Abstract
Because of the tough engineering environment of the railroad industry, fatigue is a primary mode of failure. The increased competitiveness in the industry has led to increased loads, reducing the safety factor with respect to fatigue life. Therefore, the existence of corrosion pitting and manufacturing defects has become more important. This article presents case histories that are intended as an overview of the unique types of failures encountered in the freight railroad industry. The discussion covers failures of axle journals, bearings, wheels, couplers, rails and rail welds, and track equipment.

Abstract
This paper describes the investigation of a corrosion failure of bottom plates on an aboveground tank used for the storage of potable water. The tank was internally inspected for the first time after six years of service. Paint blisters and rust spots were observed on the bottom plates and first to third course shell plates. Sand blasting and repainting of the bottom plates and first course shell plates was to be used as a remedial measure. However, during the sand blasting, holes and deep pitting were observed on the bottom plates. On-site visual inspection, magnetic flux leakage (MFL) inspection, ultrasonic testing (UT), and evaluation of the external cathodic protection (CP) system were used in the failure analysis. The corrosion products were analyzed using energy-dispersive X-ray analysis (EDAX). The failure is attributed to the ingress of water and its impoundment under the tank bottom along the periphery inside the ring wall and failure of water side epoxy coating. Various measures to prevent such failures in the future are recommended.

Abstract
This study examines the role of calcium-precipitating bacteria (CPB) in heat exchanger tube failures. Several types of bacteria, including Serratia sp. (FJ973548), Enterobacter sp. (FJ973549, FJ973550), and Enterococcus sp. (FJ973551), were found in scale collected from heat exchanger tubes taken out of service at a gas turbine power station. The corrosive effect of each type of bacteria on mild steel was investigated using electrochemical (polarization and impedance) techniques, and the biogenic calcium scale formations analyzed by XRD. It was shown that the bacteria contribute directly to the formation of calcium carbonate, a critical factor in the buildup of scale and pitting corrosion on heat exchanger tubes.

Abstract
A type 304 stainless steel tube that failed in a boiler stack economizer was analyzed to determine the cause. The investigation consisted of visual, SEM/EDS, and metallographic analysis. Several degradation mechanisms appeared to be at work, including pitting corrosion, chloride stress corrosion cracking, and fatigue fracture. Investigators concluded that the primary failure mechanism was fatigue fracture, although either of the other mechanisms may have eventually caused the tube to fail in the absence of fatigue.

Abstract
High-level radioactive wastes generated during the processing of nuclear materials are kept in large underground storage tanks made of low-carbon steel. The wastes consist primarily of concentrated solutions of sodium nitrate and sodium hydroxide. Each of the tanks is equipped with a purge ventilation system designed to continuously remove hydrogen gas and vapors without letting radionuclides escape. Several intergranular cracks were discovered in the vent pipe of one such system. The pipe, made of galvanized steel sheet, connects to an exhaust fan downstream of high-efficiency particulate air filters. The failure analysis investigation concluded that nitrate-induced stress-corrosion cracking was the cause of the failure.

Abstract
AISI type 410 stainless steel tube bundles in a heat exchanger experienced leakage during hydrostatic testing even before being in service. The inside surfaces of the tubes was observed to have been pitted. Chloride-ion pitting was revealed by the undercutting in the cross section of a pit and further confirmed by x-ray spectrometry. It was concluded that the failure was caused by pitting due to chlorides in the water used to flush the tubes before service. The use of brackish water to flush or test stainless steel equipment was recommended to avoid pitting.

Abstract
An aluminum brass seawater surface condenser failed due to pitting after less than one year of service. Large pits filled with a green deposit were evidenced under the nonuniform black scale present over the entire inside surface of the tube. The black deposit was identified as primarily copper sulfide, with zinc and aluminum sulfides while the green deposit was revealed to be copper chloride. The combination of sulfide and chloride attack on the tubes was concluded to have resulted in the failure. Injection of ferrous sulfate upstream of the condenser which could aid the formation of protective oxide films was recommended.

