Abstract
This article reviews the mechanical behavior and fracture characteristics that discriminate structural polymers from metals, including plastic deformation. It provides overviews of crack propagation and fractography. The article presents the distinction between ductile and brittle fracture modes. Several case studies of field failure in various polymers are also presented to illustrate the applicability of available analytical tools in conjunction with an understanding of failure mechanisms.

Abstract
The discussion on the fracture of solid materials, both metals and polymers, customarily begins with a presentation of the stress-strain behavior and of how various conditions such as temperature and strain-rate affect the mechanisms of deformation and fracture. This article describes crazing and fracture in polymeric materials, with a review of the behavior of the elastic modulus as a function of temperature or time parameters, emphasizing the importance of the viscoelastic nature of their deformation and fracture. The discussion covers the behavior of polymers under stress, provides information on ductile and brittle behaviors, and describes craze initiation in polymers and crack formation and fracture by crazing. Macroscopic permanent deformation of polymeric materials caused by shear-yielding and crazing, which eventually can result in fracture and failure, is also covered.

Abstract
In addition to failures in shafts, this article discusses failures in connecting rods, which translate rotary motion to linear motion (and conversely), and in piston rods, which translate the action of fluid power to linear motion. It begins by discussing the origins of fracture. Next, the article describes the background information about the shaft used for examination. Then, it focuses on various failures in shafts, namely bending fatigue, torsional fatigue, axial fatigue, contact fatigue, wear, brittle fracture, and ductile fracture. Further, the article discusses the effects of distortion and corrosion on shafts. Finally, it discusses the types of stress raisers and the influence of changes in shaft diameter.

Abstract
This article focuses on characterizing the fracture-surface appearance at the microscale and contains some discussion on both crack nucleation and propagation mechanisms that cause the fracture appearance. It begins with a discussion on microscale models and mechanisms for deformation and fracture. Next, the mechanisms of void nucleation and void coalescence are briefly described. Macroscale and microscale appearances of ductile and brittle fracture are then discussed for various specimen geometries (smooth cylindrical and prismatic) and loading conditions (e.g., tension compression, bending, torsion). Finally, the factors influencing the appearance of a fracture surface and various imperfections or stress raisers are described, followed by a root-cause failure analysis case history to illustrate some of these fractography concepts.

Abstract
This article aims to identify and illustrate the types of overload failures, which are categorized as failures due to insufficient material strength and underdesign, failures due to stress concentration and material defects, and failures due to material alteration. It describes the general aspects of fracture modes and mechanisms. The article briefly reviews some mechanistic aspects of ductile and brittle crack propagation, including discussion on mixed-mode cracking. Factors associated with overload failures are discussed, and, where appropriate, preventive steps for reducing the likelihood of overload fractures are included. The article focuses primarily on the contribution of embrittlement to overload failure. The embrittling phenomena are described and differentiated by their causes, effects, and remedial methods, so that failure characteristics can be directly compared during practical failure investigation. The article describes the effects of mechanical loading on a part in service and provides information on laboratory fracture examination.

Abstract
The case study presented in this article details the failure investigation of an M50 alloy steel bearing used in a jet engine gearbox drive assembly. It discusses the investigative steps and analytic tools used to determine the root cause, highlighting the importance of continuous, thorough questioning by the investigating activity. The combined analyses demonstrated that the bearing failed by a single event overload as evidenced by bulk deformation and traces of foreign material on the rolling elements. The anomalous transferred metal found on the rolling elements subsequently led to the discovery of overlooked debris in an engine chip detector, and thus resulted in a review of several maintenance practices.

Abstract
A number of failures involving carbon and alloy steels were analyzed to assess the effects of inclusions and their influence on mechanical properties. Inclusions, including brittle oxides and more ductile manganese sulfides (MnS), affect fatigue endurance limit, fatigue crack propagation rates, fracture toughness, notch toughness, and transverse tensile properties, and do so in an anisotropic manner with respect to rolling direction. Significant property anisotropy has been documented in the failures investigated, providing evidence that designers failed to account for it. Typical fracture morphologies observed in such cases and metallographic appearances of MnS-containing materials are illustrated.

