Abstract
A combination of adverse factors was present in the disruption of a turbo-alternator gearbox. The major cause was the imposition of a gross overload far in excess of that for which the gearbox was designed. The contributory factors were a rim material (EN9 steel) that was inherently notch-sensitive and liable to rupture in a brittle manner. Discontinuities were present in the rims formed by the drain holes drilled in their abutting faces, and possibly enhanced by the stress-raising effect of microcracks in the smeared metal at their surfaces It is probable that the load reached a value in excess of the yield point within the delay time of the material so when the fracture was initiated, it was preceded by several microcracks giving rise to the propagation of a brittle fracture.

Abstract
Copper shortening has been found to occur in the rotor windings of turbo alternators and takes the form of a progressive reduction in the length of the coils leading to distortion of the end windings. The trouble results from the high loading which develops between successive layers of the strip conductor due to centrifugal force. This leads to a high frictional binding force between turns and prevents axial expansion under normal heating in service. Rotor trouble which proved to be due to copper shortening was found in a set rated at 27.5 MW. It was manufactured in 1934 at which time silver-bearing copper was not available. The use of hard-drawn silver-bearing copper for a rewind, in conjunction with special attention to blocking up the end windings, is confidently expected to effect a complete cure.

A shotgun barrel fabricated from 1138 steel deformed when test firing alternative nontoxic ammunition. The test shells contained soft iron shot, which at 72 HB, is much harder than traditional lead shot (typically 30 to 40 HB). An investigation based on ID and OD profiling supported the conclusion that the iron shot increased stresses in the choke zone of the barrel, causing it to deform. Variations in the amount of bulging were attributed to a lack of uniformity in wall thickness. Recommendations included making the barrel from steel with a higher yield strength, making the barrel walls thicker and more uniform, and/or developing an alternative nontoxic metal shot with a hardness in the range of 30 to 40 HB.

Abstract
The presence of secondary, branching intergranular stress-corrosion cracking in a type 440C stainless bearing caused the analyst to overlook the real culprit, which was a mechanically-initiated, primary transgranular crack that propagated through the steel's hard chromium carbide. Failure was actually caused by overload. Had the original conclusion been accepted, a relatively exotic alloy would have been specified. In another case, brass heat exchanger tube failure was automatically attributed to attack by an acidic cleaner, and a decision was made to stop using the solution. A more thorough analysis showed failure was caused by tube vibration. In a third case, a type 304 stainless steel bellows in a test loop was thought to have failed because of chloride stress corrosion. The report concluded with a recommendation that carbon steel be used as an alternative bellows material. Caustic, not chloride, stress corrosion was the culprit. Had material substitutions been made on the original premise of countering chloride stress corrosion, most of the loop's highly stressed components would have eventually failed.

Abstract
The origins of the casting are unknown. It is included here as a classic case of intergranular corrosion. The part (apparently a pump outlet) was named the “rubber casting” because of the severity of the intergranular attack. Every grain boundary has been attacked to the extent that the casting could be twisted and stretched as through made of rubber. The chemistry of the casting was acceptable for CN-7M. The reason the part failed is a continuous film of carbide with a continuous crack running parallel to the carbides. This sensitized structure produces an area depleted in protective chromium, making it susceptible to corrosion. Two solutions to this problem are available. The simplest is to ensure correct heat treatment to dissolve grain-boundary carbide film and return the protective chromium to the depleted zone. Alternatively, a low-carbon (0.03% maximum C, for example, CF-3) grade can be specified. Procedures are given in a reference for screening castings that may be susceptible to intergranular corrosion due to processing errors.

Abstract
Following the discovery numerous cracks at many of the welded seams of a mild steel CO2 absorber vessel, a sample for examination was removed from the worst affected area where repairs had been effected. A 12 in. long circumferential crack was visible. Specimens were taken to cover the several locations of cracking which, in all cases, were found to be similar and of the intergranular type filled with oxide or corrosion product. The association of the cracks with the weld seams indicated that contraction stresses from welding were primarily responsible. Failure of the absorber vessel was found to be due to stress corrosion. Although the active agent present was not positively identified, the aqueous solution of monoethanolamine was thought to be the most probable. The origin of the stresses was not elucidated but the association of the cracks with the welded seams indicated inherent residual stresses as being primarily responsible. Tests carried out tend to suggest that stress relief was not carried out. For the replacement plant, consideration of stress relieving or the use of an alternative material was advised.

Abstract
A water tube boiler with two headers and 15.5 atm working pressure became leaky in the lower part due to the formation of cracks in the rivet-hole edges. The boiler plate of 20 mm thickness was a rimming steel with 0.05% C, traces of Si, 0.38% Mn, 0.027% P, 0.035% S, and 0.08% Cu. The mean value of the yield point was 24 (24) kg/sq mm, the tensile strength 39 (38) kg/sq mm, the elongation at fracture, d10, 26 (24)%, the necking at fracture 71 (66)% and notch impact value 11.5 (9.4) kgm/sq cm (the values in brackets are for the transverse direction). The specimen from inside surface of the boiler was polished and etched with Fry-solution, which revealed parallel striations formed due to the cold bending of the plate. The zones of slip were concentrated around the rivet holes. The cracks were formed here. The structure examination proved that the cracks had taken an exactly intercrystalline path, which is characteristic for caustic corrosion cracks. It was recommended that the internal stresses be removed through annealing or alternatively lye-resistant steel should be used.

