Abstract
A steel lifting eye, manufactured from grade 1144 steel, failed during service. The eye ring fractured in two places, adjacent to the threaded shank and diametrically opposite to this region. Woody overload features, typical for resulfurized steels were revealed by SEM. The directionality of the features was found to be suggestive of shear overload. It was observed that fracture preferentially followed the nonmetallic inclusions. The fracture was revealed to be parallel to the direction of the manganese sulfide stringer inclusions. The presence of significant banding of the ferrite and pearlite microstructure was revealed by etching. It was also observed that the fracture is primarily along the inclusions and through bands of ferrite. It was concluded that the lifting eye failed as a result of overload. Fracture occurred parallel to the rolling direction, through manganese-sulfide stringers and ferrite bands in the base metal matrix. The material used for this application was very anisotropic, exhibiting substantially poorer long and short transverse mechanical properties than longitudinal properties.

Abstract
A failed tapered-ring sprocket locking device consisted of an assembly of four tapered rings that are retained by a series of cap screws. The middle wedge-shaped rings were pulled closer as the screws were tightened forcing the split inner ring to clamp tightly onto the shaft. One of the wedge-shaped middle rings fractured prior to having been fully torqued, preventing the sprocket from being locked to the shaft. “Woody” fracture features, as a result of decohesion between a high volume fractions of manganese sulfide stringers and the matrix, was revealed during examination. The material was revealed by chemical analysis to be resulfurized grade of carbon steel (SAE type 1144, UNS G11440) which has enhanced longitudinal tensile properties but low transverse properties. It was observed that when the fastening screws were torqued, a significant hoop stress was placed on the middle rings and it caused the failure at the large inclusion present at the minimum section thickness zone of the middle ring. It was concluded that since the material contained a high volume fraction of these inclusions, the material choice was not appropriate for this application. A nonresulfurized grade of low-alloy steel was suggested as recommendation.

Abstract
A number of failures involving carbon and alloy steels were analyzed to assess the effects of inclusions and their influence on mechanical properties. Inclusions, including brittle oxides and more ductile manganese sulfides (MnS), affect fatigue endurance limit, fatigue crack propagation rates, fracture toughness, notch toughness, and transverse tensile properties, and do so in an anisotropic manner with respect to rolling direction. Significant property anisotropy has been documented in the failures investigated, providing evidence that designers failed to account for it. Typical fracture morphologies observed in such cases and metallographic appearances of MnS-containing materials are illustrated.

Abstract
A drive-line assembly failed during vehicle testing. The vehicle had traveled 9022 km (5606 mi) before the failure occurred. Both the intact and fractured parts of the assembly were analyzed to determine the cause and sequence of failure. Visual examination of the assembly showed three of four bearing caps, two cap screws, and one universal-joint spider had fractured. Examination of the three fractured bearing caps and the spider showed no evidence of fatigue but showed that fracture occurred in a brittle manner. The bearing cap that was not destroyed still contained portions of the two fractured cap screws. It was found that the two cap screws failed in fatigue under service stresses. The three bearing caps and the universal-joint spider broke in a brittle manner. The properties of the material in the cap screws did not fulfill the specifications. The modified 1035 steel was of insufficient alloy content. Also, the tensile strength and endurance limit were lower than specified and were inadequate for the application. The material for the cap screw was changed from modified 1035 steel to 5140 steel.

This article briefly reviews the general aspects of materials selection as a concern in proactive failure prevention during design and as a possible root cause of failed parts. It discusses the overall concept of design and describes the role of the materials engineer in the design and materials selection process. The article highlights the significance of materials selection in both the prevention and analysis of failures.

Abstract
Materials selection is closely related to the objectives of failure analysis and prevention. This article briefly reviews the general aspects of materials selection as a concern in both proactive failure prevention during design and as a possible root cause of failed parts. Coverage is more conceptual, with general discussions on the following topics: design and failure prevention, materials selection in design, materials selection for failure prevention, and materials selection and failure analysis. Because materials selection is just one part of the design process, the overall concept of design is discussed. The article also describes the role of the materials engineer in the design and materials selection process. It provides information on the significance of materials selection in both the prevention and analysis of failures.

