Abstract
During disassembly of an engine that was to be modified, a fractured turbine blade was found. When the fracture was examined at low magnification, it was observed that a fatigue fracture had originated on the concave side of the leading edge and had progressed slightly more than halfway from the leading edge to the trailing edge on the concave surface before ultimate failure occurred in dynamic tension. Analysis (including visual inspection, SEM, and 250x/500x micrographic examination) supported the conclusions that the blades failed due to thermal fatigue. Recommendations included application of a protective coating to the blades, provided the coating was sufficiently ductile to avoid cracking during operation to prevent surface oxidation. Such a coating would also alleviate thermal differentials, provided the thermal conductivity of the coating exceeded that of the base metal. It was also determined that directionally solidified blades could minimize thermal fatigue cracking by eliminating intersection of grain boundaries with the surface. However, this improvement would be more costly than applying a protective coating.

Abstract
A failed H-11 tool steel pylon attachment stud was found during a routine walk-around inspection. The stud exhibited gross localized corrosion pitting at several different areas on its surface. Light general rust was also evident. Severe pitting occurred near the fracture location. The fracture face contained evidence of intergranular SCC as well as ductile dimples. The protective coating was found to be an inorganic water-base aluminide coating having a coating thickness of 7.5 to 13 micron (0.3 to 0.5 mil). The coating was of a nonuniform mottled nature. It was concluded that the failure of the pylon attachment stud was caused by general corrosion followed by SCC. The stud was not adequately protected against corrosion by the coating. It was recommended that the coating be applied to a thickness of 38 to 75 micron (1.5 to 3 mil) to provide long-time corrosion resistance. The coating must be either burnished or cured at 540 deg C (1000 deg F) to provide cathodic protection to the steel. Other coatings, such as cadmium or aluminum, were also recommended if a thinner coating is needed.

This article explains a number of potential degradation processes, namely, oxidation, carburization and metal dusting, sulfidation, hot corrosion, chloridation, hydrogen interactions, molten metals, molten salts, and aging reactions including sensitization, stress-corrosion cracking, and corrosion fatigue. It concludes with a discussion on various protective coatings, such as aluminide coatings, overlay coatings, thermal barrier coatings, and ceramic coatings.

Abstract
During a routine shear-pin check, the end lug on the barrel of the forward canopy actuator on a naval aircraft was found to have fractured. The lug was forged from aluminum alloy 2014-T6. Investigation (visual inspection, 2x views, and 140X micrographs etched with Keller's reagent) supported the conclusion that the cause of failure was SCC resulting from exposure to a marine environment. The fracture occurred in normal operation at a point where damage from pitting and intergranular corrosion acted as a stress raiser, not because of overload. The pitting and intergranular attack on the lug were evidence that the surface protection of the part had been inadequate as manufactured or had been damaged in service and not properly repaired in routine maintenance. Recommendations included anodizing the lug and barrel in sulfuric acid and giving them a dichromate sealing treatment, followed by application of a coat of paint primer. During routine maintenance checks, a careful examination was suggested to look for damage to the protective coating, and any necessary repairs should be made by cleaning, priming, and painting. Abstract
Aluminide-coated and uncoated IN-713 turbine blades were returned for evaluation after service in a marine environment because of severe corrosion. Based on service time, failure of these blades by corrosive deterioration was considered to be premature. Analysis (visual inspection, 2.7x micrographic examination on sections etched with ferric chloride and hydrochloric acid in methanol) supported the conclusions that the blades failed by hot-corrosion attack. Variation in rate of attack on coated blades was attributed to variation in integrity of the aluminide coating, which had been applied in 1966, when these coatings were relatively new. It is evident that maintaining the integrity of a protective coating could significantly increase the life of a nickel-base alloy blade operating in a hot and corrosive environment.

