Abstract
Two acrylic-coated polymeric motorcycle components exhibited fisheye blemishes after painting. SEM and EDS results showed relatively high levels of sulfur and chlorine associated with the blemishes in both parts. This suggested some adherent residual substances, possibly in the form of processing fluids and/or cleaning agents, were left on the surface just prior to painting and resulted in the observed fisheye blemishes. One of the components also showed evidence of mechanical damage, in addition to detectable iron, which suggests that the part surface may have been damaged from contact with a ferrous material, such as a steel chip.

Abstract
A section from a stainless steel kitchen sink showed a round red stain on its surface after a short time in storage. Several of these sinks were stacked together and separated by felt pads while in storage, and the mating sink showed a nearly identical stain in the same area, while no stains were noted on any of the other sinks in the stack. The stain was located near one of the pads; however, no discoloration was noted at the pad location. EDS analysis showed the base metal to be austenitic stainless steel, such as type 304. FTIR analysis of the stain revealed carboxylic salts and salts of organic acids, in addition to hydroxyl functionality. The discoloration was caused by localized rusting of the sink from prolonged exposure to a chloride containing substance. The appearance suggests that the substance may actually have been a drop of perspiration (sweat) from a factory worker. Based on these findings, one of the recommendations was to use thicker pads between the sinks to allow any such liquids to dry before causing corrosion damage.

Abstract
Bending fatigue caused crack propagation and catastrophic failures at several locations near the welds on the low-carbon steel tubular cargo box frame of police three-wheel motorcycles. ANSYS finite element analysis revealed that bending stresses in some of the frame members were aggravated by poor detail design between vertical and horizontal tubes. Stresses observed in the ANSYS analysis were not sufficient to cause the onset of fatigue. However when compounded by stress concentration factors and in-service dynamic loading, the frame could have been regularly subjected to stresses over the fatigue limit of the material. A strain gage static loading test verified FEM results, and finite element techniques were applied in the design of reinforcing members to renovate the frames. Material properties were determined and welding procedures specified for the reinforcing members. Inspection intervals were devised to avoid future problems.

Abstract
This paper presents a failure analysis of a reverse shaft in the transmission system of an all-terrain vehicle (ATV). The reverse shaft with splines fractured into two pieces during operation. Visual examination of the fractured surface clearly showed cracks initiated from the roots of spline teeth. To find out the cause of fracture of the shaft, a finite element analysis was carried out to predict the stress state of the shaft under steady loading and shock loading, respectively. The steady loading was produced under normal operation, while the shock loading could be generated by an abrupt change of operation such as start-up or sudden braking during working. Results of stress analysis reveal that the highest stressed area coincided with the fractured regions of the failed shaft. The maximum stress predicted under shock loading exceeded the yield strength and was believed to be the stimulant for crack initiation and propagation at this weak region. The failure analysis thus showed that the premature fatigue fracture of the shaft was caused by abnormal operation. Finally, some suggestions to enhance service durability of the transmission system of ATV are discussed.

Abstract
Bearing in mind the three-legged stool approach of device design/manufacturing, patient factors, and surgical technique, this article aims to inform the failure analyst of the metallurgical and materials engineering aspects of a medical device failure investigation. It focuses on the device "failures" that include fracture, wear, and corrosion. The article first discusses failure modes of long-term orthopedic and cardiovascular implants. The article then focuses on short-term implants, typically bone screws and plates. Lastly, failure modes of surgical tools are discussed. The conclusion of this article presents several case studies illustrating the various failure modes discussed throughout.

Abstract
This article describes some of the welding discontinuities and flaws characterized by nondestructive examinations. It focuses on nondestructive inspection methods used in the welding industry. The sources of weld discontinuities and defects as they relate to service failures or rejection in new construction inspection are also discussed. The article discusses the types of base metal cracks and metallurgical weld cracking. The article discusses the processes involved in the analysis of in-service weld failures. It briefly reviews the general types of process-related discontinuities of arc welds. Mechanical and environmental failure origins related to other types of welding processes are also described. The article explains the cause and effects of process-related discontinuities including weld porosity, inclusions, incomplete fusion, and incomplete penetration. Different fitness-for-service assessment methodologies for calculating allowable or critical flaw sizes are also discussed.

Abstract
This article discusses the three legal theories on which a products liability lawsuit is based and the issues of hazard, risk, and danger in the context of liability. It describes manufacturing and design defects of various products. The article explains a design that is analyzed from the human factors viewpoint and details the preventive measures of the defects, with examples. It presents four paramount questions relating to the probability of injury which are asked even when one executes all possible preventive measures carefully and thoroughly.
