Abstract
The presence of secondary, branching intergranular stress-corrosion cracking in a type 440C stainless bearing caused the analyst to overlook the real culprit, which was a mechanically-initiated, primary transgranular crack that propagated through the steel's hard chromium carbide. Failure was actually caused by overload. Had the original conclusion been accepted, a relatively exotic alloy would have been specified. In another case, brass heat exchanger tube failure was automatically attributed to attack by an acidic cleaner, and a decision was made to stop using the solution. A more thorough analysis showed failure was caused by tube vibration. In a third case, a type 304 stainless steel bellows in a test loop was thought to have failed because of chloride stress corrosion. The report concluded with a recommendation that carbon steel be used as an alternative bellows material. Caustic, not chloride, stress corrosion was the culprit. Had material substitutions been made on the original premise of countering chloride stress corrosion, most of the loop's highly stressed components would have eventually failed.

Abstract
Within the first few months of operation of an 8 km (5 mile) long 455 mm (18 in.) diam high-pressure steam line between a coal-fired electricity-generating plant and a paper mill, several of the Inconel 600 bellows failed. The steam line operated at 6030 kPa (875 psi) and 420 deg C (790 deg F). Metallographic sections, energy-dispersive x-ray spectra, chemical analyses, tensile tests, and Auger microscope analyses showed the failed bellows met the specifications for the material. However, investigation also showed entire oxide thickness was contaminated with relatively large amounts of sodium, calcium, potassium, aluminum, and sulfur, alkali, alkali earth, and other contaminants that completely permeated even the thin oxides on the fracture surfaces. Additional investigation of the purity of the steam itself as reported by the power plant showed that corrosion and cracks were ultimately caused by the steam. While under normal operation, the steam's purity posed no problem to the material, during boiler cleaning operations, the generating plant had allowed contamination to get into the steam line.

Abstract
Failure occurred in a steel superheater tube in a power plant. The tube was specified as ASTM A 213 grade T 22, and the reported operating conditions were 13 MPa (1900 psi) at 482 deg C (900 deg F). The tube carried superheated steam and was coal fired. Investigation (visual inspection, 2% nital etched 297x images, chemical analysis, and SEM fractographs) supported the conclusion that the superheater tube failed as a result of long-term overheating. Substantial creep damage reduced the strength of the tube to the point that overload failure occurred. No recommendations were made.

Abstract
At a power plant, C-276 nickel alloy welds (N10276) on a C-276 duct floor completely disappeared in less than half a year. A continuous supply of flue gas came in contact with the closed bypass duct. The unscrubbed combustion products condensed on the cold duct, then the closed damper conducted heat from the chimney and reheated the condensate. Investigation (visual inspection and welded coupon testing) supported the conclusion that the corrosion was caused by “Green Death,” a corrosive medium used to test for pitting resistance (11.9% H2SO4 + 1.3% HCl + 1% FeCl3 + 1% CuCl2) at 103 deg C (217 deg F). Such conditions exist at power plants. Recommendations included repairing the C-276 plates with a 686CPT weld alloy, and if that did not correct the situation, replacing the plates with 686 plate (N06686) welded with 686 CPT.

Abstract
The horizontal heat-exchanger tubes made of copper alloy C70600, in one of two hydraulic-oil coolers in an electric power plant, leaked after 18 months of service. River water was used as the coolant in the heat-exchanger tubes. Several nodules on the inner surface and holes through the tube wall, which appeared to have formed by pitting under the nodules, were revealed by visual examination. Steep sidewalls, which indicated a high rate of attack, were revealed by microscopic examination of a section through the pit which had penetrated the tube wall. The major constituent of reddish deposit on the inner surfaces of the tubes was revealed to be iron oxide and slight manganese dioxide. Effluent from steel mills upstream was indicated by the presence of these and other constituents to be the source of most of the solids found in the tubes. It was concluded that the tubing failed by crevice corrosion. The tubing in the cooler was replaced, and cooling-water supply was changed from river to city water, which contained no dirt to deposit on the tube surfaces. An alternate solution of installing replacement tubes in the vertical position to make deposition of solids from river water less likely was suggested.

Abstract
Four cadmium-plated ASTM A193 grade B studs from a steam line connector associated with a power turbine failed unexpectedly in a nil-ductility manner. Fracture surfaces were covered with a light-colored, lustrous deposit. Optical microscope, SEM, and EDS analyses were conducted on sections from one of the studs and revealed that the coating on the fracture surface was cadmium. The fracture had multiple origins, and secondary cracks also contained cadmium. The fracture topography was intergranular. The failures were attributed to liquid metal embrittlement caused by the presence of a cadmium plating and operating temperatures at approximately the melting point of cadmium. It was recommended that components exposed to the cadmium be replaced.