High-temperature corrosion can occur in numerous environments and is affected by various parameters such as temperature, alloy and protective coating compositions, stress, time, and gas composition. This article discusses the primary mechanisms of high-temperature corrosion, namely oxidation, carburization, metal dusting, nitridation, carbonitridation, sulfidation, and chloridation. Several other potential degradation processes, namely hot corrosion, hydrogen interactions, molten salts, aging, molten sand, erosion-corrosion, and environmental cracking, are discussed under boiler tube failures, molten salts for energy storage, and degradation and failures in gas turbines. The article describes the effects of environment on aero gas turbine engines and provides an overview of aging, diffusion, and interdiffusion phenomena. It also discusses the processes involved in high-temperature coatings that improve performance of superalloy.

Abstract
Brittle intergranular fracture, typical of a hydrogen-induced delayed failure, caused the failure of an AISI 4340 Cr-Mo-Ni landing gear beam. Corrosion resulting from protective coating damage released nascent hydrogen, which diffused into the steel under the influence of sustained tensile stresses. A second factor was a cluster of non-metallic inclusions which had ‘tributary’ cracks starting from them. Also, eyebolts broke when used to lift a light aircraft (about 7000 lb.). The bolt failure was a brittle intergranular fracture, very likely due to a hydrogen-induced delayed failure mechanism. As for the factors involved, cadmium plating, acid pickling, and steelmaking processes introduce hydrogen on part surfaces. As a second contributing factor, both bolts were 10 Rc points higher in hardness than specified (25 Rc), lessening ductility and notch toughness. A third factor was inadequate procedure, which resulted in bending moments being applied to the bolt threads.

Abstract
A section of cast iron water main pipe contained a hole approximately 6.4 x 3.8 cm (2.5 x 1.5 in.). The pipe was laid in clay type soil. Examination revealed severe pitting around the hole and at the opposite side of the outside diam. A macroscopic examination of a pipe section at the hole area showed that the porosity extended a considerable distance into the pipe wall. Metallographic examination revealed a graphite structure distribution expected in centrifugally cast iron with a hypoeutectic carbon equivalent. Chemical analyses of a nonporous sample had a composition typical of cast iron pipe. Chemical analyses of the porous region had a substantial increase in carbon, silicon, phosphorus, and sulfur. The porous appearance and the composition of the soft porous residue confirmed graphitic corrosion. The selective leaching of iron leaves a residue rich in carbon, silicon, and phosphorus. The high sulfur content is attributed to ferrous sulfide from a sulfate reducing bacteria frequently associated with clay soils. Reinforced coal tar protective coating was recommended.

Two 20 MW turbines suffered damage to second-stage blades prematurely. The alloy was determined to be a precipitation-hardening nickel-base superalloy comparable to Udimet 500, Udimet 710, or Rene 77. Typical protective coatings were not found. Test results further showed that the fuel used was not adequate to guarantee the operating life of the blades due to excess sulfur trioxide, carbon, and sodium in the combustion gases, which caused pitting. A molten salt environmental cracking mechanism was also a factor and was enhanced by the working stresses and by the presence of silicon, vanadium, lead, and zinc. A change of fuel was recommended.

Abstract
Piping and structural components used in space launch facilities such as NASA's Kennedy Space Center and the Air Force's Cape Canaveral Air Station face extreme operating conditions. Launch effluent and residue from solid rocket boosters react with moisture to form hydrochloric acid that settles on exposed surfaces as they are being subjected to severe mechanical loads imparted during lift-off. Failure analyses were performed on 304 stainless steel tubing that ruptured under such conditions, while carrying various gases, including nitrogen, oxygen, and breathing air. Hydrostatic testing indicated a burst strength of 13,500 psi for the intact sections of tubing. Scanning electron microscopy and metallographic examination revealed that the tubing failed due to corrosion pitting exacerbated by stress-corrosion cracking (SCC). The pitting originated on the outer surface of the tube and ranged from superficial to severe, with some pits extending through 75% of the tube's wall thickness. The SCC emanated from the pits and further reduced the service strength of the component until it could no longer sustain the operating pressure and final catastrophic fracture occurred. Corrosion-resistant coatings added after the investigation have proven effective in preventing subsequent such failures.

