Abstract
Large quantities of aluminum-clad spent nuclear materials have been in interim storage in the fuel storage basins at The Savannah River Site while awaiting processing since 1989. This extended storage as a result of a moratorium on processing resulted in corrosion of the aluminum clad. Examinations of this fuel and other data from a corrosion surveillance program in the water basins have provided basic insight into the corrosion process and have resulted in improvements in the storage facilities and basin operations. Since these improvements were implemented, there has been no new initiation of pitting observed since 1993. This paper describes the corrosion of spent fuel and the metallographic examination of Mark 31A target slugs removed from the K-basin storage pool after 5 years of storage. It discusses the SRS Corrosion Surveillance Program and the improvements made to the storage facilities which have mitigated new corrosion in the basins.

Aluminum-clad spent nuclear fuel is stored in water filled basins at the Savannah River Site awaiting processing or other disposition. After more than 35 years of service underwater, the aluminum storage racks that position the fuel bundles in the basin were replaced. During the removal of the racks from the basin, a failure occurred in one of the racks and the Savannah River Technology Center was asked to investigate. This paper presents the results of the failure analysis and provides a discussion of the effects of corrosion on the structural integrity of the storage racks.

Abstract
Countersunk riveted joints in aluminum sheet are widely employed in the aircraft industry. The preparation of the sheet for the riveting process consists either of countersinking where the sheet is sufficiently thick or of dimpling. Metallographic assessment of dimple defects is described in specimens made of clad aluminum sheet of alloy type AlZnMgCu1.5. Addressed are a dimple with partially missing stamped surface (bell-mouth), a cylindrical prominence because the dimpling force was too great and the stamping cylinder force too low, and a dimple with flashes at the top surfaces of the sheet as a result of play between the stamping cylinder and the anvil head (ringed dimple). Frequently, overlapping of several defects occurs, especially with steel or titanium sheet, with the result that it is difficult to identify the defects.

Abstract
This article addresses the effects of damage to equipment and structures due to explosions (blast), fire, and heat as well as the methodologies that are used by investigating teams to assess the damage and remaining life of the equipment. It discusses the steps involved in preliminary data collection and preparation. Before discussing the identification, evaluation, and use of explosion damage indicators, the article describes some of the more common events that are considered in incident investigations. The range of scenarios that can occur during explosions and the characteristics of each are also covered. In addition, the article primarily discusses level 1 and level 2 of fire and heat damage assessment and provides information on level 3 assessment.

Abstract
A longeron assembly constructed of Alclad 2024, some parts being in the T3 condition, others in the T42 condition, failed at a rivet hole. Plastic deformation at the crack site was found, but no plastic deformation was found in similar failed components. It was concluded that the numerous hairline cracks in the Alclad layer adjacent to the main fracture were fatigue cracks. In another case, bonded samples of 2024-T3 sheet were fatigue tested at various stress levels. Failures could be separated into three groups: those that failed in the adhesive bond, those that failed in the base material, and those that exhibited a dual failure. The last category failed in the adhesive bond and also showed a type of pitting on one face of the base material. In a third case, a 2024-T4 extrusion section was found to exhibit blistering after chemical milling. The presence of interconnecting microcracks between adjacent discontinuities supported a hydrogen blistering diagnosis.

Abstract
Explosive cladding is a viable method for cladding different materials together, but the complicated behavior of materials under ballistic impacts raises the probability of interfacial shear failure. To better understand the relationship between impact energy and interfacial shear, investigators conducted an extensive study on the shear strength of explosively cladded Inconel 625 and plain carbon steel samples. They found that by increasing impact energy, the adhesion strength of the resulting cladding can be improved. Beyond a certain point, however, additional impact energy reduces shear strength significantly, causing the cladding process to fail. The findings reveal the decisive role of plastic strain localization and the associated development of microcracks in cladding failures. An attempt is thus made to determine the optimum cladding parameters for the materials of interest.

Abstract
On 16 July 1999, a Boeing 737-800 on final approach for landing sustained a major lightning strike. Damage to the fuselage structure primarily was in the form of melting or partial melting of widely-separated rivets and adjacent Alclad 2024-T3 fuselage skin. The damage was confined to a 0.25-in. (6.4-mm) radii around the affected rivets. The repair process involved removal of the locally-affected material and addition of a skin doubler to restore the aircraft structure to the originally designed condition. Abstract
A series of resistance spot welds joining Z-shape and C-shape members of an aircraft drop-tank structure failed during ejection testing. The members were fabricated of alclad aluminum alloy 2024-T62. The back surface of the C-shape members showed severe electrode-indentation marks off to one side of the spot weld, suggesting improper electrode contact. Visual examination of the weld fractures showed that the weld nuggets varied considerably in size, some being very small and three exhibiting an HAZ but no weld. Of 28 welds, only nine had acceptable nugget diameters and fusion-zone widths. The weld deficiencies were traced to problems in forming and fit-up of the C-shape members and to difficulties in alignment and positioning of the weld tooling. The failure of the resistance spot welds was attributed to poor weld quality caused by unfavorable fit-up and lack of proper weld-tool positioning. The problem could be solved by better forming procedures to provide an accurate fit-up that would not interfere with electrode alignment.