Abstract
Eddy current inspection was performed on a vertical evaporator unit (that contained 180 tubes) used in a chemical processing plant. It was advised that the tube material was type 316 stainless steel. The shell-side fluid was condensate and gaseous methylene chloride, while the tube-side fluid was contaminated liquid methylene chloride. More than 100 tubes exhibiting severe outer surface pitting and cracklike indications near each tube sheet were revealed during eddy current inspection. It was observed that the indications correlated with rust-stained, pitted, and cracked areas on the outer surfaces. The cracking was revealed by metallographic examination to have initiated from the outer surface, frequently at pits, and penetrated the tube wall in a transgranular, branching fashion. The crack features were characteristic of chloride stress-corrosion cracking. A change in tube material was recommended to avoid future failures and loss of service.

Abstract
A failure of an aboveground storage tank occurred due to external corrosion of the tank floor. The liquid asphalt tank operated at elevated temperatures (approximately 177 deg C, or 350 deg F) and had been in service for six years. Cathodic protection (rectifiers) had been installed since start-up of the tank operation. It was noted, however, that some operational problems with the rectifier may have interrupted its protection. Investigation (visual inspection, on-site examination and testing, EDS analysis of scale deposits, and MIC testing of the soil) supported the conclusion that corrosion may have been caused by an interruption in cathodic protection. The effectiveness of cathodic protection on established microbial deposits is questionable. Recommendations included ultrasonically testing the tank floor and replacing portions based on the remaining wall thickness. Doubling the wall thickness of the floor plates was also recommended.

Abstract
AISI type 321 stainless steel heat exchanger tubes failed after only three months of service. Macroscopic examination revealed that the leaks were the result of localized pitting attack originating at the water side surfaces of the tubes. Metallographic sections were prepared from both sets of tubes. Microscopic examination revealed that the pits had a small mouth with a large subsurface cavity which is typical of chloride pitting of austenitic stainless steel. However, no pitting was found in other areas of the system, where the chloride content of the process water was higher. This was attributed to the fact that they were downstream from a deaeration unit. It was concluded that the pitting was caused by a synergistic effect of chlorine and oxygen in the make-up water. Because it was not possible to install a deaeration unit upstream of the heat exchangers, it was recommended that a molybdenum-bearing stainless steel such as 316L or 317L be used instead of 321.

Abstract
This investigation involved two AISI 304L acid storage tanks and one AISI 304L spent solvent tank from a sewage treatment facility. After installation, these tanks were hydrostatically tested using sewage effluent. No leaks were found and after a year or two, the tanks were drained and filled with nitric acid in preparation for service. Three weeks later the two acid tanks were found to be leaking from the bottom. Samples from the spent solvent tank revealed that pitting was located in a depressed area near a suction hole, beneath a black residue. It was concluded that the acid tanks failed by chloride-induced pitting initiated by microbial activity. Further, the spent solvent tank failed by a similar, but anaerobic mechanism. The use of the effluent for the hydrostatic test and the failure to remove it and clean and dry the tanks was the primary cause of failure. Localized carbide segregation in the original plate served as preferential corrosion sites. Had the tanks been hydrostatically tested in a proper manner, the pitting may not have occurred.

Abstract
Post-service destructive evaluation was performed on two commercially pure zirconium pump impellers. One impeller failed after short service in an aqueous hydrochloric acid environment. Its exposed surfaces are bright and shiny, covered with pockmarks, and peppered with pitting. Uniform corrosion is evident and two deep linear defects are present on impeller blade tips. In contrast, the undamaged impeller surfaces are covered with a dark oxide film. This and many other impellers in seemingly identical service conditions survive long lives with little or no apparent damage. No material or manufacturing defects were found to explain the different service performance of the two impellers. Microstructure, microhardness and material chemistry are consistent with the specified material. Examination reveals the damage mechanism to be corrosion-enhanced cavitation erosion, the most severe form of erosion corrosion. Cavitation damage to the protective oxide film caused the zirconium to lose its normally outstanding corrosion resistance. The root cause of the impeller failure is most likely the introduction of excessive air into the pump due to low liquid level, a bad seal or inadequate head. Corrosion pitting, crevice corrosion, and solidification cracks (casting defect) also contributed to the failure.

