Abstract
The failure of an ATAR engine accessory angle drive gear assembly caused an engine flame-out in a Mirage III aircraft of the Royal Australian Air Force (RAAF) during a landing. Stripping of the engine revealed that the bevel gear locating splines (16 NCD 13) had failed. Visual and low-power microscope examination of the spline of the shaft showed evidence of fretting wear debris; similar wear was observed on the splines of the mating bevel gear. It was concluded that the splines had failed by severe fretting wear. Fretting damage was also observed on the shaft face adjacent to the splines and on the bevel gear abutment shoulder. Additional tests included a metrological inspection of the shaft, bevel gear and support ring; metallographic examination of a section from the shaft; chemical analysis of the shaft material (16 NCD 13); and hardness testing of a sample of the yoke material. The wear had been caused by incorrect machining of the shaft splines, which prevented the bevel gear nut from locating correctly against the gear.

Abstract
An outer fan-duct assembly of titanium alloy Ti-5Al-2.5Sn (AMS 4910) for a gas-turbine fan section cracked 75 mm (3 in.) circumferentially through a repair weld in an arc weld in the front flange-duct segment. Examination of the crack with a binocular microscope revealed no evidence of fatigue. A blue etch-anodize inspection showed the presence of an alpha case along the edges of the repair weld. The alpha case, a brittle oxide-enriched layer, forms when welds are inadequately shielded from the atmosphere during deposition. The brittleness of this layer caused transgranular cracks to form and propagate in tension under the thermal stresses created by the repair-weld heat input. The crack resulted from contamination and embrittlement of a repair weld that had received inadequate gas shielding. Thermal stresses cracked the oxide-rich layer that formed. The gas-shielding accessories of the welding torch were overhauled to ensure that leak-in or entrainment of air was eliminated. Also, the purity of the shielding-gas supplies was rechecked to make certain that these had not become contaminated.

Abstract
An oil scavenge pump was found to have failed when a protective shear neck fractured during the start of a jet engine. Visual inspection revealed that the driven gear in one of the bearing compartments was frozen as was the corresponding drive gear. Spacer wear and thermal discoloration (particularly on the driven gear) were also observed. The gears were made from 32Cr-Mo-V13 steel, hardened and nitrided to 750 to 950 HV. Micrographic inspection of the gear teeth revealed microstructural changes that, in context, appear to be the result of friction heating. The spacers consist of Cu alloy (AMS4845) bushings force fit into AA2024-T3 Al alloy spacing elements. It was found that uncontrolled fit interference between the two components had led to Cu alloy overstress. Thermal cycling under operating conditions yielded the material. The dilation was directed inward to the shaft, however, because the bushing had only a few microns of clearance. The effect caused the oil to squeeze out, resulting in metal-to-metal contact, and ultimately failure.

Abstract
A routine examination on a seat ejection system found that the catapult attachment swivel fabricated from 7075-T651 aluminum alloy plate contained cracks on opposite sides of the part. This swivel, or bath tub, does not experience extreme loads prior to activation of the catapult system. Some loads could be absorbed however, when the aircraft is subjected to G loads. Visual examination of the part revealed that cracks through the wall thickness initiated on the inner walls of the fixture. Scanning electron microscopy (SEM) and electron optical examination revealed that the cracking pattern initiated and progressed by an intergranular failure mechanism. It was concluded that failure of the catapult attachment swivel fixture occurred by SCC. It was recommended that the 7075 aluminum ejection seat fixture be supplied in the T-73 temper to minimize susceptibility to SCC.

Abstract
The failure of HP turbine blades in a low bypass turbofan engine was analyzed to determine the root cause. Forensic and metallurgical investigations were conducted on all failed blades as well as failed downstream components. It was found that one of the blades fractured in the dovetail region, causing extensive damage throughout the turbine. Remedial measures were suggested to prevent such failures in the future.

Abstract
The scanning electron microscopy (SEM) is one of the most versatile instruments for investigating the microstructure of metallic materials. This article highlights the development of SEM technology and describes the operation of basic systems in an SEM, including the electron optical column, signal detection and display equipment, and vacuum system. It discusses the preparation of samples for observation using an SEM and describes the application of SEM in fractography. If the surface remains unaffected and undamaged by events subsequent to the actual failure, it is often a simple matter to determine the failure mode by the use of an SEM. In cases where the surface is altered after the initial failure, the case may not be so straightforward. The article presents typical examples that illustrate these points. Image dependence on the microscope type and operating parameters is also discussed.

