Abstract
Because of the tough engineering environment of the railroad industry, fatigue is a primary mode of failure. The increased competitiveness in the industry has led to increased loads, reducing the safety factor with respect to fatigue life. Therefore, the existence of corrosion pitting and manufacturing defects has become more important. This article presents case histories that are intended as an overview of the unique types of failures encountered in the freight railroad industry. The discussion covers failures of axle journals, bearings, wheels, couplers, rails and rail welds, and track equipment.

Abstract
A type 304 stainless steel tube that failed in a boiler stack economizer was analyzed to determine the cause. The investigation consisted of visual, SEM/EDS, and metallographic analysis. Several degradation mechanisms appeared to be at work, including pitting corrosion, chloride stress corrosion cracking, and fatigue fracture. Investigators concluded that the primary failure mechanism was fatigue fracture, although either of the other mechanisms may have eventually caused the tube to fail in the absence of fatigue.

Abstract
High-level radioactive wastes generated during the processing of nuclear materials are kept in large underground storage tanks made of low-carbon steel. The wastes consist primarily of concentrated solutions of sodium nitrate and sodium hydroxide. Each of the tanks is equipped with a purge ventilation system designed to continuously remove hydrogen gas and vapors without letting radionuclides escape. Several intergranular cracks were discovered in the vent pipe of one such system. The pipe, made of galvanized steel sheet, connects to an exhaust fan downstream of high-efficiency particulate air filters. The failure analysis investigation concluded that nitrate-induced stress-corrosion cracking was the cause of the failure.

Abstract
This paper describes the investigation of a corrosion failure of bottom plates on an aboveground tank used for the storage of potable water. The tank was internally inspected for the first time after six years of service. Paint blisters and rust spots were observed on the bottom plates and first to third course shell plates. Sand blasting and repainting of the bottom plates and first course shell plates was to be used as a remedial measure. However, during the sand blasting, holes and deep pitting were observed on the bottom plates. On-site visual inspection, magnetic flux leakage (MFL) inspection, ultrasonic testing (UT), and evaluation of the external cathodic protection (CP) system were used in the failure analysis. The corrosion products were analyzed using energy-dispersive X-ray analysis (EDAX). The failure is attributed to the ingress of water and its impoundment under the tank bottom along the periphery inside the ring wall and failure of water side epoxy coating. Various measures to prevent such failures in the future are recommended.

Abstract
This study examines the role of calcium-precipitating bacteria (CPB) in heat exchanger tube failures. Several types of bacteria, including Serratia sp. (FJ973548), Enterobacter sp. (FJ973549, FJ973550), and Enterococcus sp. (FJ973551), were found in scale collected from heat exchanger tubes taken out of service at a gas turbine power station. The corrosive effect of each type of bacteria on mild steel was investigated using electrochemical (polarization and impedance) techniques, and the biogenic calcium scale formations analyzed by XRD. It was shown that the bacteria contribute directly to the formation of calcium carbonate, a critical factor in the buildup of scale and pitting corrosion on heat exchanger tubes.

Abstract
Fretting and fretting corrosion at the contact area between the screw hole of a type 316LR stainless steel bone plate and the corresponding screw head was studied. The attack on the 316LR stainless steel was only shallow. Mechanical grinding and polishing structures were exhibited by a large portion of the contact area. Fine corrosion pits in the periphery were observed and intense mechanical material transfer that can take place during fretting was revealed. Smearing of material layers over each other during wear was observed and attack by pitting corrosion was interpreted to be possible.

Abstract
Heavy pitting corrosion on type 304 stainless steel bone screw was studied. A screw head that exhibited heavy pitting corrosion attack was observed. Deep tunnels that penetrated the screw head and followed the inclusion lines were revealed. The screw was inserted in a plate made of type 316LR stainless steel and some mechanical fretting and very few corrosion pits were revealed. Type 304 stainless steel was deemed not to be satisfactory as an implant material.

Abstract
TiN coated back surgery wires were made of Ti-6Al-4V. The reported failure was the presence of pits located in the uncoated area of the wires. The uncoated area of the wire is where the wire is fixtured in the coating chamber during coating. Examination and analysis of the pits using SEM/EDX detection unit revealed significant peaks of B, O, Zr and Fe. Moreover, the shape of the pits was similar to an arc crater. The formation of pits in the wire was caused during coating due to microarcing. A contaminated fixture used during the coating most likely caused the microarcing.

Abstract
Coiled tubing used in drilling operations failed at the halfway point of its estimated fatigue life. The failure was found to be transverse to the tubing axis. Visual examination revealed a flat fracture surface extending 13 mm with the rest of the fracture showing shear lips indicative of tensile overload. The flat portion of the fracture surface was typical of fatigue cracking. Fatigue striations were revealed by SEM. Corrosion pitting on the tubing ID from which the fatigue crack had propagated were observed on closer examination. The corrosion pitting was speculated to have occurred when the tubing was idle and fluids accumulated at the bottom of the tubing wraps. The coiled tubing was concluded to have failed prematurely due to low-cycle fatigue initiated at corrosion pitting sites. Corrosive attack on the coiled tubing was recommended to be reduced by completely removing fluids or modifying the fluids in the tubing or purging by flowing dry nitrogen to dry it out.

