Abstract
Cracking occurred within the plastic jacket (injection molded from an impact-modified, 15% glass-fiber-reinforced PET resin.) of several assemblies used in a transportation application during an engineering testing regimen which involved cyclic thermal shock (exposing the parts to alternating temperatures of -40 and 180 deg C (-40 and 360 deg F)). Prior to molding, the resin had reportedly been dried at 135 deg C (275 deg F). The drying process usually lasted 6 h, but occasionally, the material was dried overnight. Comparison investigation (visual inspection, 20x SEM views, micro-FTIR, and analysis using DSC and TGA) with non-failed parts supported the conclusion that that the failure was via brittle fracture associated with the exertion of stresses that exceeded the strength of the resin as-molded caused by the disparity in the CTEs of the PET jacket and the mating steel sleeve. The drying process had exposed the resin to relatively high temperatures, which caused substantial molecular degradation, thus limiting the part's ability to withstand the stresses. The drying temperature was found to be significantly higher than the recommendation for the PET resin, and the testing itself exposed the parts to temperatures above the recognized limits for PET.

Wet natural gas was dried by being passed through a carbon steel vessel that contained a molecular-sieve drying agent. The drying agent became saturated after several hours in service and was regenerated by a gas that was heated to 290 to 345 deg C in a salt-bath heat exchanger. The tee joint in the piping between the heat exchanger and the sieve bed failed after 12 months. A hole in the tee fitting and a corrosion product on the inner surface of the pitting was revealed by visual examination. Iron sulfide was revealed by chemical analysis of the scale which indicated hydrogen sulfide attack on the carbon steel. The presence of oxygen was indicated by the carbon and sulfur found in the scale on the piping and in the sieves indicated that oxygen combined with moisture produced conditions for attack of hydrogen sulfide on carbon steel. Turbulence with some effect from the coarse grain size was interpreted to have contributed. The piping material was changed from carbon steel to AISI type 316 stainless steel as it is readily weldable and resistant to corrosion by hydrogen sulfide.

Abstract
Coiled tubing used in drilling operations failed at the halfway point of its estimated fatigue life. The failure was found to be transverse to the tubing axis. Visual examination revealed a flat fracture surface extending 13 mm with the rest of the fracture showing shear lips indicative of tensile overload. The flat portion of the fracture surface was typical of fatigue cracking. Fatigue striations were revealed by SEM. Corrosion pitting on the tubing ID from which the fatigue crack had propagated were observed on closer examination. The corrosion pitting was speculated to have occurred when the tubing was idle and fluids accumulated at the bottom of the tubing wraps. The coiled tubing was concluded to have failed prematurely due to low-cycle fatigue initiated at corrosion pitting sites. Corrosive attack on the coiled tubing was recommended to be reduced by completely removing fluids or modifying the fluids in the tubing or purging by flowing dry nitrogen to dry it out.

Abstract
A CF-8M (cast type 316) neck liner or manway was removed from the top of a digester vessel. Repeated attempts to repair the part in the field during its life cycle of many years had failed to keep the unit from leaking. The casting was a CF-8M modified with the molybdenum level at the top end of the range. The plate was standard 317L material. The filler metal was type 316, although marginal in molybdenum content. Investigation (visual inspection, chemical analysis, micrographs, and metallographic examination) supported the conclusion that the damage to the neck liner was due to Cl-SCC in an area of debris buildup. It appeared the original casting suffered SCC in a low-oxygen area high in chlorides from repeated wet/dry cycles where there was a buildup of debris. Recommendations included redesigning the neck liner to eliminate the abrupt change where there was debris buildup. If redesign was impossible, an alloy more resistant to Cl-SCC, such as a duplex stainless steel or a high-molybdenum (4 to 6%) austenitic stainless steel, should be used.

Abstract
A paper drier head manufactured from gray cast iron was removed from service as a result of NDE detection of crack-like surface discontinuities. This component was subjected to internal steam pressure to provide heat energy for drying. Investigation (visual inspection, chemical analysis, mechanical testing, as-polished 54x magnification, etched with nital 33x/54x/215x/230x magnification) supported the conclusions that the NDE indications were the consequence of a cold-shut condition in the casting. The cold shut served as a stress-concentration site and was therefore a potential source of crack initiation. The combination of low material strength and a casting defect was a potential source of unexpected fracture during service, because the component was under pressure from steam. Recommendations included removing other dryer heads exhibiting similar discontinuities and/or material quality from service.

Abstract
A section from a stainless steel kitchen sink showed a round red stain on its surface after a short time in storage. Several of these sinks were stacked together and separated by felt pads while in storage, and the mating sink showed a nearly identical stain in the same area, while no stains were noted on any of the other sinks in the stack. The stain was located near one of the pads; however, no discoloration was noted at the pad location. EDS analysis showed the base metal to be austenitic stainless steel, such as type 304. FTIR analysis of the stain revealed carboxylic salts and salts of organic acids, in addition to hydroxyl functionality. The discoloration was caused by localized rusting of the sink from prolonged exposure to a chloride containing substance. The appearance suggests that the substance may actually have been a drop of perspiration (sweat) from a factory worker. Based on these findings, one of the recommendations was to use thicker pads between the sinks to allow any such liquids to dry before causing corrosion damage.