Abstract
A large pressure vessel that had been in service as a hydrogen sulfide (H2S) absorber developed cracks and began leaking at a nozzle. The vessel contained a 20% aqueous solution of potassium hydroxide (KOH), potassium carbonate (K2CO3), and arsenic. The vessel wall was manufactured of ASTM A516, grade 70, low-carbon steel plate. A steel angle had been formed into a ring was continuously welded to the inside wall of the vessel. The groove formed by the junction of the lower tray-support weld and the top part of the weld around the nozzle was found to have a crack. Pits and scale near the crack origin were revealed by microscopic examination and cracking was found to be transgranular. Periods of corrosion alternated with sudden instances of cleavage, under a tensile load, along preferred slip planes were interpreted during examination with a microscope. It was concluded that the combination of the residual plus operating stresses and the amount of KOH present would have caused stress corrosion as a result of caustic embrittlement. It was recommended that the tray support should be installed higher on the vessel wall to prevent coincidence of the lower tray-support weld with the nozzle weld.

Abstract
Two examples concerning fabricated mild steel rotor spiders which failed due to lack of torsional rigidity, probably supplemented by the presence of high internal stress, are described. The machine concerned in the first case was a 3,000 hp three-phase slip-ring motor. In the second case the machine was a 200 kW alternator, direct-driven by a diesel engine running at 750 rpm. Both the foregoing failures reveal the same basic weakness, i.e., insufficient rigidity when subjected to variations or reversals of torque. In the first case, the bars welded to the arms were inadequately supported in a lateral direction, so that excessive stresses of a fluctuating nature were set up in the welds as a result of the frequent load changes that arose in service. This weakness was eliminated when designing the replacement spider. In the second example, failure also arose as a result of deficient torsional rigidity with the consequent development of excessive stresses in the welds at the junctions of the bars with the sleeve, the torque being of a fluctuating character due to the impulses imparted by the engine.

When bulging occurred in mortar tubes made of British I steel during elevated-temperature test firing, a test program was formulated to evaluate the high-temperature properties (at 540 to 650 deg C, or 1000 to 1200 deg F) of the British I steel and of several alternative alloys including a maraging steel (18% Ni, grade 250), a vanadium-modified 4337 gun steel (4337V), H19 tool steel, and high-temperature alloys Rene 41, Inconel 718, and Udimet 630. All the alloys evaluated had been used in mortar tubes previously or were known to meet the estimated minimum yield strength. The alloys fall in this order of decreasing strengths: Udimet 630, Inconel 718, Rene 41, H19 tool steel, British I steel, 4337V gun steel, and maraging steel. When cycled between room temperature and 540 to 650 deg C (1000 to 1200 deg F), only Udimet 630, Inconel 718, and Rene 41 retained yield strengths higher than the minimum. Also, these three alloys maintained high strengths over the tested range, whereas the others decreased in yield strength as cycling progressed. Analysis showed Inconel 718 was considered best suited for 81-mm mortar tubes, and widespread industrial use ensured its availability.

Leakage was detected in a malleable iron elbow (ASTM A 47, grade 35018) after only three months in service. Life expectancy for the elbow was 12 to 24 months. The piping alternately supplied steam and cooling water to a tire-curing press. The supply line and elbow were subjected to 14 heating and cooling cycles per hour for at least 16 h/day, or a minimum of 224 cycles/day. Steam and water pressure were 1035 kPa (150 psi) and 895 kPa (130 psi) respectively, and water-flow rate was estimated to be 1325 L/min (350 gal/min) based on pump capacity. Water-inlet temperature was 10 to 15 deg C (50 to 60 deg F) and outlet temperature was 50 to 60 deg C (120 to 140 deg F). The pH of the water was 6.9. Investigation (visual inspection, chemical analysis, and 67x nital etched micrographs) supported the conclusion that the elbows had been given the usual annealing and normalizing treatment for ferritizing malleable iron. This resulted in lower resistance to erosion and corrosion than pearlitic malleable iron. Recommendations included replacing the elbows with heat-treated fittings with a pearlitic malleable microstructure.

Abstract
Practical examples of stress-corrosion cracking (SCC) and methods for its prevention were presented. Cracks in chloride-sensitive austenitic steels were very branched and transcrystalline. Etched cross sections of molybdenum-free samples showed chloride-induced cracks running out of the pitted areas. Alternatively polishing and etching micro-sections for viewing at high magnification made crack detail more visible. Optical and scanning electron micrographs showed cracking in austenitic cast steel and cast iron due to both internal tensile and critical residual stresses; the latter causes flake-like spalling. Measures to prevent SCC include stress reduction, use of austenitic steels or nickel alloys not susceptible to grain boundary attack, use of ferritic chromium steels, surface slag removal, control of temperature and chloride concentration, and cathodic protection.

Abstract
A fused-salt electrolytic-cell pot containing a molten eutectic mixture of sodium, potassium, and lithium chlorides and operating at melt temperatures from 500 to 650 deg C (930 to 1200 deg F) exhibited excessive corrosion after two months of service. The pot was a welded cylinder with 3-mm thick type 304 stainless steel walls and was about 305 mm (12 in.) in height and diam. Analysis (visual inspection and 500x micrographs etched with CuCl2) supported the conclusions that the pot failed by intergranular corrosion because an unstabilized austenitic stainless steel containing more than 0.03% carbon had been sensitized and placed in contact in service with a corrosive medium at temperatures in the sensitizing range. Recommendations included changing material for the pot from type 304 stainless steel to Hastelloy N (70Ni-17Mo-7Cr-5Fe). Maximum corrosion resistance and ductility are developed in Hastelloy N when the alloy is solution heat treated at 1120 deg C (2050 deg F) and is either quenched in water or rapidly cooled in air. An alternative, but less suitable, material for the pot was type 347 (stabilized grade) stainless steel. After welding, the 347 should be stress relieved at 900 deg C (1650 deg F) for 2 h and rapidly cooled to minimize residual stresses.