Abstract
Type 316L stainless steel pipes carrying brine at 120 deg C (250 deg F) and at a pH of about 7, failed by perforation at or near circumferential butt-weld seams. The failure was examined optically and radiographically in the field. Specimens were removed and examined metallographically and with a SEM in the laboratory. The examinations revealed a combination of failure mechanisms. The pitting failure of the welds was attributed to localized attack of an activated surface, in which anodic pits corroded rapidly. Additionally, SCC driven by residual welding stresses occurred in the base metal adjacent to the welds. Use of highly stressed austenitic stainless steels in high-chloride environments having a temperature above 65 deg C (150 deg F) should be discouraged. Solution annealing or shot peening to reduce residual stresses may be advisable. If heat treatment is not feasible after welding, the substitution of a more corrosion-resistant alloy, such as Incoloy 800 or 825, may be necessary.

Abstract
Visual examination, optical and scanning electron microscopy were used to determine the cause of failure in the connector groove of a marine riser coupling. The specified steel was AISI 4142 (0.40 to 0.45% C; 0.75 to 1.00% Mn; 0.20 to 0.35% Si; 0.80 to 1.10% Cr; 0.15 to 0.25% Mo) normalized from 9000C. Microscopic examination revealed the crack's initiation point and subsequent propagation. SEM examination of chemically stripped corrosion showed that corrosion fatigue and stress corrosion might have contributed to the initial slow crack growth. Impact tests revealed a fracture transition temperature in excess of 1000C. The sequence of events leading to failure was detailed. The main recommendation was to quench and temper existing couplings and to use a lower carbon quenched and tempered steel for new couplings.

Abstract
On 9 March 2000, a gasoline pipeline failed near Greenville, TX releasing approximately 12,000 barrels of fuel. After the on-scene portion of the investigation was completed, an 8.5 ft. (2.6 m) section of the 28 in. (71 cm) diam pipe was sent to the materials laboratory for examination. Examination included optical and scanning electron microscopy of the fracture surfaces and metallographic examination of cross sections through the fracture surface. From the outer to inner edge of the fracture surface, three different areas were observed. Fracture features in area 1 were obliterated by corrosion. The fracture features in region 2 were relatively smooth, and striations were observed, typical of fatigue. In region 3, dimple features were observed, typical of ductile overstress. Also, corrosion pits were observed on the outer surface of the pipe section in locations where the protective black tar-like coating was cracked.

Abstract
Sucker-rod pumps are operating in very aggressive environments in oil well production. The combined effect of a corrosive environment and significant mechanical loads contribute to frequent cases of failure of the rod string during operation. Standards and recommendations have been developed to control and avoid those failures. This study presents various failure cases of sucker rods in different applications. The heat treatment of the steel material and the resulting microstructure are an important factor in the behavior of the sucker rod. A spheroidized microstructure presents a weaker resistance to corrosion affecting the rod life. Non-metallic inclusions are a pitting preferential site leading to fatigue crack initiation. Heterogeneous microstructure as banded martensite and ferrite/pearlite decreases the ductility of the material affecting the fatigue propagation resistance.

Abstract
A steel galvanizing vat measuring 3 x 1.2 x 1.2 m (10 x 4 x 4 ft) and made of 19 mm thick carbon steel plate (ASTM A285, grade B)) at a shipbuilding and ship-repair facility failed after only three months of service. To verify suspected failure cause, two T joints were made in 12.5 mm thick ASTM A285, grade B, steel plate. One joint was welded using the semiautomatic submerged arc process with one pass on each side. A second joint was welded manually by the shielded metal arc process using E6010 welding rod and four passes on each side. The silicon content of the shielded metal arc weld was 0.54%, whereas that of the submerged arc weld was 0.86%. After being weighed, the specimens were submerged in molten zinc for 850 h. Analysis (visual inspection, chemical analysis, 100x 2% nital-etched micrographs) supported the conclusions that the vat failed due to molten-zinc corrosion along elongated ferrite bands, possibly because silicon was dissolved in the ferrite and thus made it more susceptible to attack by the molten zinc. Recommendations included rewelding the vat using the manual shielded metal arc process with at least four passes on each side.