Abstract
The exhaust valve of a truck engine failed after 488 h of a 1000 h laboratory endurance test. The valve was made of 21-2 valve steel in the solution treated and aged condition and was faced with Stellite 12 alloy. The failure occurred by fracture of the underhead portion of the valve. Analysis (visual inspection, electron probe x-ray microanalysis, hardness testing, 4.5x fractograph) supported the conclusions that failure of the valve stem occurred by fatigue as a result of a combination of a nonuniform bending load, which caused a mild stress-concentration condition, and a high operating temperature in a corrosive environment. When the microstructure near the stem surface was examined, it was apparent that carbide spheroidization had occurred. Also, there was a coarsening of the carbide network within the austenite grains. The microstructure indicated that the underhead region of the valve was heated to about 930 deg C (1700 deg F) during operation. The cause of fatigue fracture, therefore, was a combination of non-uniform bending loads and overheating. No recommendations were made.

Abstract
Severe pitting corrosion of a carbon steel tube was observed in the air preheater of a power plant, which runs on rice straw firing. Approximately 1450 tubes were removed from Stage 3 of the preheater (air inlet and flue gas outlet) due to corrosion and local bursting. Samples from Stage 2 (where corrosion was low) and Stage 3 (severe corrosion) were taken and subjected to visual inspection, SEM, x-ray diffraction, microhardness measurement, and chemical and microstructural analysis. It was determined that extended non-operation of the plant resulted in the settlement of corrosive species on the tubes in Stage 3. The complete failure of the tube occurred due to diffusion of these elements into the base metal and precipitation of potassium and chlorine compounds along the grain boundaries, with subsequent dislodging of grains. The nonmetallic inclusions acted as nucleating sites for local pitting bursting. Nonuniform heat transfer in Stage 3 operation accelerated the selective corrosion of front-end tubes. The relatively high heat transfer in this stage resulted in condensation of some corrosive gases and consequent corrosion. Continuous operation of the plant with some precautions during assembly of the tubes reduced the corrosion problem.

Abstract
An arsenical admiralty brass (UNS C44300) finned tube in a generator air cooler unit at a hydroelectric power station failed. The unit had been in operation for approximately 49,000 h. The cooling medium for the tubes was water from a river. Air flowed over the finned exterior of the tubes, while water circulated through the tubes. Investigation (visual inspection, leak testing, history review, 100X micrographs etched in potassium dichromate, chemical analysis, and EDS and XRD analysis of internal tube deposits) supported the conclusion that the cause of the tube leaks was ammonia-induced SCC. Because the cracks initiated on the inside surfaces of the tubes and because the river water was not treated before it entered the coolers, the ammonia was likely present in the river water and probably concentrated under the internal deposits. Recommendations included either eliminating the ammonia (prohibitively expensive in cost and time) or using an alternate material (such as a 70Cu-30Ni alloy or a more expensive titanium alloy) that is resistant to ammonia corrosion as well as to chlorides and sulfur species.

Abstract
A superheater in a generator produced 80 t/h of steam at 400 deg C and 41 kPa. Failure took place at the connection from the collector to the vent line used during start up. The material of construction was carbon steel, and the unit had 240,000 h of operation at the time of failure, with 99 shutdowns. Widespread cracking on the inside was apparent, the most severe cracking being some distance from the nozzle connection in a downstream direction. Widespread cracking and pitting were observed also at the connections to the safety valve and soot blower. Pitting was most apparent on the downstream sides of the openings in the shell. In all the damaged areas the mechanism of failure involved surface pitting and subsequent SCC. This failure showed the problems that can develop where there are long lines in which condensation may occur and return periodically to a superheater or other hot component. In this particular case, control of dissolved solids in the boiler feedwater may have been inadequate.