This article provides an overview of the effects of various material- and process-related parameters on residual stress, distortion control, cracking, and microstructure/property relationships as they relate to various types of failure. It discusses phase transformations that occur during heat treating and describes the metallurgical sources of stress and distortion during heating and cooling. The article summarizes the effect of materials and the quench-process design on distortion and cracking and details the effect of cooling characteristics on residual stress and distortion. It also provides information on the methods of minimizing distortion and tempering. The article concludes with a discussion on the effect of heat treatment processes on microstructure/property-related failures.

Abstract
This article describes the general aspects of creep, stress relaxation, and yielding for homogeneous polymers. It then presents creep failure mechanisms in polymers. The article discusses extrapolative methods for the prediction of long-term creep failure in polymer materials. Then, the widely used models to simulate the service life of polymers are highlighted. These include the Burgers power-law model, the Findley power-law model, the time-temperature superposition (or equivalence) principle (TTSP), and the time-stress superposition principle (TSSP). The Larson-Miller parametric method, one of the most common to describe the material deformation and rupture time, is also discussed.

Abstract
Rolling-contact fatigue (RCF) is a common failure mode in components subjected to rolling or rolling-sliding contact. This article provides a basic understanding of RCF and a broad overview of materials and manufacturing techniques commonly used in industry to improve component life. A brief discussion on coatings to improve surface-initiated fatigue and wear is included, due to the similarity to RCF and the increasing criticality of this failure mode. The article presents a working knowledge of Hertzian contact theory, describes the life prediction of rolling-element bearings, and provides information on physics and testing of rolling-contact fatigue. Processes commonly used to produce bearings for demanding applications are also covered.

Abstract
This article discusses different types of mechanical fasteners, including threaded fasteners, rivets, blind fasteners, pin fasteners, special-purpose fasteners, and fasteners used with composite materials. It describes the origins and causes of fastener failures and with illustrative examples. Fatigue fracture in threaded fasteners and fretting in bolted machine parts are also discussed. The article provides a description of the different types of corrosion, such as atmospheric corrosion and liquid-immersion corrosion, in threaded fasteners. It also provides information on stress-corrosion cracking, hydrogen embrittlement, and liquid-metal embrittlement of bolts and nuts. The article explains the most commonly used protective metal coatings for ferrous metal fasteners. Zinc, cadmium, and aluminum are commonly used for such coatings. The article also illustrates the performance of the fasteners at elevated temperatures and concludes with a discussion on fastener failures in composites.

Abstract
This article briefly introduces the concepts of failure analysis and root cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It reviews four fundamental categories of physical root causes, namely, design deficiencies, material defects, manufacturing/installation defects, and service life anomalies, with examples. The article describes several common charting methods that may be useful in performing an RCA. It also discusses other failure analysis tools, including review of all sources of input and information, people interviews, laboratory investigations, stress analysis, and fracture mechanics analysis. The article concludes with information on the categories of failure and failure prevention.

Abstract
This article briefly introduces the concepts of failure analysis, including root-cause analysis (RCA), and the role of failure analysis as a general engineering tool for enhancing product quality and failure prevention. It initially provides definitions of failure on several different levels, followed by a discussion on the role of failure analysis and the appreciation of quality assurance and user expectations. Systematic analysis of equipment failures reveals physical root causes that fall into one of four fundamental categories: design, manufacturing/installation, service, and material, which are discussed in the following sections along with examples. The tools available for failure analysis are then covered. Further, the article describes the categories of mode of failure: distortion or undesired deformation, fracture, corrosion, and wear. It provides information on the processes involved in RCA and the charting methods that may be useful in RCA and ends with a description of various factors associated with failure prevention.
