Abstract
A wall section of a carbon steel choke body in gas service at 4400 psig blew out three months after the use of a corrosion inhibitor was stopped. Corrosion damage occurred in ripples, leaving both smoothly polished and unattacked areas. The corrodent in condensate wells was principally carbon dioxide dissolved in water condensed from the gas stream, with organic acids possibly an aggravating factor. A gas analysis showed no other corrosive agents. No metallurgical or fabrication defects were found in the carbon steel part. The mode of attack was corrosion-erosion, caused by the corrosive, high velocity gas flow. The corrosion rate of either the inhibited or uninhibited gas stream was too high for equipment in high pressure gas service. Type 410 (12% Cr) stainless steel was recommended for the choke bodies because other equipment such as valves made of type 410 showed no evidence of corrosion damage after three years' exposure. This change was made five years ago and there have been no failures since.

Abstract
An interstage radiator gas coil began leaking after only 45 days of service. The original brass coil with several aluminum fins was replaced three times but each replacement lasted less than a day. After removing the fins, leaks were found at circumferential cracks. A section of a tube was removed and split, revealing a series of cracks, evenly spaced. Crack spacing coincided with fin spacing, indicating that stresses incurred during installation of the fins promoted failure. Metallographic examination showed intergranular, branched cracking, characteristic of stress corrosion failures, with the cracks starting on the inside surfaces of the tubes. There was no known corrosive agent in the system, and no other corrosion damage could be found. Qualitative tests and spectrographic analysis gave a positive indication for mercury. The spacing of the cracks, the branched intergranular cracking, the rapid failure, and presence of mercury led to the conclusion of stress-corrosion cracking. It was impossible to remove mercury from the system so carbon steel coils were substituted for the brass ones. The carbon steel coils gave failure-free service for over nine years.

Abstract
A carbon steel furnace tube which should have given good service for ten years ruptured after one year. The tube showed obvious swelling at the point of rupture, and the bulged surface of the tube was oxidized at a temperature far above the design temperature. There was little or no loss in wall thickness due to corrosion or scaling, and the tube wall was thinned to a knife edge at the rupture. Metallographic examination showed the condition of the material was satisfactory. The failure was mechanical in nature, typical of short time creep rupture. The localized oxidation indicated improper furnace operation or blockage of the tube. The furnace was checked and found to have a burner tip out of order. After the tip was repaired, localized overheating was minimized and further premature failures did not occur.

Abstract
A case of continual product refinement stimulated by product failures was described. Brittle fracture of gas transmission line pipe steels occurred demonstrating a poor combination of materials, environment, manufacturing and installation problems, and loads. Initial efforts were concentrated towards decreasing the Charpy ductile-to-brittle transition temperature to avoid brittle fracture. It was subsequently revealed that the absorbed energy on the upper shelf of the Charpy energy-temperature curve was critical for arresting a moving crack. Both fracture initiation and fracture propagation were needed be controlled. It was concluded that improved steel processing procedures, chiefly hot-working temperature and deformation control, were also required to optimize microstructure and properties.

Abstract
Several tin plated, low-alloy steel couplings designed to connect sections of 180 mm (7 in.) diam casing for application in a gas well fractured under normal operating conditions. The couplings were purchased to American Petroleum Institute (API) specifications for P-110 material. Chemical analysis and mechanical testing of the failed couplings showed that they had been manufactured to the API specification for Q-125, more stringent specification than P-110, and met all requirements of the application. Fractographic examination showed that the point of initiation was an embrittled region approximately 25 mm (1 in.) from the end of the coupling. The source of the embrittlement was determined to be hydrogen charging during tin plating. Changes in the plating process were recommended.

Abstract
While an attempt was being made to start the diesel engine of a dragline type excavator, a severe explosion took place, the operator unfortunately being killed. The engine was normally started by compressed air from an air bottle or receiver charged to a pressure of 350 psi by a small auxiliary engine driven compressor fitted on the excavators. On an occasion when this had broken down, the air bottle was replenished by the engineer-in-charge from a cylinder of compressed air. The compressor engine remained unserviced and when the starting bottle again became depleted, the excavator operator re-charged it himself from another cylinder without the knowledge of the engineer. The engine was started satisfactorily, and the bottle charged again to 350 psi in readiness for the start on the following day. It was while an attempt was being made to start the engine on this occasion that the explosion took place. The cylinder used by the operator for re-charging the receiver contained not air but oxygen.

Abstract
This article focuses on the mechanisms of microbiologically influenced corrosion as a basis for discussion on the diagnosis, management, and prevention of biological corrosion failures in piping, tanks, heat exchangers, and cooling towers. It begins with an overview of the scope of microbial activity and the corrosion process. Then, various mechanisms that influence corrosion in microorganisms are discussed. The focus is on the incremental activities needed to assess the role played by microorganisms, if any, in the overall scenario. The article presents a case study that illustrates opportunities to improve operating processes and procedures related to the management of system integrity. Industry experience with corrosion-resistant alloys of steel, copper, and aluminum is reviewed. The article ends with a discussion on monitoring and preventing microbiologically influenced corrosion failures.

Abstract
Graphitization, the formation of graphite nodules in carbon and low alloy steels, contributes to many failures in high-temperature environments. Three such failures in power-generating systems were analyzed to demonstrate the unpredictable nature of this failure mechanism and its effect on material properties and structures. In general, the more randomly distributed the nodules, the less effect they have on structural integrity. In the cases examined, the nodules were found to be organized in planar arrays, indicating they might have an effect on material properties. Closer inspection, however, revealed that the magnitude of the effect depends on the relative orientation of the planar arrangement and principle tensile stress. For normal orientation, the effect of embrittlement tends to be most severe. Conversely, when the orientation is parallel, the nodules have little or no effect. The cases examined show that knowledge is incomplete in regard to graphitization, and the prediction of its occurrence is not yet possible.