Abstract
Large quantities of aluminum-clad spent nuclear materials have been in interim storage in the fuel storage basins at The Savannah River Site while awaiting processing since 1989. This extended storage as a result of a moratorium on processing resulted in corrosion of the aluminum clad. Examinations of this fuel and other data from a corrosion surveillance program in the water basins have provided basic insight into the corrosion process and have resulted in improvements in the storage facilities and basin operations. Since these improvements were implemented, there has been no new initiation of pitting observed since 1993. This paper describes the corrosion of spent fuel and the metallographic examination of Mark 31A target slugs removed from the K-basin storage pool after 5 years of storage. It discusses the SRS Corrosion Surveillance Program and the improvements made to the storage facilities which have mitigated new corrosion in the basins.

Abstract
A straight-tube cooler type heat exchanger had been in service for about ten years serving a coal pulverizer in Georgia. Non-potable cooling water from a local lake passed through the inner surfaces of the copper tubing and was cooling the hot oil that surrounded the outer diametral surfaces. Several of the heat exchangers used in the same application at the plant had experienced a severe reduction in efficiency in the past few years. One heat exchanger reportedly experienced some form of leakage following discovery of oil contaminating the cooling water. This heat exchanger was the subject of a failure investigation to determine the cause and location of the leaks. Corrosion products primarily contained copper oxide, as would be expected from a copper tubing. The product also exhibited the presence of a significant amount of iron oxides. Metallographic cross sectioning of the tubes and microscopic analysis revealed several large and small well rounded corrosion pits present at the inner diametral surfaces. The cause of corrosion was attributed to corrosive waters that were not only corroding the copper, but were corroding steel pipes upstream from the tubing.

Abstract
Aluminum-clad spent nuclear fuel is stored in water filled basins at the Savannah River Site awaiting processing or other disposition. After more than 35 years of service underwater, the aluminum storage racks that position the fuel bundles in the basin were replaced. During the removal of the racks from the basin, a failure occurred in one of the racks and the Savannah River Technology Center was asked to investigate. This paper presents the results of the failure analysis and provides a discussion of the effects of corrosion on the structural integrity of the storage racks.

Abstract
Several mercury diffusion pump stages in the Tritium Purification process at the Savannah River Site (SRS) have been removed from service for scheduled preventive maintenance. These stages have been examined to determine if failure has occurred. Evidence of fatigue around the flange portion of the pump has been seen. In addition, erosion and cavitation inside the throat of the venturi tube and corrosion on the other surface of the venturi tube has been observed. Several measures are being examined in an attempt to improve the performance of these pumps. These measures, as well as the noted observations, are described. Six stages [two machined (MP) and four electron beam (EB) welded] from the mercury diffusion pumps operating in the Tritium Purification process at SRS have been analyzed to determine their condition after nine months of usage. Several cracks were found around the necked region of the two MP stages. The EB welded stages, however, seemed to perform better in service; only two of four stages showed cracking. The cracking is caused by fatigue that has been enhanced by high stresses and tritium in the flange area. The EB welded stage appears to be a step in the right direction. Since the EB weld is a shrink fit, the surface is in compression, thereby eliminating crack propagation. In addition, shot peening has been employed to produce a compressive material surface since fatigue usually originates at the surface. Pitting was observed down the throat of the venturi. This pitting was caused by cavitation and erosion along the length of the venturi tube. Corrosion and pitting was seen on the exterior walls of the diffuser tubes. Stress-corrosion cracks were observed emanating from these corrosion pits. The corrosion likely occurred from the chloride ions present in the process cooling water. Shot peening is now being used in an attempt to place the outside of the diffuser tube in compression to eliminate the stress-corrosion cracking.
