Abstract
A group of control valves that regulate production in a field of sour gas wellheads performed satisfactorily for three years before pits and cracks were detected during an inspection. One of the valves was examined using chemical and microstructural analysis to determine the cause of failure and provide preventive measures. The valve body was made of A216-WCC cast carbon steel. Its inner surface was covered with cracks stemming from surface pits. Investigators concluded that the failure was caused by a combination of hydrogen-induced corrosion cracking and sulfide stress-corrosion cracking. Based on test data and cost, A217-WC9 cast Cr–Mo steel would be a better alloy for the application.

It reviews the various mechanisms of SCC and addresses electrochemical and stress-sorption theories. The article explains the SCC, which occurs due to welding, metalworking process, and stress concentration, including options for investigation and corrective measures. It describes the sources of stresses in service and the effect of composition and metal structure on the susceptibility of SCC. The article provides information on specific ions and substances, service environments, and preservice environments responsible for SCC. It details the analysis of SCC failures, which include on-site examination, sampling, observation of fracture surface characteristics, macroscopic examination, microscopic examination, chemical analysis, metallographic analysis, and simulated-service tests. It provides case studies for the analysis of SCC service failures and their occurrence in steels, stainless steels, and commercial alloys of aluminum, copper, magnesium, and titanium.

A pressure vessel failed causing an external fire on a nine-story coke gasifier in a refinery power plant. An investigation revealed that the failure began as cracking in the gasifier internals, which led to bulging and stress rupture of the vessel shell, and the escape of hot syngas, setting off the fire. The failure mechanisms include stress relaxation cracking of a large diameter Incoloy 825 tube, stress rupture of a 4.65 in. thick chromium steel shell wall, and the oxidation of chromium steel exposed to hot syngas. The gasifier process and operating conditions that contributed to the high-temperature degradation were also analyzed and are discussed.

Abstract
Copper electrical feedthrough pins used in a bolting application in a refrigeration compressor had functioned without failure for years of production and thousands of units. When some of the pins began to fail, an investigation was conducted to determine the cause. Visual examination revealed that the observed fractures were mixed brittle intergranular with ductile microvoid dimples. An extensive analysis of failed samples combined with a process of elimination indicated that the fractures were due to stress-corrosion cracking caused by an unidentified chemical species within the sealed compressor chamber. A unique combination of applied stress, residual stress, stress riser, and grain size helped isolate the failure mechanism to a single production lot of material.

Nineteen out of 26 bolts in a multistage water pump corroded and cracked after a short time in a severe working environment containing saline water, CO 2 , and H 2 S. The failed bolts and intact nuts were to be made from a special type of stainless steel as per ASTM A 193 B8S and A 194. However, the investigation (which included visual, macroscopic, metallographic, SEM, and chemical analysis) showed that austenitic stainless steel and a nickel-base alloy were used instead. The unspecified materials are more prone to corrosion, particularly galvanic corrosion, which proved to be the primary failure mechanism in the areas of the bolts directly exposed to the working environment. Corrosion damage on surfaces facing away from the work environment was caused primarily by chloride stress-corrosion cracking, aided by loose fitting threads. Thread gaps constitute a crevice where an aggressive chemistry is allowed to develop and attack local surfaces.

This case study describes the failure analysis of a steel nozzle in which cracking was observed after a circumferential welding process. The nozzle assembly was made from low-carbon CrMoV alloy steel that was subsequently single-pass butt welded using gas tungsten arc welding. Although no cracks were found when the welds were visually inspected, X-ray radiography showed small discontinuous surface cracks adjacent to the weld bead in the heat affected zone. Further investigation, including optical microscopy, microhardness testing, and residual stress measurements, revealed that the cracks were caused primarily by the presence of coarse untempered martensite in the heat affected zone due to localized heating. The localized heating was caused by high welding heat input or low welding speed and resulted in high transformation stresses. These transformation stresses, working in combination with thermal stresses and constraint conditions, resulted in intergranular brittle fracture.

