The project to compile this collection of case histories and to develop the original ASM Failure Analysis Center, launched in 2004, was overseen by the ASM Failure Analysis Committee.

ASM staff involved in the project (circa 2003 to 2005) included Scott D. Henry, William W. Scott, Jr., and Sally Fahrenholz-Mann. Editorial assistance was provided by Joseph R. Davis, J.R. Davis & Associates.

In 2019, the present volume was organized, the format of the electronic files was updated, and additional subject tagging was added. ASM staff involved in the 2019 project included Larry Berardinis, Scott D. Henry, Karen Marken, and Madrid Tramble.