Abstract
Numerous flaws were detected in a steam turbine rotor during a scheduled inspection and maintenance outage. A fracture-mechanics-based analysis of the flaws showed that the rotor could not be safely returned to service. Material, samples from the bore were analyzed to evaluate the actual mechanical properties and to determine the metallurgical cause of the observed indications. Samples were examined in a scanning electron microscope and subjected to chemical analysis and several mechanical property tests, including tensile, Charpy V-notch impact, and fracture toughness. The material was found to be a typical Cr-Mo-V steel, and it met the property requirements. No evidence of temper embrittlement was found. The analyses showed that the observed flaws were present in the original forging and attributed them to lack of ingot consolidation. A series of actions, including overboring of the rotor to remove indications close to the surface and revision of starting procedures, were implemented to extend the remaining life of the rotor and ensure its fitness for continued service.

Abstract
A power plant using two steam generators (vertical U-tube and shell heat exchangers, approximately 21 m (68 ft) high with a steam drum diameter of 6 m (20 ft)) experienced a steam generator tube rupture. Each steam generator contained 11,012 Inconel alloy 600 (nickel-base alloy) tubes measuring 19 mm OD, nominal wall thickness of 1.0 mm (0.042 in.), and average length of 18 m (57.75 ft). The original operating temperature of the reactor coolant was 328 deg C (621 deg F). A tube removal effort was conducted following the tube rupture event. Investigation (visual inspection, SEM fractographs, and micrographs) showed evidence of IGSCC initiating at the OD and IGA under ridgelike deposits that were analyzed and found to be slightly alkaline to very alkaline (caustic) in nature. Crack oxide analysis indicated sulfate levels in excess of expected values. The analysis supported the conclusion that that the deposits formed at locations that experienced steam blanketing or dryout at the higher levels of the steam generators. Recommendations included steam generator water-chemistry controls, chemical cleaning, and reduction of the primary reactor coolant system temperature.

Abstract
A 44.5 mm (1.75 in.) diam type 321 stainless steel seamless tube in a power-generating turbine failed after 19,000 h in service. The tube was used to carry a mixture of approximately 25% steam and 75% hot air. Three fractured pieces and part of the tube containing the mating fracture surface were examined. Both fractographic and metallographic features revealed that the failure was by thermal fatigue caused by the presence of biaxial thermal stresses on the inner surface of the tube. It was recommended that the steam and air be thoroughly mixed prior to entering the tube to decrease the temperature fluctuations of the inner surface.

Abstract
A main steam pipe was found to be leaking due to a large circumferential crack in a pipe-to-fitting weld in one of two steam leads between the superheater outlet nozzles and the turbine stop valves (a line made of SA335-P22 material). The main crack surface was found to be rough, oriented about normal to the outside surface, and had a dark oxidized appearance. The cracking was found to be predominantly intergranular. Distinct shiny bands that etched slower than the remainder of the sample at the top of each individual weld bead were revealed by microscopic examination. These bands were found contain small cracks and microvoids. A mechanism of intergranular creep rupture at elevated temperature was identified as a result of a series of stress-rupture and tensile tests. It was revealed by the crack shape that cracking initiated on the pipe exterior, then propagated inward and in the circumferential direction in response to a bending moment load. It was concluded that the primary cause of failure was the occurrence of bending stresses that exceeded the stress levels predicted by design calculations and that were higher than the maximum allowable primary membrane stress.

Abstract
Alloy 430 stainless steel tube-to-header welds failed in a heat recovery steam generator (HRSG) within one year of commissioning. The HRSG was in a combined cycle, gas-fired, combustion turbine electric power plant. Alloy 430, a 17% Cr ferritic stainless steel, was selected because of its resistance to chloride and sulfuric acid dewpoint corrosion under conditions potentially present in the HRSG low-pressure feedwater economizer. Intergranular corrosion and cracking were found in the weld metal and heat-affected zones. The hardness in these regions was up to 35 HRC, and the weld had received a postweld heat treatment (PWHT). Metallographic examination revealed that the corroded areas contained undertempered martensite. Fully tempered weld areas with a hardness of 93 HRB were not attacked. No evidence of corrosion fatigue was found. Uneven temperature control during PWHT was the most likely cause of failure.

Abstract
A valve stem made of 17-4 PH (AISI type 630) stainless steel, which was used for operating a gate valve in a steam power plant, failed after approximately four months of service, during which it had been exposed to high-purity water at approximately 175 deg C (350 deg F) and 11 MPa (1600 psi). The valve stem was reported to have been solution heat treated at 1040 +/-14 deg C (1900 +/-25 deg F) for 30 min and either air quenched or oil quenched to room temperature. The stem was then reportedly aged at 550 to 595 deg C (1025 to 1100 deg F) for four hours. Investigation (visual inspection, 0.7x/50x images, hardness testing, reheat treatment, and metallographic examination) supported the conclusion that failure was by progressive SCC that originated at a stress concentration. Also, the solution heat treatment had been either omitted or performed at too high of a temperature, and the aging treatment had been at too low of a temperature. Recommendations included the following heat treatments: after forging, solution heat treat at 1040 deg C (1900 deg F) for one hour, then oil quench; to avoid susceptibility to SCC, age at 595 deg C (1100 deg F) for four hours, then air cool.

