Abstract
This article introduces some of the general sources of heat treating problems with particular emphasis on problems caused by the actual heat treating process and the significant thermal and transformation stresses within a heat treated part. It addresses the design and material factors that cause a part to fail during heat treatment. The article discusses the problems associated with heating and furnaces, quenching media, quenching stresses, hardenability, tempering, carburizing, carbonitriding, and nitriding as well as potential stainless steel problems and problems associated with nonferrous heat treatments. The processes involved in cold working of certain ferrous and nonferrous alloys are also covered.

Abstract
The various methods of furnace, torch, induction, resistance, dip, and laser brazing are used to produce a wide range of highly reliable brazed assemblies. However, imperfections that can lead to braze failure may result if proper attention is not paid to the physical properties of the material, joint design, prebraze cleaning, brazing procedures, postbraze cleaning, and quality control. Factors that must be considered include brazeability of the base metals; joint design and fit-up; filler-metal selection; prebraze cleaning; brazing temperature, time, atmosphere, or flux; conditions of the faying surfaces; postbraze cleaning; and service conditions. This article focuses on the advantages, limitations, sources of failure, and anomalies resulting from the brazing process. It discusses the processes involved in the testing and inspection required of the braze joint or assembly.

Fatigue Fracture of 1045 Steel U-Bolts on a Tractor Fitting

Abstract
SAE grade 5 U-bolts were used to fasten auxiliary dual wheels to the axles on a farm tractor. Under typical farm usage, the bolts are expected to have infinite life. However, several U-bolts made of 29 mm diam rod broke after less than 100 h of service. The bolt legs in which the failures occurred were all in the same position relative to the direction of wheel rotation. Visual examination showed the break was a fairly flat transverse fracture in the threaded section between the washer and the nut. The appearance of the fracture surfaces was characteristic of failure by low-cycle fatigue, with a smooth matte fatigue failure region showing beach marks and generally extending over about 40 to 60% of the fracture surface, which indicated severe overload. The point of initiation of fatigue was at the root of the last thread at the edge of the nut on the side toward this washer. The U-bolts fractured in fatigue because the bolt material had poor hardenability relative to the diam of the bolts. The bolt material was changed from 1045 steel to 1527 steel, a warm-finished low-alloy steel. The diameter of the bolts was reduced to 27.2 mm and the threads were rolled rather than cut.

Abstract
Induction-hardened teeth on a sprocket cast of low-alloy steel wore at an unacceptably high rate. A surface hardness of 50 to 51 HRC was determined; 55 HRC minimum had been specified. Analysis revealed that the alloy content of the steel was adequate for the desired hardenability but that the specified carbon content (0.29%) was too low. The low specified carbon content resulted in unacceptably low hardness. Because hardness largely controls wear rate, an early failure occurred. The specification for this part was changed so that a higher carbon content (0.45% C) was required.

Poorly Drawable Steel Wire for Ball Bearings

Abstract
A drawing plant which processed steel wire of designation 105 Cr 2 for ball bearings had losses due to crack formation and wire breakage during drawing. To establish the reason for the breakage, seven fractures were submitted for investigation with contiguous wire segments on both sides of the fracture of 300 mm each. Missing in the lamellar surface structure, with the exception of the remnants of a coarse network, were the pre-eutectically precipitated carbides to be expected in this steel. Surrounding the ferritic region in the surface structure, a ring of lamellar pearlite is seen, which turns into the granular annealed structure towards the core. The described structural phenomena were noted in all of the seven fracture regions. Their intensity always decreased with increasing distance from the fracture. Surface decarburization caused the formation of lamellar pearlite during annealing. This investigation further revealed that the localized decarburization and pearlite formation was present already in the rolled wire in uneven distribution over the entire coil length.

