds should be inspected and ground or dressed to minimize stress concentrations, postweld heating is advisable, and radiographic and penetrant inspection tests should be performed.

Abstract
A system of carbon steel headers, handling superheated water of 188 deg C (370 deg F) at 2 MPa (300 psi) for automobile-tire curing presses, developed a number of leaks within about four months after two to three years of leak-free service. All the leaks were in shielded metal arc butt welds joining 200 mm (8 in.) diam 90 deg elbows and pipe to 200 mm (8 in.) diam welding-neck flanges. A flange-elbow-flange assembly and a flange-pipe assembly that had leaked were removed for examination. Investigation (visual inspection, hardness testing, chemical analysis, magnetic-particle testing, radiographic inspection, and 2% nital etched 1.7x views) showed varying IDs on the assemblies and supported the conclusions that the failures of the butt welds were the result of fatigue cracks caused by cyclic thermal stresses that initiated at stress-concentrating notches at the toes of the interior fillet welds on the surfaces of the flanges. Recommendations included using ultrasonic testing to identify the appropriate joints and then replacing them. Special attention to accuracy of fit-up in the replacement joints was also recommended to achieve smooth, notch-free contours on the interior surfaces.

Abstract
One of five underground drain lines intended to carry a highly acidic effluent from a chemical-processing plant to distant holding tanks failed in just a few months. Each line was made of 304L stainless steel pipe 73 mm (2 in.) in diam with a 5 mm (0.203 in.) wall thickness. Lengths of pipe were joined by shielded metal arc welding. Soundness of the welded joints was determined by water back-pressure testing after several lengths of pipe had been installed and joined. Before completion of the pipeline, a pressure drop was observed during back-pressure testing. An extreme depression in the backfill revealed the site of failure. Analysis (visual inspection, electrical conductivity, and soil analysis) supported the conclusions that the failure had resulted from galvanic corrosion at a point where the corrosivity of the soil was substantially greater than the average, resulting in a voltage decrease near the point of failure of about 1.3 to 1.7 V. Recommendations included that the pipelines be asphalt coated and enclosed in a concrete trough with a concrete cover. Also, magnesium anodes, connected electrically to each line, should be installed at periodic intervals along their entire length to provide cathodic protection.

Abstract
An unacceptable degree of porosity was identified in several closure welds on stainless steel containers for plutonium-bearing materials. The pores developed in the weld tie-in region due to gas trapped by the weld pool during the closure process. This paper describes the efforts to trace the root cause of the porosity to the geometric conditions of the weld joint and establish corrective actions to minimize such porosity.

Abstract
This article briefly reviews the general causes of weldment failures, which may arise from rejection after inspection or failure to pass mechanical testing as well as loss of function in service. It focuses on the general discontinuities observed in welds, and shows how some imperfections may be tolerable and how the other may be root-cause defects in service failures. The article explains the effects of joint design on weldment integrity. It outlines the origins of failure associated with the inherent discontinuity of welds and the imperfections that might be introduced from arc welding processes. The article also describes failure origins in other welding processes, such as electroslag welds, electrogas welds, flash welds, upset butt welds, flash welds, electron and laser beam weld, and high-frequency induction welds.

Abstract
A nozzle in a wastewater vaporizer began leaking after approximately three years of service with acetic and formic acid wastewaters at 105 deg C (225 deg F) and 414 kPa (60 psig). The shell of the vessel was weld fabricated from 6.4 mm (0.25 in.) E-Brite stainless steel plate and measured 1.5 m (58 in.) in diameter and 8.5 m (28 ft) in length. Investigation (visual inspection, chemical analysis, radiography, dye-penetrant inspection, and hydrostatic testing of all E-Brite welds, 4x images, 100x/200x images electrolytically etched with 10% oxalic acid, and V-notch Charpy testing) supported the conclusion that failure of the nozzle weld was the result of intergranular corrosion caused by the pickup of interstitial elements and subsequent precipitation of chromium carbides and nitrides. Carbon pickup was believed to have been caused by inadequate joint cleaning prior to welding. The increase in the weld nitrogen level was a direct result of inadequate argon gas shielding of the molten weld puddle. Two areas of inadequate shielding were identified: improper gas flow rate for a 19 mm (0.75 in.) diam gas lens nozzle, and contamination of the manifold gas system. Abstract
This article describes some of the welding discontinuities and flaws characterized by nondestructive examinations. It focuses on nondestructive inspection methods used in the welding industry. The sources of weld discontinuities and defects as they relate to service failures or rejection in new construction inspection are also discussed. The article discusses the types of base metal cracks and metallurgical weld cracking. The article discusses the processes involved in the analysis of in-service weld failures. It briefly reviews the general types of process-related discontinuities of arc welds. Mechanical and environmental failure origins related to other types of welding processes are also described. The article explains the cause and effects of process-related discontinuities including weld porosity, inclusions, incomplete fusion, and incomplete penetration. Different fitness-for-service assessment methodologies for calculating allowable or critical flaw sizes are also discussed.

Abstract
A main steam pipe was found to be leaking due to a large circumferential crack in a pipe-to-fitting weld in one of two steam leads between the superheater outlet nozzles and the turbine stop valves (a line made of SA335-P22 material). The main crack surface was found to be rough, oriented about normal to the outside surface, and had a dark oxidized appearance. The cracking was found to be predominantly intergranular. Distinct shiny bands that etched slower than the remainder of the sample at the top of each individual weld bead were revealed by microscopic examination. These bands were found contain small cracks and microvoids. A mechanism of intergranular creep rupture at elevated temperature was identified as a result of a series of stress-rupture and tensile tests. It was revealed by the crack shape that cracking initiated on the pipe exterior, then propagated inward and in the circumferential direction in response to a bending moment load. It was concluded that the primary cause of failure was the occurrence of bending stresses that exceeded the stress levels predicted by design calculations and that were higher than the maximum allowable primary membrane stress.

Abstract
Nondestructive metallographic examination of materials frequently must be performed on-site when the component in question cannot be moved or destructively examined. Often, it is imperative that specific microstructural information (i.e., material type, heat treatment condition, homogeneity, etc.) be obtained either before initial use of a component, or before the use of a component can be safely resumed. In this paper, the use of standard metallurgical laboratory equipment, and the procedures required to conduct nondestructive on-site metallographic analyses of engineering materials, is presented. As an example, the materials and metallographic techniques employed in an actual on-site investigation of a gas tungsten-arc weldment joining two large diameter Ti-6Al-4V alloy cylinders are discussed in depth to illustrate what can be accomplished.

This case study describes the failure analysis of a steel nozzle in which cracking was observed after a circumferential welding process. The nozzle assembly was made from low-carbon CrMoV alloy steel that was subsequently single-pass butt welded using gas tungsten arc welding. Although no cracks were found when the welds were visually inspected, X-ray radiography showed small discontinuous surface cracks adjacent to the weld bead in the heat affected zone. Further investigation, including optical microscopy, microhardness testing, and residual stress measurements, revealed that the cracks were caused primarily by the presence of coarse untempered martensite in the heat affected zone due to localized heating. The localized heating was caused by high welding heat input or low welding speed and resulted in high transformation stresses. These transformation stresses, working in combination with thermal stresses and constraint conditions, resulted in intergranular brittle fracture.