Abstract
Three instances involving the failure of aluminum wiring at the service entrance to single-family homes are discussed. Arcing led to a fire which severely damaged a home in one case. In a second, the failure sequence was initiated by water intrusion into the service entrance electrical box during construction of the home. In the third, failure was caused by a marginal installation. Strict adherence to all applicable electrical codes and standards is critical in the case of aluminum wiring. Electrical components not specifically designed for aluminum must never be used with this type of wiring. All doors, panels and similar portions of electrical boxes should be secured to prevent damage to surroundings in the event of an electrical fault. If symptoms of arcing are observed, professional service should be sought. The latest designs of connectors for use with aluminum wiring are less susceptible to deviations in installation practice.

Abstract
In Nov. 1998, the west superheater outlet header at an electricity generating plant began to leak steam. Subsequent investigation revealed the presence of a crack that extended for 360 deg around the full circumference of the header and through the full cross-sectional thickness. The subsequent inspection of this header and the east superheater header revealed the presence of extremely severe ligament cracking. They operated at 2400 psi (16.5 MPa) and a temperature of 540deg C (1005 deg F). Both were fabricated from seamless pipe produced in accordance with ASME Specification SA-335, and the steel was Grade P22, a 2.25Cr-1Mo alloy steel. Visual and metallurgical evaluations showed the cracking in the west superheater outlet header was caused by thermal fatigue. Abstract
A reversible four-way carbon steel flap valve in a thermal power station failed after 7 years of service. The flap had been fabricated by welding two carbon steel plates to both sides of a carbon steel forging. The valve was used for reversing the flow direction of seawater in the cooling system of a condenser. Visual examination of the flap showed crystalline fracture, indicating a brittle failure. Metallographic examination, chemical analyses, and tensile and impact testing indicated that the failure was caused by the notch sensitivity of the forging material, which resulted in low toughness. It was recommended that fully killed carbon steel with a fine-grain microstructure be used. Redesign of the flap to remove the step in the forging that acted as a notch was also recommended.

Abstract
Argonne National Laboratory has conducted analyses of failed components from nuclear power-generating stations since 1974. The considerations involved in working with and analyzing radioactive components are reviewed here, and the decontamination of these components is discussed. Analyses of four failed components from nuclear plants are then described to illustrate the kinds of failures seen in service. The failures discussed are (1) intergranular stress-corrosion cracking of core spray injection piping in a boiling water reactor, (2) failure of canopy seal welds in adapter tube assemblies in the control rod drive head of a pressurized water reactor, (3) thermal fatigue of a recirculation pump shaft in a boiling water reactor, and (4) failure of pump seal wear rings by nickel leaching in a boiling water reactor.

Abstract
Cracks on the outer surface near a hanger lug were revealed by visual inspection of a type 316 stainless steel main steam line of a major utility boiler system. Cracking was found to have initiated at the outside of the pipe wall or immediately beneath the surface. The microstructure of the failed pipe was found to consist of a matrix precipitate array (M23C6) and large s-phase particles in the grain boundaries. A portable grinding tool was used to prepare the surface and followed by swab etching. All material upstream of the boiler stop valve was revealed to have oriented the cracking normally or nearly so to the main hoop stress direction. Residual-stress measurements were made using a hole-drilling technique and strain gage rosettes. Large tensile axial residual stresses were measured at nearly every location investigated with a large residual hoop stress was found for locations before the stop valve. It was concluded using thermal stress analysis done using numerical methods and software identified as CREPLACYL that one or more severe thermal downshocks might cause the damage pattern that was found. The root cause of the failure was identified to be thermal fatigue, with associated creep relaxation.

Abstract
An investigation was conducted to determine the cause of numerous cracks and other defects on the surface of a cast ASTM A743 grade CA-15 stainless steel main boiler feed pump impeller. The surface was examined using a stereomicroscope, and macrofractography was conducted on several cross sections removed from the impeller body. Areas that appeared to have the most severe surface damage were sectioned, fractured open, and examined using SEM. The chemistry of the impeller and an apparent repair weld were also analyzed. The examination indicated that the cracks were shrinkage voids from the original casting process. Surface repair welds had been used to fill in or cover over larger shrinkage cavities. It was recommended that more stringent visual and nondestructive examination criteria be established for the castings.

Abstract
A type 410 stainless steel circulating water pump shaft used in a fossil power steam generation plant failed after more than 7 years of service. Visual examination showed the fracture surface to be coated with a thick, spalling, rust-colored scale, along with evidence of pitting. Samples for SEM fractography, EDS analysis, and metallography were taken at the crack initiation site. Hardness testing produced a value of approximately 27 HRC. The examinations clearly established that the shaft failed by fatigue. The fatigue crack originated at a localized region on the outside surface where pitting and intergranular cracking had occurred. The localized nature of the initial damage indicated that a corrosive medium had concentrated on the surface, probably due to a leaky seal. Reduction of hardness to 22 HRC or lower and inspection of seals were recommended to prevent future failures.