Abstract
An austenitic stainless steel (type 316/316L stainless steel, schedule 40, 64 mm (2.5 in.) diam and larger) piping network used in the fire-sprinkler system in a large saltwater passenger and car ferry failed by rapid leaking. Operating conditions involved stagnant seawater at ambient temperatures. The pipe was in service for four weeks when three leaks appeared. Investigation (visual inspection and photographic images) supported the conclusion that the failure was caused by attack and corrosion damage of Cl ions in conditions that were ideal for three modes of highly accelerated pitting of austenitic stainless steel: the bottom surface, weld or HAZ pits, and crevices. Recommendations included proper material selection for piping, flanges, and weld rods with greater corrosion resistance. Proper filtering to prevent entrained abrasives and timely breakdown inspections were also advised.

Abstract
Plate perforation occurred in the cylindrical section and walls of the inlet foot (2.38 mm thick Incoloy 825 plate welded using INCO welding rod 135) of an inert gas fire prevention system in an oil tanker. Cross-sectional microprobe analysis showed the corrosion product to contain sulfur, mainly from the flue gas, and calcium and chlorine, mainly from the sea water. The gray corrosion product was interspersed with rust and a black carbonaceous deposit. Corrosion pitting and poor weld penetration, with carbide precipitation and heavy etching at grain boundaries, indicated sensitization and susceptibility to aqueous intergranular corrosion. Chemical analysis showed the predominant acid radical to be sulfate (6.20% in the carbonaceous deposit and 0.60% in the corrosion product), suggesting that oxidation of SO2 in the flue gas caused the corrosion. Moisture condensation, the carbon acting as a cathode, and alloy susceptibility to intergranular corrosion contributed to the corrosion.

Abstract
While in the stationary mode, capillary action at the contact line between roller and race in a steel rolling mill taper bearing caused a concentration of lubricant and moisture to occur. This lead to lines of corrosion pits at roller intervals. During subsequent operation, the individual corrosion pits acted as stress raisers and initiated coarse grain spalling. Due to a bending moment on the rotating element, this in turn initiated bending fatigue normal to the longitudinal axis, which propagated through to the bore of the inner ring. Stain marks were visible in the bore at a spacing corresponding to roller intervals where lubricant had flowed through the cracks from the race.

Abstract
The metallurgical condition of a cylindrical induction melter (CIM) vessel was evaluated after approximately 375 h of operation over a two-year span at temperatures between 1400 to 1500 deg C. Wall thinning and significant grain growth was observed in the lower portion of the conical section and the drain tube. No through-wall penetrations were found in the cylindrical and conical sections of the CIM vessel and only one leak site was identified in the drain tube. Failure of the drain tube was associated with localized overheating and creep. The observed degradation resulted from cumulative service at elevated temperature. A recommendation was made to implement a support for the conical section of the CIM and to increase the wall thickness of the drain tube. Thus, the possibility of drain tube misalignment in the induction coils and localized over heating will be minimized. In addition, the use of grain stabilized Pt/Rh alloy should be evaluated as a method to prevent grain growth.

Abstract
A hollow, splined alloy steel aircraft shaft (machined from an AMS 6415 steel forging – approximately the same composition as 4340 steel – then quenched and tempered to a hardness of 44.5 to 49 HRC) cracked in service after more than 10,000 h of flight time. The inner surface of the hollow shaft was exposed to hydraulic oil at temperatures of 0 to 80 deg C (30 to 180 deg F). Analysis (visual inspection, 15-30x low magnification examination, 4x light fractograph, chemical analysis, hardness testing) supported the conclusions that the shaft cracked in a region subjected to severe static radial, cyclic torsional, and cyclic bending loads. Cracking originated at corrosion pits on the smoothly finished surface and propagated as multiple small corrosion-fatigue cracks from separate nuclei. The originally noncorrosive environment (hydraulic oil) became corrosive in service because of the introduction of water into the oil. Recommendations included taking additional precautions in operation and maintenance to prevent the use of oil containing any water through filling spouts or air vents. Also, polishing to remove pitting corrosion (but staying within specified dimensional tolerances) was recommended as a standard maintenance procedure for shafts with long service lives.

Abstract
A failed H-11 tool steel pylon attachment stud was found during a routine walk-around inspection. The stud exhibited gross localized corrosion pitting at several different areas on its surface. Light general rust was also evident. Severe pitting occurred near the fracture location. The fracture face contained evidence of intergranular SCC as well as ductile dimples. The protective coating was found to be an inorganic water-base aluminide coating having a coating thickness of 7.5 to 13 micron (0.3 to 0.5 mil). The coating was of a nonuniform mottled nature. It was concluded that the failure of the pylon attachment stud was caused by general corrosion followed by SCC. The stud was not adequately protected against corrosion by the coating. It was recommended that the coating be applied to a thickness of 38 to 75 micron (1.5 to 3 mil) to provide long-time corrosion resistance. The coating must be either burnished or cured at 540 deg C (1000 deg F) to provide cathodic protection to the steel. Other coatings, such as cadmium or aluminum, were also recommended if a thinner coating is needed.