Brittle Cracking of a Cylindrical Spiral Gear of Axle Gearbox

Abstract
A cylindrical spiral gear, part of a locomotive axle assembly, cracked ten days after it had been press-fit onto a shaft, after which it sat in place as other repairs were made. Workers at the locomotive shop reported hearing a sound, and upon inspecting the gear, found a crack extending radially from the bore to the surface of one of the tooth flanks. The crack runs the entire width of the bore, passing through an oil hole in the hub, across the spoke plate and out to the tip of one of the teeth. Design requirements call for the gear teeth to be carburized, while the remaining surfaces, protected by an anti-carburizing coating, stay unchanged. Based on extensive testing, including metallographic examination, microstructural analysis, microhardness testing, and spectroscopy, the oil hole was not protected as required, evidenced by the presence of a case layer. This oversight combined with the observation of intergranular fracture surfaces and the presence of secondary microcracks in the case layer point to hydrogen embrittlement as the primary cause of failure. It is likely that hydrogen absorption occurred during the gas carburizing process.

Abstract
A wire hoisting rope on a drilling rig failed during a lift, after a few cycles of operation, causing extensive damage to support structures. The failure investigation that followed included mechanical property testing and chemical, metallurgical, and finite element analysis. The rope was made from multiple strands of 1095 steel wire. Its chemical composition, ferrite-pearlite structure, and high hardness indicate that the wire is a type of extra improved plow steel (EEIPS grade). The morphologies of the fracture surfaces suggest that the wires were subjected to tensile overloading. This was confirmed by finite element analysis, which also revealed compressive contact stresses between the wires and between the rope and sheave surface. Based on the results, it was concluded that a tensile overload, due to the combined effect of a sudden load and undersized sheave, is what ultimately caused the rope to fail.

Abstract
Wind turbine blades are secured by a number of high-strength bolts. The failure of one such bolt, which caused a turbine blade to detach, was investigated to determine why it fractured. Based on the results of a detailed analysis, consisting of stress calculations, chemical composition testing, metallurgical examination, mechanical property testing, and fractographic analysis, it was determined that the bolt failed by fatigue accelerated by stress concentration at low temperatures. The investigation also provided suggestions for avoiding similar failures.

Abstract
The head on a golf club driver developed multiple cracks during normal use. The head was a hollow shell construction made from a titanium alloy. Analysis and additional investigation revealed a progressive failure that initiated on the interior surface of the face plate along a deep, concentric groove created during a press forming operation. It was also determined that atmospheric contamination occurred during the welding of the head, causing embrittlement, which may have also contributed to the failure. Recommendations were made addressing the problems that were observed.

Abstract
A 13/16-in. hex socket failed while in use. Analysis (hardness testing, optical and scanning electron microscopy, and EDS) revealed that the socket was made of low carbon steel formed in a powder metallurgy process. A number of flaws were found including nonuniform wall thickness, poor geometric design with sharp corners as stress raisers, and incomplete sintering evidenced by unsintered particles. These were determined to be the primary cause of failure, although inclusions on the fracture surface containing S and Al may have played a role as well.

Abstract
A pipe in the lateral wall of a boiler powering an aircraft carrier flat-top boat failed during a test at sea. The pipe was made from ASTM 192 steel, an adequate material for the application. Microstructural analysis along with equipment operating records provided valuable insight into what caused the pipe to rupture. Although the pipe had been replaced just 50 h before the accident, the analysis revealed incrustations and corrosion pits on the inner walls and oxidation on the outer walls. Microstructural changes were also observed, indicating that the steel was exposed to high temperatures. The combined effect of pitting, incrustations, and phase transformations caused the pipe to rupture.

Abstract
An electric transport vehicle, similar to an electric trolley or subway rail car, experienced frequent breakdowns due to in-service fractures of torsion springs that support the weight of an overhead electric pickup assembly. Scanning electron microscopy and metallographic examinations determined that the fractures stemmed from electric arc damage. Intergranular quench cracks in the transformed untempered martensite on the surface of the spring provided crack initiations that propagated during operation causing fatigue fracture.

Abstract
Superelastic nitinol wires that fractured under various conditions were examined under a scanning electron microscope in order to characterize the fracture surfaces, produce reference data, and compare the findings with prior published work. The study revealed that nitinol fracture modes and morphologies are generally consistent with those of ductile metals, such as austenitic stainless steel, with one exception: Nitinol exhibits a unique damage mechanism under high bending strain, where damage occurs at the compression side of tight bends or kinks while the tensile side is unaffected. The damage begins as slip line formation due to plastic deformation, which progresses to cracking at high strain levels. The cracks appear to initiate from slip lines and extend in shear (mode II) manner.