Stainless steel pipe (273-mm OD x 8-mm wall thickness) used in the fabrication of large manifolds developed crack-like decohesions during a routine inductive bending procedure. The imperfections, which were found near the outside diameter, were around 3 mm in length oriented in the circumferential direction and penetrated nearly 2 mm into the pipe wall. The pipes were made of titanium-stabilized austenitic stainless steel X6CrNiMoTi17-12-2. Six hypotheses were considered during the investigation, which ultimately concluded that the failure was caused by liquation cracking due to overheating.

Several stainless steel coils cracked during a routine unwinding procedure, prompting an investigation to determine the cause. The analysis included optical and scanning electron microscopy, energy-dispersive x-ray spectrometry, and tensile testing. An examination of the fracture surfaces revealed a brittle intercrystalline mode of fracture with typical manifestations of clear grain facets. Branched and discrete stepwise microcracks were also found along with unusually high levels of residual hydrogen. Mechanical tests revealed a marked loss of tensile ductility in the defective steel with elongations barely approaching 8%, compared to 50% at the time of delivery weeks earlier. Based on the timing interval and the fact that failure occurred at operating stresses well below the yield point of the material, the failure is being attributed to hydrogen-induced damage. Potential sources of hydrogen are considered as are remedial measures for controlling hydrogen content in steels.

Following the crash of a Mirage III-0 aircraft (apparently caused by engine failure), a small crack was detected in a bolt hole in the wing main spar (AU4SG aluminum alloy). Because this area was considered to be critical to aircraft safety and similar cracking was found in other spars in service, the Royal Australian Air Force requested that the crack growth rate during service be determined. The loading history of the aircraft was made available in the form of flight by-flight records of the counts from the vertical accelerometer sensors fitted to the airframe and a series of “overstress” events recorded during the life of the aircraft. The bolt hole was examined by eddy current testing, visual examination, high-powered light microscope, and scanning electron microscope. Simulation tests were also conducted. The use of simulation specimens permitted actual crack growth rate data to be determined for the configuration of interest.

Abstract
The 4340 steel main rotor yoke of a helicopter failed during a hovering exercise. Visual examination of the yoke revealed no evidence of gross external damage. Visual fracture surface examination, macrofractography, scanning electron micrography, and metallography of a section cut from the yoke in the region of the cracking indicated that the failure was caused by fatigue-crack initiation and growth from severe corrosion damage to a pillow-block bolt hole. Corrosion occurred because of failure of the protection scheme. An upgraded corrosion protection scheme for the bolt holes was recommended, along with nondestructive inspection of the region at intervals determined by fractographic analysis of the fatigue crack growth.

Cracks were discovered between interference-fit fasteners (MoS2-coated Ti-6Al-4V) that had been incorporated into a fighter aircraft primary structural frame (D6ac steel) to enhance structural fatigue life. Examination of sections cut from the cracked frame established that the cracks propagated by stress-corrosion cracking. The cause of cracking was twofold: use of interference-fit fasteners exposed to moisture intrusion from a marine environment and poor hole quality. Failure was intensified by dissimilar-metal contact in the presence of weak acidic electrolyte (dissociated MoS2). Control of machining parameters to prevent formation of brittle martensite, use of galvanically compatible fasteners, and use of an alternate lubricant were recommended.

Several type 304L (UNS S30400) stainless steel seamless tubes in a high-pressure synthesis gas cooler condensing ammonia in a fertilizer plant leaked in an unexpectedly short time. Representative samples of the tubes were subjected to chemical analysis, hardness tests, and optical microscopy examination. The tests revealed that the tubes conformed to specification. Routine inspections of a carbon steel wood pulp digester revealed a sharply increasing number of cracks in the overlay metal on the internal surface of the digester after 1 and 2 years of service. The weld overlay was composed of type 309 stainless steel on the top fourth of the digester and of a proprietary high-nickel material on the bottom three-fourths. Examination revealed three distinct modes of deterioration. General corrosion was linked to the use of unspecified overlay metal. Cracking resulted during installation from the use of a material susceptible to hot cracking. Deep corrosion fissures then developed at hot crack sites as a result of crevice corrosion. Abstract
A pair of steam generators operating at a pressurized water reactor site were found to be leaking near a closure weld. The generators were the vertical U-tube type, constructed from ASTM A302 grade B steel. The shell material exhibited high hardness values prior to confirmatory heat treatment, indicating high residual stresses in the area of the weld. All cracks were transgranular and were associated with pits on the inside surfaces of the vessels. It was concluded that the cracking was caused by a low-cycle corrosion fatigue phenomenon, with cracks initiating at areas of localized corrosion and propagating by fatigue. The cause of the pitting/cracking was related to the unit's copper species in solution.