Abstract
Nickel anodes failed in several electrolysis cells in a heavy-water upgrading plant. Dismantling of a cell revealed gouging and the presence of loosely attached black porous masses on the anode. The carbon steel top, plate was severely corroded. An appreciable quantity of black powder was also present on the bottom or the cell. SEM/EDX studies of the outer and inner surfaces of the gouged anode showed the presence of iron globules at the interface between the gouged and the unattacked anode. The chemical composition of the black powder was determined to be primarily iron. Cell malfunction was attributed to the accelerated dissolution of the carbon steel anode top, dislodgment of grains from the material, and subsequent closing of the small annular space between the anode and the cathode by debris from the anode top. Cladding of the carbon steel top with a corrosion-resistant material, such as nickel, nickel-base alloy, or stainless steel, was recommended.

Abstract
Postflight inspection of a gas-turbine aircraft engine that had experienced compressor stall revealed that the engine air-intake bullet assembly had dislodged and was seated against the engine-inlet guide vanes at the 3 o'clock position. The bullet assembly consisted of an outer aerodynamic shell and an inner stiffener shell, both of 1.3 mm (0.050 in.) thick aluminum alloy 6061-T6, and four attachment clips of 1 mm (0.040 in.) thick alclad aluminum alloy 2024-T42. Each clip was joined to the outer shell by 12 spot welds and was also joined to the stiffener. Analysis (visual inspection, dye-penetrant inspection, and 10x/150x micrographs of sections etched with Keller's reagent) supports the conclusion that the outer shell of the bullet assembly separated from the stiffener because the four attachment clips fractured through the shell-to-clip spot welds. Fracture occurred by fatigue that initiated at the notch created by the intersection of the faying surfaces of the clip and shell with the spot weld nuggets. The 6061 aluminum alloy shell and stiffener were in the annealed (O) temper rather than T6, as specified. Recommendations included heat treating the shell and stiffener to the T6 temper after forming.

Abstract
Stator vanes (cast from a Cu-Mn-Al alloy) in a hydraulic dynamometer used in a steam-turbine test facility were severely eroded. The dynamometer was designed to absorb up to 51 MW (69,000 hp) at 3670 rpm, and constituted an extrapolation of previous design practices and experience. Its stator was subject to severe erosion after relatively short operating times and initially required replacement after each test program. Although up to 60 cu cm (3.7 cu in.) of material was being lost from each vane, it only reduced the power-absorption capacity by a small amount. Analysis supported the conclusion that the damage was due to liquid erosion, but it could not be firmly established whether it was caused by cavitation or by liquid impact. Recommendations included making a material substitution (to Mo-13Cr-4Ni stainless steel) and doing a redesign to reduce susceptibility to erosion as well as erosion-producing conditions.

Abstract
Aluminum alloy BS.1476-HE.15 by virtue of its high strength and low density finds application in the form of bars or sections for cranes, bridges, and other such structures where a reduction in dead weight load and inertia stresses is advantageous. Bars and sections in H.15 alloy are mostly produced by extrusion. Some material processed this way has been prone to exfoliation corrosion. Extended aging for 24 h at a temperature of 185 deg C (365 deg F) virtually suppresses the tendency for exfoliation corrosion to develop. Also, the use of a sprayed coating, either of aluminum or Al-1Zn alloy, was effective in halting and preventing this form of attack. While alarming, the appearance of exfoliation corrosion provides a valuable warning to the engineer or inspector before a severe weakening of the particular sections has occurred.

Abstract
Heat exchangers are devices used to transfer thermal energy between two or more fluids, between a solid surface and a fluid, or between a solid particulate and a fluid at different temperatures. This article first addresses the causes of failures in heat exchangers. It then provides a description of heat-transfer surface area, discussing the design of the tubular heat exchanger. Next, the article discusses the processes involved in the examination of failed parts. Finally, it describes the most important types of corrosion, including uniform, galvanic, pitting, stress, and erosion corrosion.

This article addresses the forms of corrosion that contribute directly to the failure of metal parts or that render them susceptible to failure by some other mechanism. It describes the mechanisms of corrosive attack for specific forms of corrosion such as galvanic corrosion, uniform corrosion, pitting and crevice corrosion, intergranular corrosion, and velocity-affected corrosion. The article contains a table that lists combinations of alloys and environments subjected to selective leaching and the elements removed by leaching.

Abstract
Corrosion is the electrochemical reaction of a material and its environment. This article addresses those forms of corrosion that contribute directly to the failure of metal parts or that render them susceptible to failure by some other mechanism. Various forms of corrosion covered are galvanic corrosion, uniform corrosion, pitting, crevice corrosion, intergranular corrosion, selective leaching, and velocity-affected corrosion. In particular, mechanisms of corrosive attack for specific forms of corrosion, as well as evaluation and factors contributing to these forms, are described. These reviews of corrosion forms and mechanisms are intended to assist the reader in developing an understanding of the underlying principles of corrosion; acquiring such an understanding is the first step in recognizing and analyzing corrosion-related failures and in formulating preventive measures.

Abstract

It provides information on pressure vessels made of composite materials and their welding practices. The article explains the failure of pressure vessels with emphasis on stress-corrosion cracking, hydrogen embrittlement, brittle and ductile fractures, creep and stress rupture, and fatigue with examples.