Abstract
A plastic bracket exhibited relatively brittle material properties, which ultimately led to catastrophic failure. The part had been injection molded from a medium-viscosity polycarbonate resin and had been in service for a short duration prior to the failure. Investigation (visual inspection and analysis using micro-FTIR in the ATR mode) revealed the spectrum showed changes in the relative intensities of several bands, as compared to the results representing the base material. A spectral subtraction was performed, and the results produced a good match with diphenyl carbonate, which is a common breakdown product produced during the decomposition of polycarbonate. The conclusion was that the most likely cause of the molecular degradation was improper drying and/or exposure to excessive heat during the injection molding process that in turn caused the material degradation.

Abstract
The self-powered flux detectors used in some nuclear reactors are Pt or V-cored co-axial cables with MgO as an insulator and Inconel 600 as the outer sheath material. The detectors are designed to operate in a He atmosphere; to maximize the conduction of heat (generated from the interaction with gamma radiation) and to prevent corrosion. A number of failures have occurred over the years because of a loss of the He cover gas in the assembly. This has resulted in either acid attack on the Inconel 600 sheath in a wet environment or gaseous corrosion in a dry environment. In the latter case, nitriding and embrittlement occurred at temperatures as low as 300 to 400 deg C (determined from an examination of the oxidation of the Zircaloy-2 carrier rod on which the detectors were mounted). Recent results are described and discussed in terms of the oxidation and nitriding kinetics of Zircaloy-2 and Inconel 600, respectively.

Abstract
A welded natural gas line of 400 mm OD and 9 mm wall thickness made of unalloyed steel with 0.22C had to be removed from service after four months because of a pipe burst. Metallographic examination showed the pipe section located next to the gas entrance was permeated by cracks or blisters almost over its entire perimeter in agreement with the ultrasonic test results. Only the weld seam and a strip on each side of it were crack-free. Based on this investigation, the pipeline was taken out of service and reconstructed. To avoid such failures in the future, two preventative measures may be considered. One is to desulfurize the gas. Based on tests, however, the desulfurization would have to be carried very far to be successful. The second possibility is to dry the gas to such an extent as to prevent condensate, and this corrosion, from forming no matter how low winter temperatures may drop. This measure was ultimately recommended, deemed more effective and cheaper.

Five cylinders out of a group of nine in a drying machine developed leaks after a few months service in a textile mill. Leakage was reported from locations between the hoop and body and from the circumferential welds. The materials in the affected area were 18/8 Ti and 18/10/3/Mo austenitic stainless steels. Examination of the cracks at high magnification revealed them to be of the stress-corrosion type. The welds were of satisfactory quality. Cracking was also visible at these locations, this again being of the stress corrosion type. The method of cylinder construction introduced a crevice between the outer hoop and the cylinder at the inboard edge so that during washing of the rolls, water could penetrate the crevice and subsequent heating would lead to the concentration of chlorides within the crevice. Redesign of the cylinder to eliminate the crevice was recommended.

Abstract
Immediately after installation, leakage was observed at the mounting surface of several rebuilt hydraulic actuators that had been in storage for up to three years. At each joint, there was an aluminum alloy spacer and a vellum gasket. The mounting flanges of the steel actuators had been nickel plated. During assembly of the actuators a lubricant containing molybdenum disulfide had been applied to the gaskets as a sealant. The vellum gasket was found to be electrically conductive, and analysis (visual inspection, 500x unetched micrographs, galvanic action testing, and x-ray diffraction) supported the conclusions that leakage was the result of galvanic corrosion of the aluminum alloy spacers while in storage. The molybdenum disulfide was apparently suspended in a volatile water-containing vehicle that acted as an electrolyte between the aluminum alloy spacer and the nickel-plated steel actuator housing. Initially, the vellum gasket acted as an insulator, but the water-containing lubricant gradually impregnated the vellum gasket, establishing a galvanic couple. Recommendations included discontinuing use of molybdenum disulfide lubricant as a gasket sealer, and assembling the actuators using dry vellum gaskets.

Abstract
After 10 to 20 months of service, the carbon steel hoppers on three trucks used to transport bulk ammonium nitrate prills developed extensive cracking in the upper walls. The prills were discharged from the steel hoppers using air superchargers that generated an unloading pressure of approximately 11 kPa (7 psi). Each hopper truck held from 9,100 to 11,800 kg (10 to 13 tons) of prills when fully loaded and handled approximately 90,700 kg (100 tons) per month. The walls of the hoppers were made of 2.7 mm (0.105 in.) thick flat-rolled carbon steel sheet of structural quality, conforming to ASTM A 245 (obsolete specification replaced by A 570 and A 611). Investigation (visual inspection and 100x micrographs polished and etched with nital) supported the conclusion that failure of the hoppers was the result of intergranular SCC of the sheet-steel walls because of contact with a highly concentrated ammonium nitrate solution. Recommendations included the cost-effective solution of applying a three-coat epoxy-type coating with a total dry thickness of 0.3 mm (0.013 in.) to the interior surfaces of the hoppers.

Abstract
Each of the ten studs on one wheel of a semitrailer used to haul coal broke in half while the trailer was in operation. Both halves of each of three studs were sent to the laboratory to determine the cause of failure. Visual examination of the fracture surfaces of the studs disclosed beach marks, indicative of fatigue cracking, starting at opposite sides of each stud, with final fracture occurring across the stud. Each failure occurred in the first thread of the stud. It was concluded that the wheel studs fractured by reversed-bending fatigue. To minimize the possibility of a recurrence, the wheel nuts were tightened with an air impact wrench to a torque of 610 to 678 J (450 to 500 ft · lb) dry. All wheel studs were checked at normal maintenance periods to ensure uniform and proper loading, and no further failures occurred.