A failure analysis was conducted to determine the cause of recurring failure of flexible bellows in an exhaust hose assembly. The bellows were made of type 321 stainless steel. Visual examination showed that cracks followed a path along the seam weld in the bellows. Most of the cracks followed a multidirectional/circular pattern, occasionally chipping off the convolutions, an indication of high-resonance fatigue-type cracking. Scanning electron fractography showed fatigue striations throughout the fracture surface. The microstructure consisted of relatively large grains and an abnormal degree of titanium-base stringers. Wall thickness was about 0.15 mm (0.006 in.) underside. It was concluded that the high vane pass frequency excited the natural vibration of the bellows to a higher resonance and cracked the bellows after a relatively short service period. The assembly was redesigned, and no further cracking occurred.

Field metallography and replication were performed on a type 316 stainless steel column in diglycol amine vacuum service to determine the cause of visible OD pitting on the column in several areas above the insulation support rings. The examination revealed transgranular stress-corrosion cracking beneath the pitted areas on the OD. The likely cause of the cracking was chloride stress corrosion, with chlorides deriving from the marine atmosphere and concentrating under the insulation around the support rings. A complete insulation evaluation, including repair or replacement, was recommended to prevent chloride buildup. Painting of the steel surface with an epoxy-phenolic or epoxy-coal tar was also suggested.

Abstract
A type 316 stainless steel pipe reducer section failed in service of bleached pulp stock transfer within 2 years in a pulp and paper mill. The reducer section fractured in the heat-affected zone of the flange-to-pipe weld on the flange side. The pipe reducer section consisted of 250 and 200 mm (10 and 8 in.) diam flanges welded to a tapered pipe section. The tapered pipe section was 3.3 mm (0.13 in.) thick type 316 stainless steel sheet, and the flanges were 5 mm (0.2 in.) thick CF8M (type 316) stainless steel castings. Visual and metallographic analysis indicated that the fracture was caused by intergranular corrosion/stress-corrosion cracks that initiated from the external surface of the pipe reducer section. Contributory factors were the sensitized condition of the flange and the concentration of corrosive elements from the bleach stock plant environment on the external surface. In the absence of the sensitized condition of the flange, the service of the pipe reducer section was acceptable. A type 316L stainless steel reducer section was recommended to replace the 316 component because of its superior resistance to sensitization.

An aluminum bronze propeller tap bolt from a twin-screw vessel fractured just below the bolt head. Liquid penetrant testing revealed a large network of cracks that extended radially from sites in and just below the bolthead. Metallographic analysis indicated that the tap bolt failed by stress-corrosion cracking. It was surmised that seawater or some other corrosive substance was present in sufficient quantity to induce intergranular cracking at regions of high stress concentration. It was recommended that all tap bolts be replaced with new bolts made from an alloy with a higher copper content and at least the same yield strength. Steps to exclude seawater and any possible source of ammonia from the bolt shank were also suggested.

Six cracked A36 steel gusset plates that formed part of the roof trusses of a large manufacturing facility were discovered during a routine final inspection of a new building construction. Two different-size plates from different locations in the building were removed and tested. It was determined that the gusset plates failed in the heat-affected zone via an intergranular microcracking mode due to hydrogen-assisted underbead and toe-weld cracking. Proper nondestructive testing techniques for magnetic particle and radiographic inspection of the plate-weld gusset areas were recommended.