The scanning electron microscope (SEM) is one of the most versatile instruments for investigating the microscopic features of most solid materials. The SEM provides the user with an unparalleled ability to observe and quantify the surface of a sample. This article discusses the development of SEM technology and operating principles of basic systems of SEM. The basic systems covered include the electron optical column, signal detection and display equipment, and the vacuum system. The processes involved in the preparation of samples for observation using an SEM are described, and the application of SEM in fractography is discussed. The article covers the failure mechanisms of ductile failure, brittle failure, mixed-mode failure, and fatigue failure. Lastly, image dependence on microscope type and operating parameters is also discussed.

Abstract
This article provides the framework for the investigation of bridge failures. It explains the types of bridge loading and presents the regulatory provisions for bridges. Some bridge failures in the U.S. that resulted in significant changes in bridge manufacturing, design, regulation, and/or maintenance are also discussed. In addition, the article provides information on traffic damage and fatigue cracking that result in bridge failures. The need for steels with better fracture toughness in bridge design is also discussed.

The first part of this article focuses on two major forms of machining-related failures, namely machining workpiece (in-process) failures and machined part (in-service) failures. Discussion centers on machining conditions and metallurgical factors contributing to (in-process) workpiece failures, and undesired surface layers and metallurgical factors contributing to (in-service) machined part failures. The second part of the article discusses the effects of microstructure on machining failures and their preventive measures.

Abstract
This article reviews analytical techniques that are most often used in plastic component failure analysis. The description of the techniques is intended to familiarize the reader with the general principles and benefits of the methodologies, namely Fourier transform infrared spectroscopy, energy-dispersive x-ray spectroscopy, differential scanning calorimetry, thermogravimetric analysis, and dynamic mechanical analysis. The article describes the methods for molecular weight assessment and mechanical testing to evaluate plastics and polymers. The descriptions of the analytical techniques are supplemented by a series of case studies to illustrate the significance of each method. The case studies also include pertinent visual examination results and the corresponding images that aided in the characterization of the failures.

Abstract
This article is dedicated to the fields of mechanical engineering and machine design. It also intends to give a nonexhaustive view of the preventive side of the failure analysis of rolling-element bearings (REBs) and of some of the developments in terms of materials and surface engineering. The article presents the nomenclature, numbering systems, and worldwide market of REBs as well as provides description of REBs as high-tech machine components. It discusses heat treatments, performance, and properties of bearing materials. The processes involved in the examination of failed bearings are also explained. Finally, the article discusses in detail the characteristics and prevention of the various types of failures of REBs: wear, fretting, corrosion, plastic flow, rolling-contact fatigue, and damage. High temperature corrosion may occur in numerous environments and is affected by factors such as temperature, alloy or protective coating composition, time, and gas composition. This article explains a number of potential degradation processes, namely, oxidation, carburization and metal dusting, sulfidation, hot corrosion, chloridation, hydrogen interactions, molten metals, molten salts, and aging reactions including sensitization, stress-corrosion cracking, and corrosion fatigue. Abstract
Rolling-element bearings use rolling elements interposed between two raceways, and relative motion is permitted by the rotation of these elements. This article presents an overview of bearing materials, bearing-load ratings, and an examination of failed bearings. Rolling-element bearings are designed on the principle of rolling contact rather than sliding contact; frictional effects, although low, are not negligible, and lubrication is essential. The article lists the typical characteristics and causes of several types of failures. It describes failure by wear, failure by fretting, failure by corrosion, failure by plastic flow, failure by rolling-contact fatigue, and failure by damage. The article discusses the effects of fabrication practices, heat treatment and hardness of bearing components, and lubrication of rolling-element bearings with a few examples.

High-temperature corrosion can occur in numerous environments and is affected by various parameters such as temperature, alloy and protective coating compositions, stress, time, and gas composition. This article discusses the primary mechanisms of high-temperature corrosion, namely oxidation, carburization, metal dusting, nitridation, carbonitridation, sulfidation, and chloridation. Several other potential degradation processes, namely hot corrosion, hydrogen interactions, molten salts, aging, molten sand, erosion-corrosion, and environmental cracking, are discussed under boiler tube failures, molten salts for energy storage, and degradation and failures in gas turbines. The article describes the effects of environment on aero gas turbine engines and provides an overview of aging, diffusion, and interdiffusion phenomena. It also discusses the processes involved in high-temperature coatings that improve performance of superalloy.