Failure of a Main Steam Line of a Power-Generating Station

Abstract
A main steam pipe was found to be leaking due to a large circumferential crack in a pipe-to-fitting weld in one of two steam leads between the superheater outlet nozzles and the turbine stop valves (a line made of SA335-P22 material). The main crack surface was found to be rough, oriented about normal to the outside surface, and had a dark oxidized appearance. The cracking was found to be predominantly intergranular. Distinct shiny bands that etched slower than the remainder of the sample at the top of each individual weld bead were revealed by microscopic examination. These bands were found contain small cracks and microvoids. A mechanism of intergranular creep rupture at elevated temperature was identified as a result of a series of stress-rupture and tensile tests. It was revealed by the crack shape that cracking initiated on the pipe exterior, then propagated inward and in the circumferential direction in response to a bending moment load. It was concluded that the primary cause of failure was the occurrence of bending stresses that exceeded the stress levels predicted by design calculations and that were higher than the maximum allowable primary membrane stress.

Crevice-Corrosion Failure of Evaporator Tubes Because of Defective Seam Welds

Abstract
Several tubes in a tube bundle in an evaporator used to concentrate an acid nitrate solution failed by leakage. The feed to the evaporator contained about 6% nitrate, and the discharge about 60% nitrate. The tube bundle was comprised of type 309S (Nb) stainless steel drawn-and-welded tubes expanded and welded into two type 304L stainless steel tube sheets. The tubes failed by crevice corrosion. The failed tubes were defective as-received, and the establishment of concentration cells within the longitudinal cracks in the seam welds led to ultimate corrosive penetration of the wall. There was no evidence of crevice corrosion or any localized penetration of tubes that had sound welds. The leaking type 309S (Nb) welded tubes should be replaced with seamless tubes of type 304L stainless steel to minimize the areas requiring welding and to provide maximum weldability for the tube-sheet joints.

Abstract
Rolling-element bearings use rolling elements interposed between two raceways, and relative motion is permitted by the rotation of these elements. This article presents an overview of bearing materials, bearing-load ratings, and an examination of failed bearings. Rolling-element bearings are designed on the principle of rolling contact rather than sliding contact; frictional effects, although low, are not negligible, and lubrication is essential. The article lists the typical characteristics and causes of several types of failures. It describes failure by wear, failure by fretting, failure by corrosion, failure by plastic flow, failure by rolling-contact fatigue, and failure by damage. The article discusses the effects of fabrication practices, heat treatment and hardness of bearing components, and lubrication of rolling-element bearings with a few examples.

Abstract
This article describes the general root causes of failure associated with wrought metals and metalworking. This includes a brief review of the discontinuities or imperfections that may be the common sources of failure-inducing defects in bulk working of wrought products. The article discusses the types of imperfections that can be traced to the original ingot product. These include chemical segregation; ingot pipe, porosity, and centerline shrinkage; high hydrogen content; nonmetallic inclusions; unmelted electrodes and shelf; and cracks, laminations, seams, pits, blisters, and scabs. The article provides a discussion on the imperfections found in steel forgings. The problems encountered in sheet metal forming are also discussed. The article concludes with information on the causes of failure in cold formed parts.

Abstract
This article briefly reviews the general causes of weldment failures, which may arise from rejection after inspection or failure to pass mechanical testing as well as loss of function in service. It focuses on the general discontinuities observed in welds, and shows how some imperfections may be tolerable and how the other may be root-cause defects in service failures. The article explains the effects of joint design on weldment integrity. It outlines the origins of failure associated with the inherent discontinuity of welds and the imperfections that might be introduced from arc welding processes. The article also describes failure origins in other welding processes, such as electroslag welds, electrogas welds, flash welds, upset butt welds, flash welds, electron and laser beam weld, and high-frequency induction welds.

Abstract
A femoral knee implant was returned to the casting vendor for analysis after exhibiting poor bond strength between the cast substrate and a sintered porous coating. Both the coating and the substrate were manufactured from a cobalt-chromium-molybdenum alloy. Metallographic analysis indicated that a decarburized layer existed on all surfaces of the casting, which prevented bonding during the sintering thermal cycle. Bead-to-bead bonding within the coating appeared sufficient, and no decarburized layer was present on the bead surfaces. It was concluded that the decarburization did not occur during the sintering thermal cycle. It was recommended that the prosthetic manufacturer investigate atmosphere controls for all thermal cycles prior to coating.