Abstract
This paper describes the investigation of a corrosion failure of bottom plates on an aboveground tank used for the storage of potable water. The tank was internally inspected for the first time after six years of service. Paint blisters and rust spots were observed on the bottom plates and first to third course shell plates. Sand blasting and repainting of the bottom plates and first course shell plates was to be used as a remedial measure. However, during the sand blasting, holes and deep pitting were observed on the bottom plates. On-site visual inspection, magnetic flux leakage (MFL) inspection, ultrasonic testing (UT), and evaluation of the external cathodic protection (CP) system were used in the failure analysis. The corrosion products were analyzed using energy-dispersive X-ray analysis (EDAX). The failure is attributed to the ingress of water and its impoundment under the tank bottom along the periphery inside the ring wall and failure of water side epoxy coating. Various measures to prevent such failures in the future are recommended.

Abstract
A 2000-T6 aluminum alloy bracket failed in a coastal environment because corrosive chlorides got between the bracket and attachment bolt. The material used for the part was susceptible to stress corrosion under the service conditions. Cracking may have been aggravated by galvanic action between aluminum alloy bracket and steel bolt. To preclude or minimize recurrences, fittings in service should be inspected periodically by dye penetrant for signs of cracking on the end face and within the fitting hole and protected with a suitable coating to exclude damaging chlorides. Also, a 2000-T6 aluminum alloy swivel fitting experienced intergranular corrosion fracture as the result of stress-accelerated corrosion. Corrosion began because of a loose fit between the aluminum swivel fitting and steel tube assembly, which caused fretting. Inadequate maintenance and/or abnormal service operation may have loosened the fitting.

Abstract
Sucker-rod pumps are operating in very aggressive environments in oil well production. The combined effect of a corrosive environment and significant mechanical loads contribute to frequent cases of failure of the rod string during operation. Standards and recommendations have been developed to control and avoid those failures. This study presents various failure cases of sucker rods in different applications. The heat treatment of the steel material and the resulting microstructure are an important factor in the behavior of the sucker rod. A spheroidized microstructure presents a weaker resistance to corrosion affecting the rod life. Non-metallic inclusions are a pitting preferential site leading to fatigue crack initiation. Heterogeneous microstructure as banded martensite and ferrite/pearlite decreases the ductility of the material affecting the fatigue propagation resistance.

Aluminum alloy BS.1476-HE.15 by virtue of its high strength and low density finds application in the form of bars or sections for cranes, bridges, and other such structures where a reduction in dead weight load and inertia stresses is advantageous. Bars and sections in H.15 alloy are mostly produced by extrusion. Some material processed this way has been prone to exfoliation corrosion. Extended aging for 24 h at a temperature of 185 deg C (365 deg F) virtually suppresses the tendency for exfoliation corrosion to develop. Also, the use of a sprayed coating, either of aluminum or Al-1Zn alloy, was effective in halting and preventing this form of attack. While alarming, the appearance of exfoliation corrosion provides a valuable warning to the engineer or inspector before a severe weakening of the particular sections has occurred.

This article focuses on common failures of the components associated with the flow path of industrial gas turbines. Examples of steam turbine blade failures are also discussed, because these components share some similarities with gas turbine blading. Some of the analytical methods used in the laboratory portion of the failure investigation are mentioned in the failure examples. Abstract
This article focuses on the mechanisms of microbially induced or influenced corrosion (MIC) of metallic materials as an introduction to the recognition, management, and prevention of microbiological corrosion failures in piping, tanks, heat exchangers, and cooling towers. It discusses the degradation of various protective systems, such as corrosion inhibitors and lubricants. The article describes the failure analysis of steel, iron, copper, aluminum, and their alloys. It also discusses the probes available to monitor conditions relevant to MIC in industrial systems and the sampling and analysis of conditions usually achieved by the installation of removable coupons in the target system. The article also explains the prevention and control strategies of MIC in industrial systems.

