Abstract
After about two years in service, a 303 stainless steel valve in contact with a carbonated soft drink in a vending machine occasionally dispensed a discolored drink with a sulfide odor. According to the laboratory at the bottling plant, the soft drink in question was strongly acidic, containing citric and phosphoric acids and having a pH of 2.4 to 2.5. Investigation (visual inspection, chemical analysis, immersion testing in the soft drink, and 100x unetched micrographs) supported the conclusion that the failure was caused by the size and distribution of sulfide stringers in the alloy used in the valve. Manganese sulfide stringers in the valve were exposed at end-grain surfaces in contact with the beverage. The stringers, which were anodic to the surrounding metal, were subject to corrosion, producing a hydrogen sulfide concentration in the immediately adjacent liquid. Recommendations included changing the valve material to type 304 stainless steel.

Abstract
Cold-drawn type 303 stainless steel wire sections, 6.4 mm (0.25 in.) in diameter, failed during a forming operation. All of the wires failed at a gradual 90 deg bend. Investigation (visual inspection and 5.3x/71x/1187x SEM views) supported the conclusion that the wires cracked due to ductile overload. The forming stresses were sufficient to initiate surface ruptures, suggestive of having exceeded the forming limit. Recommendations included examining the forming process, including lubrication and workpiece fixturing.

Abstract
An AISI type 303(Se) stainless steel eye terminal that was roll swaged on the end of a 9.5 mm diam wire rope cracked extensively after one year of service. A hairline crack that had initiated at the inner surface of the fitting was revealed by metallographic examination of a sectioned terminal specimen. It was indicated by the holes in the region adjoining the crack and rough texture of the crack surface that a corrosive medium (presumably seawater) had entered the crack from the inner surface of the fitting and coupled with the hairline crack to develop crevice corrosion. The crack propagated toward the outer surface due to high residual stresses in the swaged metal and was followed closely by corrosion. Stress corrosion as result of a combination of residual stresses plus load stress and corrosion was found to cause the failure. Rotary swaging or swaging in a punch press was recommended instead of roll swaging as they made deformation more symmetrical.

Abstract
A sand-cast LM6M aluminum alloy sprocket drive wheel in an all-terrain vehicle failed. Extensive cracking had occurred around each of the six bolt holes in the wheel. Evidence of considerable deformation in this area was also noted. Examination indicated that the part failed because of gross overload. Use of an alloy with a much higher yield strength and improvement in design were recommended.

Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. Abstract
Copper electrical feedthrough pins used in a bolting application in a refrigeration compressor had functioned without failure for years of production and thousands of units. When some of the pins began to fail, an investigation was conducted to determine the cause. Visual examination revealed that the observed fractures were mixed brittle intergranular with ductile microvoid dimples. An extensive analysis of failed samples combined with a process of elimination indicated that the fractures were due to stress-corrosion cracking caused by an unidentified chemical species within the sealed compressor chamber. A unique combination of applied stress, residual stress, stress riser, and grain size helped isolate the failure mechanism to a single production lot of material.

Abstract
This paper describes the analysis of the failure of a Zr-2.5Nb pressure tube in a CANDU reactor. The failure sequence was established as: (1) the existence of an undetected manufacturing flaw in the form of a lamination, (2) in-service development of the flaw by oxidation of the lamination, (3) delayed hydride cracking, which extended the flaw through the wall of the tube, resulting in leakage, and (4) rupture of the tube by cold pressurization while the reactor was shut down. The comprehensive failure analysis led to a remedial action plan that permitted the reactor to be returned to full-power operation and ensured a low probability of a similar occurrence for all CANDU reactors.

Abstract
Over the last 15 years there has been an increasing incidence of failure in rockbolts used in underground mines in Australia. Failures have also been observed in the United Kingdom where Australian Technology rockbolting is also used. Most of the failures in the United Kingdom were found to be initiated by corrosion pits, but in Australia, the fractures were considered likely to be due to stress-corrosion cracking (SCC). This paper reports a metallurgical study of 44 failed rockbolts from four different underground mines in Australia. The study confirmed that failure was generally due to SCC and showed that this was usually initiated by bending of the bolts that occurred due to lateral movement of the rock strata. It also showed that many of the failed bolts had very low toughness with Charpy impact values of 4 to 7 Joules.

Abstract
Corrosion is the electrochemical reaction of a material and its environment. This article addresses those forms of corrosion that contribute directly to the failure of metal parts or that render them susceptible to failure by some other mechanism. Various forms of corrosion covered are galvanic corrosion, uniform corrosion, pitting, crevice corrosion, intergranular corrosion, selective leaching, and velocity-affected corrosion. In particular, mechanisms of corrosive attack for specific forms of corrosion, as well as evaluation and factors contributing to these forms, are described. These reviews of corrosion forms and mechanisms are intended to assist the reader in developing an understanding of the underlying principles of corrosion; acquiring such an understanding is the first step in recognizing and analyzing corrosion-related failures and in formulating preventive measures.

Abstract
A storage tank had been in service at a petrochemical plant for 13 years when inspectors discovered cracks adjacent to weld joints and in the base plate near the foundation. The tank was made from AISI 304 stainless steel and held styrene monomer, a derivative of benzene. The cracks were subsequently welded over with 308 stainless steel filler wire and the base plate was replaced with new material. Soon after, the tank began leaking along the weld bead, triggering a full-scale investigation; spectroscopy, optical and scanning electron microscopy, fractography, SEM-EDS analysis, and microhardness, tensile, and impact testing. The results revealed transgranular cracks in the HAZ and base plate, likely initiated by stresses developed during welding and the presence of chloride from seawater used in the plant. It was also found that the repair weld was improperly done, nor did it include a postweld heat treatment to remove weld sensitization and minimize residual stresses.

Abstract
Thermomechanical fatigue (TMF) refers to the process of fatigue damage under simultaneous changes in temperature and mechanical strain. This article reviews the process of TMF with a practical example of life assessment. It describes TMF damages caused due to two possible types of loading: in-phase and out-of-phase cycling. The article illustrates the ways in which damage can interact at high and low temperatures and the development of microstructurally based models in parametric form. It presents a case study of the prediction of residual life in a turbine casing of a ship through stress analysis and fracture mechanics analyses of the casing.

Abstract
This article considers two mechanisms of cavitation failure: those for ductile materials and those for brittle materials. It examines the different stages of cavitation erosion. The article explains various cavitation failures including cavitation in bearings, centrifugal pumps, and gearboxes. It provides information on the cavitation resistance of materials and other prevention parameters. The article describes two American Society for Testing and Materials (ASTM) standards for the evaluation of erosion and cavitation, namely, ASTM Standard G 32 and ASTM Standard G 73. It concludes with a discussion on correlations between laboratory results and service.