Abstract
This article provides some new developments in elevated-temperature and life assessments. It is aimed at providing an overview of the damage mechanisms of concern, with a focus on creep, and the methodologies for design and in-service assessment of components operating at elevated temperatures. The article describes the stages of the creep curve, discusses processes involved in the extrapolation of creep data, and summarizes notable creep constitutive models and continuum damage mechanics models. It demonstrates the effects of stress relaxation and redistribution on the remaining life and discusses the Monkman-Grant relationship and multiaxiality. The article further provides information on high-temperature metallurgical changes and high-temperature hydrogen attack and the steps involved in the remaining-life prediction of high-temperature components. It presents case studies on heater tube creep testing and remaining-life assessment, and pressure vessel time-dependent stress analysis showing the effect of stress relaxation at hot spots.

Abstract
The welds joining the liner and shell of a fluid catalytic cracking unit failed. The shell was made of ASTM A515 carbon steel welded with E7018 filler metal. The liner was made of type 405 stainless steel and was plug welded to the shell using ER309 and ER310 stainless steel filler metal. Fine cracks starting inside the weld zone and spreading outward through the weld and toward the surface were observed during examination. Decarburization and graphitization of the carbon steel at the interface was noted. The high carbon level was found to allow martensite to form eventually. The structure was found to be austenitic in the area where the grain-boundary precipitates appeared heaviest. The composition of the precipitates was analyzed using an electron microprobe to reveal presence of sulfur. Microstructural changes in the weld alloy at the interface were interpreted to be caused by dilution of the alloy and the presence of sulfur caused hot shortness. The necessary internal stress to produce extensive cracking was produced by the differential thermal expansion of the carbon and stainless steels. Periodic careful gouging of the affected areas followed by repair welding was recommended.

Abstract
Sudden and unexplained bearing cap bolt fractures were experienced with reduced-shank design bolts fabricated from 42 CrMo 4 steel, quenched and tempered to a nominal hardness of 38 to 40 HRC. Fractographic analysis provided evidence favoring stress-corrosion cracking as the operating transgranular fracture failure mechanism. Water containing H7S was subsequently identified as the aggressive environment that precipitated the fractures in the presence of high tensile stress. This environment was generated by the chemical breakdown of the engine oil additive and moisture ingress into the normally sealed bearing cap chamber surrounding the bolt shank. A complete absence of fractures in bolts from one of the two vendors was attributed primarily to surface residual compressive stresses produced on the bolt shank by a finish machining operation after heat treatment. Shot cleaning, with fine cast shot, produced a surface residual compressive stress, which eliminated stress-corrosion fractures under severe laboratory conditions.

Abstract
This article describes the historical background, uncertainties in structural parameters, classifications, and application areas of probabilistic analysis. It provides a discussion on the basic definition of random variables, some common distribution functions used in engineering, selection of a probability distribution, the failure model definition, and a definition of the probability of failure. The article also explains the solution techniques for special cases and general solution techniques, such as first-second-order reliability methods, the advanced mean value method, the response surface method, and Monte Carlo sampling. A brief introduction to importance sampling, time-variant reliability, system reliability, and risk analysis and target reliabilities is also provided. The article examines the various application problems for which probabilistic analysis is an essential element. Examples of the use of probabilistic analysis are presented. The article concludes with an overview of some of the commercially available software programs for performing probabilistic analysis.

Abstract
Material samples collected from failed booster pumps were analyzed to determine the cause of failure and assess the adequacy of the materials used in the design. The pumps had been in service at a power plant, transporting feedwater from a deaerator to a main turbine boiler. Samples from critical areas of the pump were examined using optical and scanning electron microscopy, electrochemical analysis, and tensile testing. Based on microstructure and morphology, estimated corrosion rates, and particle concentrations in the feedwater, it was concluded that cavitation and erosion were the dominant failure mechanisms and that the materials and processes used to make the pumps were largely unsuited for the application.

Abstract
The circumstances surrounding the in-service failure of a cast Ni-base superalloy (Alloy 713LC) second stage turbine blade and a cast and coated Co-base superalloy (MAR-M302) first stage air-cooled vane in two turbine engines used for marine application are described. An overview of a systematic approach, analyzing the nature of degeneration and failure of the failed components, utilizing conventional metallurgical techniques, is presented. The topographical features of the turbine blade fracture surface revealed a fatigue-induced crack growth pattern, where crack initiation had taken place in the blade trailing edge. An estimate of the crack-growth rate for the stage II fatigue fracture region coupled with the metallographic results helped to identify the final mode of the turbine blade failure. A detailed metallographic and fractographic examination of the air-cooled vane revealed that coating erosion in conjunction with severe hot-corrosion was responsible for crack initiation in the leading edge area.

Abstract
Hydrogen damage is a term used to designate a number of processes in metals by which the load-carrying capacity of the metal is reduced due to the presence of hydrogen. This article introduces the general forms of hydrogen damage and provides an overview of the different types of hydrogen damage in all the major commercial alloy systems. It covers the broader topic of hydrogen damage, which can be quite complex and technical in nature. The article focuses on failure analysis where hydrogen embrittlement of a steel component is suspected. It provides practical advice for the failure analysis practitioner or for someone who is contemplating procurement of a cost-effective failure analysis of commodity-grade components suspected of hydrogen embrittlement. Some prevention strategies for design and manufacturing problem-induced hydrogen embrittlement are also provided.