Abstract
Helicopter rotor blade components that included the horizontal hinge pin, the associated nut, and the locking washer were examined. Visual examination of the submitted parts revealed that the hinge pin, fabricated from 4340 steel, was broken and that the fracture face showed a flat beach mark pattern indicative of a preexisting crack. The threaded area of the pin had an embedded thread that did not appear to come from the pin. A chemical analysis was conducted on the embedded thread and on an associated attachment to determine the origin of the thread. Analysis showed that the thread and nut were 4140 steel. Scanning electron fractographic examination of the fracture initiation site strongly suggested that the fracture progressed by fatigue. It was concluded that the failure of the horizontal hinge pin initiated at areas of localized corrosion pits. The pits in turn initiated fatigue cracks, resulting in a failure mode of corrosion fatigue. It was recommended that all of the horizontal hinge pins be inspected. Those pins determined to be satisfactory for further use should be stripped of cadmium, shot peened, and coated with cadmium to a minimum thickness of 0.0127 mm (0.0005 in.).

Abstract
Despite extensive aircraft landing gear design analyses and tests performed by designers and manufacturers, and the large number of trouble-free landings, aircraft users have experienced problems with and failures of landing gear components. Different data banks and over 200 failure analysis reports were surveyed to provide an overview of structural landing gear component failures as experienced by the Canadian Forces over the last 20 years on more than 20 aircraft types, and to assess trends in failure mechanisms and causes. Case histories were selected to illustrate typical problems, troublesome failure mechanisms, the role of high strength aluminum alloys and steels, and situations where fracture mechanics analyses provided insight into the failures. The two main failure mechanisms were: fatigue occurring mainly in steel components, and corrosion related problems with aluminum alloys. Very few overload failures were noted. A number of causes were identified: design deficiencies and manufacturing defects leading mainly to fatigue failures, and poor materials selection and improper maintenance as the principal causes of corrosion-related failures. The survey showed that a proper understanding of the failure mechanisms and causes, by thorough failure analysis, provides valuable feedback information to designers, operators and maintenance personnel for appropriate corrective actions to be taken.

Abstract
The truck beam of the left main landing gear (MGL) of a Boeing 707 airplane collapsed on the ground just after the aircraft was unloaded and refueled. The investigation revealed that failure was caused by the propagation of an intergranular crack originating from the bottom of the pit. The crack reached the critical size and caused failure by stress-corrosion cracking (SCC) under static loading conditions in service. The failed beam was protected by a well adhering paint system. However, the presence of adequate amounts of corrosion preventive compound films (CPC) on the surfaces of the failed beam could not be conclusively established because of the long term service exposure and presence of lubricants.

Abstract
Turbine buckets in a 37.5-MW gas turbine made of Udimet 500 superalloy failed in service. The power plant was located 1 km (0.6 miles) from the Pacific Ocean and operated on No. 2 diesel fuel, which was supplied by tanker ship. Turbine bucket failures occurred on three units after 2500 to 6400 h of operation. Investigation (visual inspection, metallographic examination, and stress analysis) supported the conclusion that the differing microstructure of the airfoil resulted in changes in mechanical properties. Because normal operation includes cycling of loads and temperatures, the shroud tip fractured due to thermomechanical fatigue in its degraded state. Recommendations included special chromium or silicon-rich coating to minimize corrosion in gas turbines operating in a marine environment with operating temperatures in the range of type 2 corrosion (650 to 750 deg C, or 1200 to 1380 deg F). Additionally, it was suggested that fuel delivery, handling, and treatment be high quality, to maintain fuel contamination within design limits, and inlet air filtration must be designed for the coastal site. Also, changing the bucket tip by increasing its thickness and changing the casting technique would reduce the stress and make the design more tolerant of corrosion.