Ductile Overload Failure of a T-Hook That Fractured in Service

Abstract
A ductile iron T-hook hook was reported to have fractured in service. It was further reported that the hook had been subjected to a load that did not exceed 5900 kg (13,000 lb) at the time of fracture. No information was provided regarding the type of metal used to manufacture the hook. A failure analysis was requested to determine the cause of fracture. Two hooks were submitted for examination. Analysis (visual inspection, 2.7x light fractography, chemical analysis, 110x SEM fractography, 27x/110x/215x nital-etched micrographs) supported the conclusions that this component fractured in service as a consequence of ductile tensile overload. Evidence indicates that the fractured region was subjected to a load exceeding the capacity of the material. Because the information available from the service application indicated that the component had not been subjected to a stress that exceeded 5900 kg (13,000 lb), the observations made in this investigation suggested that either the load was underestimated or that the indicated load was applied at a more rapid rate (perhaps with a jerk), which would tend to increase the effective force of the load.

Abstract
Mechanical properties of wire ropes, their chemical composition, and the failure analysis process for them are described. The wires are manufactured from high-carbon, plain carbon steel, with high-strength ropes most often manufactured from AISI Grade 1074. During visual failure examination, the rope, strand, and wire diameters should all be measured. Examination should also address the presence or absence of lubricant, corrosion evidence, and gross mechanical damage. Failed wires can exhibit classic cup-and-cone ductile features, flat fatigue features, and various appearances in-between. However, wires are often mechanically damaged after failure. Most nondestructive evaluation (NDE) techniques are not applicable to wire rope failures. Electron microscope fractography of fracture surfaces is essential in failure analysis. Fatigue is the most important fracture mode in wire ropes. Metallographic features of wire ropes that failed because of ductile overload and fatigue are described.

Abstract
On 21 April 1995, the contents of a large blender (6 cu m) reacted and caused an explosion that killed and injured a number of workers at a plant in Lodi, NJ. A mixture of sodium hydrosulfite and aluminum powder was being mixed at the time of the accident. This report focuses on evaluations of the blender to determine if material or mechanical failures were the cause of the accident. The results indicate that the mixing vessel was metallurgically sound and did not contribute to the initiation of the failure. However, the vessel was not designed for mixing chemicals that must be isolated from water and excessive heat. Water leaking into the vessel through a graphite seal may have initiated the reactions that caused the accident.

Fatigue Fracture of a Steel 8 × 19 Elevator Cable

Abstract
One of six cables on a passenger elevator was found fractured during a routine inspection. The cable is made of 16-mm steel wire rope designated 8 x 19 G Preformed Extra High Strength Special Traction Elevator Cable with fiber core. Samples of wire from the cable revealed two types of fractures: flat-type fractures were observed in 1.2 and 1 mm diam wires and cup-and-cone fractures were observed in 0.6 mm diam wires. A nick observed in the side of one of the larger wires was found to be rusted. Beach marks radiating inward, indicative of fatigue cracking, were also revealed. The smaller wires were found to be slightly oxidized and behaved in a ductile manner under excessive loads before ultimate failure. Flat-type fractures were believed to have resulted from cyclic torsional stresses along with longitudinal cracking. Restriction of free movement of the socket-end in the shackle was found to have promoted fracture due to increased magnitude of stresses. Mechanical damage to surfaces of wires was concluded to be sufficient to cause fatigue cracking under the stresses encountered in service.

Ruptured Prestressing Cables from a Viaduct

Abstract
During the construction of a prestressed concrete viaduct, several 12.2 mm diam wires ruptured after tensioning but before the channels were grouted. They were made of heat treated prestressed concrete steel St 145/160. While the wire bundles, each containing over 100 wires, were being drawn into the channels they were repeatedly pulled over the sharp edges of square section guide blocks. The fractures were initiated at these chafe zones. It was concluded that the chafing of the wires on the edges of the guide blocks, particularly the resulting martensite formation, caused the wires to rupture.
